Comparative Data On The Efficacy Of Tadalafil And Tamsulosin

BPH Treatments | Ask a Prostate Expert, Mark Scholz, MD

Another important question that rises up is the efficacy of tadalafil compared with an established treatment for BPH-associated LUTS such as an -adrenergic antagonist. In this context, Oelke and colleagues , reported on a randomized, double-blind, international, placebo-controlled, parallel-group study including tamsulosin as an active control. A total of 511 men with BPH/LUTS were randomized to placebo , tadalafil 5 mg or tamsulosin 0.4 mg once daily for 12 weeks following a 4-week run-in period. This study was not designed for statistical testing of noninferiority or superiority between tadalafil and tamsulosin but it was adequately powered for the comparison of each active treatment with placebo. IPSS scores significantly improved versus placebo with tadalafil and tamsulosin . This improvement was evident as early as 1 week . BII significantly improved versus placebo . The IPSS QoL index and the Treatment Satisfaction Scale-BPH improved significantly versus placebo with tadalafil but not with tamsulosin . Surprisingly, given the data from previously published studies, Qmax increased significantly versus placebo with both tadalafil and tamsulosin . PVR decrease was higher with tamsulosin but statistical significance was not reached by either treatment versus placebo.

What If I Miss A Dose

If you forget to take your once-a-day dose of Cialis, take it as soon as you remember. However, never take two doses in 1 day. If you missed your dose yesterday, just skip the missed dose and continue with your once-a-day dosing.

To help make sure that you dont miss a dose, try setting a reminder on your phone. A medication timer may be useful, too.

Latest Men’s Health News

THURSDAY, Oct. 6 — The U.S. Food and Drug Administration announced late Thursday that it had approved using the erectile dysfunction drug Cialis as a treatment for enlarged prostate.

According to the U.S. National Institutes of Health, enlarged prostate — clinically known as benign prostatic hyperplasia — is a “common part of aging” for men. In fact, the NIH estimates that “more than half of men in their 60s, and as many as 90% in their 70s and 80s, have some symptoms of BPH.”

The condition often leads to urinary incontinence and can raise the odds for urinary tract infections and even kidney damage.

“BPH can have a big impact on a patient’s quality of life,” Scott Monroe, director of the division of reproductive and urologic products in the FDA’s Center for Drug Evaluation and Research, said in an agency news release. “A large number of older men have symptoms of BPH. Cialis offers these men another treatment option, particularly those who also have ED, which is also common in older men,” he said.

A third study, which involved men who suffered from both impotence and enlarged prostate, found that the daily 5-milligram dose of Cialis improved symptoms for both conditions.

Other drugs have already received FDA approval for the treatment of BPH. These include Proscar, , Avodart , Jalyn , and the alpha blockers Hytrin , Cardura , Flomax , Uroxatral and Rapaflo .

What You Need To Know About The Prostate Does Cialis Shrink Prostate

The main purpose of the prostate is to produce semen, a milky fluid that sperm swims in. During puberty, the body produces semen in a large number of cases, including enlarged prostate. This fluid causes the prostate to swell and cause a number of bladder-related symptoms. This is why the prostate is important to the body. It can be caused by many factors, including infection and inflammation.

A enlarged prostate can also cause blockages in the urethra. A blocked urethra can also damage the kidneys. A patient suffering from an enlargement of the prostate may have pain in his lower abdomen and genitals. If pain is present, a digital rectal examination will reveal hard areas. A doctor may prescribe surgery or perform an endoscopic procedure. If the enlarged prostate is not completely removed, it will shrink.

While the size of an enlarged prostate will influence the extent of urinary symptoms, men may experience a range of urinary symptoms. Some men have minimal or no symptoms at all. Some men will have a very enlarged prostate, whereas others will have a mild enlargement. Generally, the symptoms can stabilize over time. Some men may have an enlarged prostate but not notice it. If they have an enlarged colon, their physician can perform a TURP procedure.

Prostate Symptoms Better With Alcohol

Its one of those things that people including urologists nearly all believe. Men with an enlarged prostate, they say, shouldnt drink alcoholic beverages. Its supposed to make the symptoms of an enlarged prostate urinary flow, urinary irritation, and poor sexual function worse.

There isnt much scientific evidence for this, says Claus Georg Roehrborn, MD, of the University of Texas Southwest Medical Center at Dallas. So Roehrborn and colleagues analyzed data from 19,000 men enrolled in six different international clinical trials of treatments for enlarged prostate and prostate cancer.

Some of these over-45-year-old men said they didnt drink at all. Others said they had no more than two, three to six, or more than six drinks a week.

The patients who claim they drank more complained of less irritative and obstructive symptoms, Roehrborn said at a news conference. The more alcohol people said they were consuming, the better their urine flow, and the better their libido.

Does this mean a man with an enlarged prostate should take up drinking? No, Roehrborn says. But he argues that doctors should stop telling men their prostate symptoms will get better if they stop drinking.

The finding may put an end to this common knowledge advice, says Mark Moyad, MD, MPH, of the University of Michigan, who moderated the news conference at which Roehrborn spoke.

What Drink Should You Avoid For Prostate Health

An alcoholic drink should ideally be avoided when a man has problems with their prostate. Another important option to consider is caffeinated drinks. This includes coffee. The problem here is that alcohol and caffeine can irritate the bladder. This can lead to unpleasant effects.

A man may find that they need to urinate more frequently. The bladder irritation can also lead to complications. Such complications may also include a risk of inflammation within the bladder. This can further contribute to the urinary symptoms the man experience.

It is important to note that some teas also contain caffeine. The person should always check the label of a product they buy. This ensures they can avoid caffeine in their drinks. In turn, there is a reduced risk of irritating the bladder.

The primary function of this gland is to produce prostatic fluid. This is the fluid that helps the sperm survive after ejaculation.

Unfortunately, factors like aging and a poor lifestyle can make the prostate weak. This results in an impairment in urinary function, sexual function and prostate inflammation.

Cialis For Erectile Dysfunction

Cialis is FDA-approved to treat erectile dysfunction.

Sometimes ED can be helped by simply addressing the cause, such as treating an illness or anxiety. In other cases, you may need medication such as Cialis to treat your ED.

Effectiveness

Cialis was shown in clinical trials to be an effective medication to treat ED. The drug was studied using part of a survey called the Index of Erectile Function . A higher score on the IIEF meant that erectile function was better, so ED symptoms improved. The people in the study took the survey after 4 weeks.

One trial looked at people who took 20 mg of Cialis or a placebo only when needed to treat their ED.

  • People who took Cialis had their IIEF scores improve by 6.9 to 9.3 points. This means that their ED symptoms eased while they were taking Cialis.
  • People who took a placebo either lost points or improved by a maximum of only 0.3 points. Losing points on the scale means that their ED symptoms worsened.

In another trial, people took 2.5 mg or 5 mg of Cialis, or a placebo on an everyday basis to treat their ED.

One size does not fit all in benign prostatic hyperplasia treatment

Benign prostatic hyperplasia , or enlarged prostate, is a common condition in men older than age 50. BPH occurs when the prostate gland becomes enlarged and begins to press on the urethra the tube that carries urine out of the body.

When the prostate continues to grow, it can lead to urinary symptoms that include:

  • Leaking or dribbling of urine
  • Frequent nighttime awakenings to urinate
  • Urinary retention or urgency
  • A weak urine stream

BPH is not prostate cancer and it doesnt lead to cancer. However, the two conditions may have similar symptoms in the early stages

So its important that men who experience these symptoms be evaluated by a mens health specialist to rule out cancer and determine the best treatment options to relieve their symptoms.

Without treatment, BPH can lead to serious complications in some men, including incontinence, kidney or bladder damage, urinary tract infections and bladder stones. In some extreme cases, BPH can cause a total inability to urinate and require emergency catheterization.

Traditional Treatments For Bph

Until recently, treatments for BPH were limited to medications such as alpha blockers and 5-alpha-reductase inhibitors or surgical procedures such as transurethral resection of the prostate . In TURP, a surgical instrument is inserted into the tip of the penis and then used to trim and remove excess prostate tissue.

There are pros and cons to both treatment approaches.

Medications can help relieve symptoms and reduce the size of the prostate. However, the medications can have unpleasant side effects, including chronic stuffy nose, lightheadedness and loss of libido.

In comparison, TURP surgery is better at improving symptoms more quickly. But in some cases, a prostate is too large to be removed through a TURP procedure. TURP also carries a risk of causing new problems such as urinary incontinence, erectile dysfunction or retrograde ejaculation .
