Prostate cancer is the most common cancer after skin cancer in men in the US and the second leading cause of cancer death. Prostate-specific antigen testing has made the detection of prostate cancer easier in its early stages. Ninety-two out of 100 men get diagnosed when the cancer is limited to the prostate.
Most men are diagnosed with prostate cancer in their senior years and only 1 out of 36 men die from it. Death from prostate cancer most often happens when cancer has spread to other organs in the body. This is known as the advanced stage of prostate cancer.
The chances of survival decrease as cancer spreads beyond the prostate. If cancer has metastasized to other parts of the body, only three out of 10 men will survive for five years after the diagnosis.
Advanced stage prostate cancer or metastasized prostate cancer
Cancerous cells may spread to organs other than the site of origin. In the case of prostate cancer, this tendency is decreased, but it can happen. Advanced stage prostate cancer is defined based on the Gleason score, which is based on the TNM staging of cancer. In order to determine whether race interacted with survival, we repeated our initial analyses after stratifying by race and by modeling an interaction term. Among patients with M1 disease, conditional mortality was similar between white and non-white patients . However, among patients with N1 disease, non-white patients did not have a significant reduction in conditional mortality at 5, 10, or 15 years of survival compared to diagnosis , while white patients had similar mortality as the overall cohort, with adjusted hazard ratios of 1.10 , 0.898 , and 0.712 , respectively. While non-white patients with T4 disease had improved 5-year PCSM after having survived 5, 10, or 15 years, their improvements were 20-39 % smaller than those of white patients .

No matter where prostate cancer spreads, its still treated as prostate cancer. Its harder to treat when it reaches an advanced stage.
Treatment for advanced prostate cancer involves targeted and systemic therapies. Most men need a combination of treatments and they may have to be adjusted from time to time.
End Stage Of Prostate Cancer
Prostate cancer is a malignant tumor which generally affects men older than the age of 50. The tumor develops inside the gland located just below the bladder, called the prostate gland. The disease has its own specific course and goes through a distinct stages.
The rate and nature of the progression of prostate cancer is individual and the first symptoms occur when the tumor becomes large enough to initiate compression or infiltrate nearby organs and tissues. Prostate cancer can be also diagnosed “accidentally”, when the man has a medical checkup for unrelated reasons and the tumor is found over the course of this treatment.
Unfortunately, prostate cancer may remain asymptomatic for a long period of time. In such cases it is diagnosed late, when it has already resulted in bone metastases.

End stage prostate cancer also called the terminal stage of prostate cancer is the most advanced stage of the disease. The Final Days To Weeks Of Life
Providing excellent care toward the end of life requires an ability to anticipate when to focus mainly on palliation of symptoms and quality of life instead of disease treatment. Prognostic information plays an important role for making treatment decisions and planning for the EOL. However, the available literature suggests that medical providers inaccurately predict how long patients will live and tend to overestimate survival times. This appears to hold true even for providers who are experienced in treating patients who are terminally ill. In a multicenter cohort study of 230 hospitalized patients with advanced cancer, palliative care providers correctly predicted time to death for only 41% of patients. Survival time was overestimated in 85% of patients for whom medical providers gave inaccurate predictions, and providers were particularly likely to overestimate survival for Black and Latino patients.
Providers attempting to make prognostic determinations may attend to symptoms that may herald the EOL , or they may observe trends in patients functional status.
The investigators assigned patients to one of four states:

Clinical trials are investigational studies that are conducted to find new treatments in the fight against prostate cancer. Gleason Score Vs Grade Groups
The International Society of Urological Pathology released a revised prostate cancer grading system in 2014. The grade group system seeks to simplify Gleason scores and give a more accurate diagnosis.
One of the major problems with the Gleason score is that some scores can be made up in different ways. For example, a score of 7 can mean:
- 3 + 4. The 3 pattern is the most common in the biopsy and 4 is the second most common. This pattern is considered favorable intermediate risk.
- 4 + 3. The 4 pattern is the most common in the biopsy and 3 is the second most common. This pattern is considered unfavorable and may mean local or metastatic spread.
So, although both situations give a Gleason score of 7, they actually have very different prognoses.

End Stage Prostate Cancer: Clinical Characteristics

What Happens Without Treatment
Healthcare providers will sometimes talk about a particular diseases natural history or typical progression if it is left untreated indefinitely.
With regard to prostate cancer, most cases of the disease are discovered while the cancer is still confined to the prostate itself. This is called local disease or localized disease.
The disease is easiest to treat while it is confined to the prostate. At this stage, surgery and radiation are most likely to be curative and completely kill or remove whatever cancer cells are present.
If left untreated, however, prostate cancer can proceed on a number of different paths.

The Grade Group And Psa Level Are Used To Stage Prostate Cancer
The stage of the cancer is based on the results of the staging and diagnostic tests, including the prostate-specific antigen test and the Grade Group. The tissue samples removed during the biopsy are used to find out the Gleason score. The Gleason score ranges from 2 to 10 and describes how different the cancer cells look from normal cells under a microscope and how likely it is that the tumor will spread. The lower the number, the more cancer cells look like normal cells and are likely to grow and spread slowly.
The Grade Group depends on the Gleason score. See the General Information section for more information about the Gleason score.
- Grade Group 1 is a Gleason score of 6 or less.
- Grade Group 2 or 3 is a Gleason score of 7.
- Grade Group 4 is a Gleason score 8.
- Grade Group 5 is a Gleason score of 9 or 10.
The PSA test measures the level of PSA in the blood. PSA is a substance made by the prostate that may be found in an increased amount in the blood of men who have prostate cancer.
Staging And Grading For Stage 4 Cancer
Most cancers are staged using some form of the TNM system. Doctors may also use the TNM system to help determine the extent of certain cancers in each stage. The TNM system stands for:
- T , or the size of the original tumor
- N , or whether the cancer is present in the lymph nodes
- M , or whether the cancer has spread to other parts of the body
Not all cancers are staged using the TNM system, though. Some cancers, especially liquid cancers, are staged through different established protocols. The Binet and Rai systems, for example, are used to stage certain types of leukemia. Female reproductive system cancers, such as cervical cancer, are staged with a separate staging system created by the International Federation of Gynecology and Obstetrics .
As your care team gathers information about your cancer for the purposes of staging, they may need to order several tests, including:
Your care team may likely also need to perform a biopsy, a procedure that involves removing a sample of cells and analyzing it for signs of cancer. Imaging scans may be able to tell your care team where your cancer is, but looking at the cancer cells specifically tell them how fast they are likely to growor what grade they are.
Grading is different from staging and is done for most, but not all, cancers.
The grade of your cancer is part of how your cancer care team stages your cancer and determines your prognosis, or outlook.

How Would I Treat Stage 4 Prostate Cancer
- I would stop all the foods that cause cancer, and stop using all the household and bodycare products that leach toxins into the body.
- I would change to those foods that resonate with my body through personal testing as I describe in my book.
- I would only eat fresh organic foods, grains, nuts and seeds properly prepared and soaked to reduce the phytic acid cooked nourishing broths and meats from grass-fed and pastured animals and eat raw, unpasteurized dairy and eggs which have a completely different profile than conventional cancer-causing, factory-farmed and produced foods which are loaded with toxins.
- I would stop all harmful foods listed on this site and in my book. For end stage prostate cancer, I would stop all sugar and its substitutes. Why? They feed the cancer.
- I would undergo cleansing short fasts with freshly made vegetable juices if personally tested “Yes.” I would do a liver flush, or many, to rid the body of major toxins quickly and easily.
- I would ensure that if couldn’t get adequate sun on my skin, I would at least take an adequate dosage of Vitamin D3 levels.
- I would take aloe vera juice each day and test for which supplements my body needed like zinc and magnesium.

End stage prostate cancer could then no longer find a host in my body for I would have changed the conditions that I either knowingly or unknowingly created that led to it.
What Will Happen In The Last Weeks Days And Months
Every man will have a different experience at the end of his life.The burden of cancer on the body can cause a number of symptoms. Bone marrow may not be able to make enough red blood cells, which can cause anaemia. Cancer can affect your ability to get energy from food, which can make you feel weak. Sometimes organs, like the kidney and liver may not work so well, which can mean you get a build up of waste products in your blood. In the later stages of the disease you may feel drowsy and drift in and out of consciousness.However, its important to know that you may not experience all or any of these effects. What Can You Do About Appetite Changes
Its important to recognize changes in appetite so that you can get help when its needed. Talk to your medical team about how much youve been eating and whether you need to do something about it. For example:
- Some causes of poor appetite can be managed with medical treatment. There are medicines that can help stimulate your appetite, decrease nausea, and help food move through your stomach more quickly.
- A nutritionist might be able to offer tips on how to get the most out of each bite you take.
- Supplemental drinks or shakes can sometimes help you get needed nutrition more easily.
- You might find that youre able to eat more when others are at the table.
- You might find it easier to eat small frequent meals or snacks during the day instead of trying to eat full meals 3 times a day.
These measures may work for some, but they wont help most people who are very close to the end of life. At later stages, these efforts can even make the person feel worse.
