How Is Prostatitis Diagnosed

Your urologist will ask you questions about the symptoms you have and are currently experiencing and about your medical history. Please click here to complete a chronic prostatitis symptom score

You will then be examined, and this will include an examination of your tummy , testicles and a digital rectal examination to feel the prostate gland. A urine dipstick will be performed to see if there is any evidence of blood in the urine or of a urinary infection, and the specimen may then be sent off for a urine culture.

Your PSA blood test may be taken, although the value of this is uncertain.Your urologist may ask you to perform a flow test to see how fast you pass urine. You will need to have a comfortably full bladder to do this. It involves passing urine as usual, but into a funneled tube which collects the urine and can measure how much you pass per second. Following this you will have a bladder scan, which takes just a minute and involves passing an ultrasound probe over the lower part of your tummy to see how much urine you leave in your bladder. You may also need to undergo a cystoscopy to examine the prostate and bladder further. This test is completed in the hospital, and takes about ten minutes. A small telescope is inserted into the urethra after an anaesthetic gel is applied. This telescope allows the specialist to see your prostate and bladder, and see if there are any abnormalities or inflammation. to read our information leaflet about flexible cystoscopy

How Are Bacterial Forms Of Prostatitis Managed Or Treated

Antibiotics can kill bacteria that cause bacterial types of prostatitis. Men with acute bacterial prostatitis may need 14 to 30 days of antibiotics, starting with IV antibiotics in the hospital. Rarely, men need surgery to drain an abscess on the prostate.

Treating chronic bacterial prostatitis is challenging. You may need up to three months of antibiotics to sterilize the prostate. If the prostate cant be sterilized, low-dose antibiotics can be used long term to prevent recurrences. Some men need surgery to remove prostate stones or scar tissue in the urethra. Rarely, surgeons remove part or all of the prostate gland .

How Do Doctors Diagnose A Prostate Infection

Best Treatment For Enlarged Prostate

The diagnosis of both acute and chronic prostate infections begins with the history of the symptoms described above, the physical exam, including the digital exam of the prostate and often confirmed by culture and identification of the infecting organism.

Acute bacterial prostatitis diagnosis

  • Usually an enlarged, firm, and tender prostate is enough to make a presumptive diagnosis and start treatment .
  • Because there is usually a low level of bacteria in urine, the doctor will perform a urinalysis to quantitate and identify infecting bacteria by urine culture.
  • If the person has symptoms suggesting the infection has spread outside the prostate , blood cultures and other blood tests are likely to be ordered.
  • If a spread of the infection is suspected, a doctor will often perform an ultrasound to help confirm the diagnosis and to rule out an abscess. If this ultrasound is not available, the doctor may perform a CT scan or MRI of the pelvis.

Chronic bacterial prostatitis diagnosis

One of two tests are sometimes performed to help with the diagnosis:

The classic test is a Meares-Stamey 3-glass test. Three separate urine samples are collected and examined during this test. The last sample is taken after prostatic massage.

Premassage and postmassage test

Men with recurrent urinary tract infections should have ultrasound imaging of their upper urinary tract and a plain abdominal X-ray or an intravenous urography to exclude a possible structural problem or a kidney stone.

Living With Chronic Prostatitis

Long-term prostatitis can be a very difficult condition to live with. The pain or discomfort can make it difficult to carry out everyday tasks, and you might have no warning before having a flare-up.

Travelling long distances or sitting in meetings when you dont know when youll be able to reach a toilet can be worrying, especially if you need to go a lot. And it could be uncomfortable to sit for a long time.

If youre living with prostatitis its natural to feel frustrated. Some men feel that other people dont understand their symptoms, making them feel alone. Some men even find that living with prostatitis and its symptoms makes them feel depressed or anxious.

Feeling depressed or anxious could actually make your prostatitis symptoms worse. If you feel depressed or anxious, speak to your doctor or nurse. There are things that can help. Sometimes just talking to someone about the way you feel can make things feel better.

If your prostatitis symptoms dont improve with the treatment offered by your GP, ask them to refer you to a doctor who specialises in managing prostatitis. Your GP may also be able to refer you to a psychologist or counsellor, or you could join a support group. Talking to other people who understand what youre going through can be helpful.

Speak to your GP about making an action plan so that you know what to do when you have a flare-up. This will help to make sure you can get treatment quickly and get a referral to a specialist if you need it.

How Is Benign Prostatic Hyperplasia Treated

Patients who have mild symptoms may not require treatment other than continued observation to make sure their condition doesnt get worse. This approach is sometimes called watchful waiting or surveillance. There are a number of treatment options available if your symptoms are severe.

Medications

Finasteride and dutasteride work by decreasing the production of the hormone dihydrotestosterone , which affects the growth of the prostate gland. These appear to be most beneficial for men with larger prostates.

Drugs that relax the muscle in the prostate are more commonly used. These include terazosin , doxazosin , tamsulosin , alfuzosin , and silodosin . The most common side effects are lightheadedness, weakness and retrograde ejaculation.

Medications are sometimes combined to help treat symptoms and improve the flow of urine. One such medication is dutasteride and tamsulosin .

Surgery

Several different types of surgery can be used to remove the prostate tissue that blocks the flow of urine, including:

When To Seek Medical Care For Prostate Infections

Contact a doctor for any of the following symptoms. These symptoms are even more significant if accompanied with high fever and chills:

  • Urinary burning or pain
  • Difficulty or pain when starting urination
  • Pain in the genital area
  • Pain with ejaculation

Doctors usually diagnose and treat prostate infections on an outpatient basis. If a person develops high fever with chills or a new onset of difficulty in urinating, seek medical care immediately or go to a hospital’s emergency department because the infection may be spreading to other areas of the body.

What Are The Types Of Prostatitis

Types of prostatitis include:

  • Acute bacterial prostatitis : A UTI causes an infection in the prostate gland. Symptoms include fever and chills. You may experience painful and frequent urination or have trouble urinating. Acute bacterial prostatitis requires immediate medical treatment.
  • Chronic bacterial prostatitis : Bacteria become trapped in the prostate gland, causing recurrent UTIs that are difficult to treat.
  • Chronic pelvic pain syndrome, or CPPS : CPPS is the most common prostatitis type. Prostate gland inflammation occurs in approximately 1 out of 3 men. As the name implies, this type causes chronic pain in the pelvis, perineum and genitals.
  • Asymptomatic inflammatory prostatitis : This condition causes prostate gland inflammation but no symptoms. You may learn you have this condition after getting tests to find the cause of other problems. For example, a semen analysis for infertility may detect asymptomatic inflammatory prostatitis. This type doesnt need treatment.

When To Seek Medical Care

A person may have urinary symptoms unrelated to prostatitis that are caused by bladder problems, UTIs, or benign prostatic hyperplasia. Symptoms of prostatitis also can signal more serious conditions, including prostate cancer.

Men with symptoms of prostatitis should see a health care provider.

Men with the following symptoms should seek immediate medical care:

  • complete inability to urinate
  • great discomfort or pain in the lower abdomen and urinary tract

How Is Chronic Pelvic Pain Syndrome Managed Or Treated

Prostatitis treatments vary depending on the cause and type. Asymptomatic inflammatory prostatitis doesnt require treatment.

For chronic pelvic pain syndrome , your healthcare provider may use a system called UPOINT to classify symptoms into six categories. Your provider uses multiple treatments at the same time to treat only the symptoms youre experiencing.

Approximately 80% of men with CPPS improve with the UPOINT system. The system focuses on these symptoms and treatments:

  • Urinary: Medications, such as tamsulosin and alfuzosin , relax muscles around the prostate and bladder to improve urine flow.
  • Psychosocial: Stress management can help. Some men benefit from counseling or medications for anxiety, depression and catastrophizing .
  • Organ: Quercetin and bee pollen supplements may relieve a swollen, inflamed prostate gland.
  • Infection:Antibiotics kill infection-causing bacteria.
  • Neurologic: Prescription pain medicines, such as amitriptyline and gabapentin , relieve neurogenic pain. This pain can include fibromyalgia or pain that extends into the legs, arms or back.
  • Tenderness: Pelvic floor physical therapy may include myofascial release . This therapy can reduce or eliminate muscle spasms.

How to Cure an Inflamed Prostate – Natural Home Remedies for Prostatitis and Treatment

For men over 50, prostate problems are common. The prostate gland is located just below the bladder and surrounds the top portion of the urethra, which is the tube that drains urine from the bladder. The prostate’s primary function is to produce the fluid that nourishes and transports sperm.

As men age, the prostate gland tends to grow larger. Benign prostatic hyperplasia, also called prostate gland enlargement, can cause uncomfortable urinary symptoms, such as blocking the flow of urine out of the bladder, or bladder, urinary tract or kidney problems. Fortunately, a wide variety of treatments are available for enlarged prostate, including medication, minimally invasive therapies and surgery.

On the next Mayo Clinic Radio program, Dr. Mitchell Humphreys, a Mayo Clinic urologist, will highlight prostate health and treatment options for benign prostatic hyperplasia. Also on the program, Dr. Francisco Lopez-Jimenez, a Mayo Clinic cardiologist, will discuss sex after a heart attack. You may find the symptoms of an enlarged prostate unpleasantfrequent urination is among the most commonbut do they require treatment? It depends on the severity of your symptoms. Up to one-third of mild benign prostatic hyperplasia cases clear up without any treatment at all, according to the National Institute of Diabetes and Digestive and Kidney Diseases .

If your enlarged prostate symptoms are more severe, or they cause frequent urinary tract infections or other complications, work with a urologist for relief. These are some of your options to treat benign prostatic hyperplasia:

  • Use of an electrical current to vaporize tissue
  • Heating tissue directly with needles
  • Heating tissue indirectly with microwave energy
  • Using a laser or focused ultrasound to destroy tissue
  • Using a stent to keep the urethra open

Traditional surgical techniques include cutting away prostate tissue, making incisions in the bladder neck to relieve pressure in the gland, or completely removing the tissue .

Drug and surgical treatments for enlarged prostate come with potential side effects, including a small risk for erectile dysfunction. Be sure to discuss possible side effects with your healthcare provider as you weigh the benefits of different BPH treatments.

Why Can Stimulating The Prostate Feel So Good

The anus and its surrounding area are packed with nerve endings, Hong explains. Thats precisely why anal play can feel so amazing whether or not you have a prostate. Dr. Hong also notes that the prostate has plenty of blood vessels that swell with increased blood flow. The extra blood helps make the area more sensitive, which can boost pleasure.

Even though the anus is surrounded by nerve endings, its also surrounded by stigma. The human body is amazing in that parts like the vagina and anus have natural functions but can also be sites of intense pleasure. Trying out different sex-related acts is normal, and discovering new ways to experience pleasure is always a good idea.
