Metastatic Prostate Cancer To The Gastrointestinal Tract
Prostate cancer is the second most common cancer among men in the United States, with close to 200,000 new cases estimated in 2016 close to 26,000 deaths will be attributed to the disease.1 Approximately 1 out of every 7 men in the US will be diagnosed with prostate cancer in his life, though diagnoses become more common with advanced age.
Prostate cancer frequently metastasizes to the bones, lymph nodes, lungs, liver, and brain. Rarely, however, patients will develop metastases to the gastrointestinal tract , which limit treatment options. In a large post-mortem study, 2.5% of patients had evidence of prostate cancer to the stomach on autopsy.2
Another large retrospective analysis that reviewed close to 75,000 patients with metastatic prostate cancer showed that only 2.7% of those patients had metastases to the GIT .3 Interestingly, 1.2% of patients who had bone metastases developed metastases in the GIT vs 11% of patients without bone metastases.
Most literature regarding metastatic prostate cancer to the GIT is in the form of case reports, case series, or retrospective reviews. There are reports of prostate cancer metastasizing to the esophagus, stomach, small bowel and colon.4-7 The exact pathophysiology to explain how prostate cancer metastasizes to the GIT is not entirely known, though it most likely occurs through the bloodstream or lymphatic channels.

Drugs To Treat Cancer Spread To Bone

Medicines that can help strengthen the bones and lower the chance of fracture are bisphosphonates and denosumab. Sometimes, radiation, radiopharmaceuticals, or pain medicines are given for pain control.
Side effects of bone medicines

Cancer cells prevent your lymph nodes from draining fluid and working properly. When this happens, your lymph nodes swell. As a result, you might experience pain or soreness in the area.

What Is Colon Or Rectal Cancer
Cancer that starts in the colon or rectum is often called colorectal cancer. It might also be called colon cancer or rectal cancer. It starts when cells in the colon or rectum grow out of control and crowd out normal cells.

Active Surveillance And Watchful Waiting
If prostate cancer is in an early stage, is growing slowly, and treating the cancer would cause more problems than the disease itself, a doctor may recommend active surveillance or watchful waiting.
Active surveillance. Prostate cancer treatments may seriously affect a person’s quality of life. These treatments can cause side effects, such as erectile dysfunction, which is when someone is unable to get and maintain an erection, and incontinence, which is when a person cannot control their urine flow or bowel function. In addition, many prostate cancers grow slowly and cause no symptoms or problems. For this reason, many people may consider delaying cancer treatment rather than starting treatment right away. This is called active surveillance. During active surveillance, the cancer is closely monitored for signs that it is worsening. If the cancer is found to be worsening, treatment will begin.
ASCO encourages the following testing schedule for active surveillance:

What Treatments Are Available
If you have advanced prostate cancer, treatment wont cure your cancer. But it can help keep it under control and manage any symptoms.
If youve just been diagnosed with advanced prostate cancer, you may be offered the following treatments:
- chemotherapy with hormone therapy
- clinical trials.
Research has found that having radiotherapy together with one of the main treatments listed above can help some men with advanced prostate cancer to live longer. But radiotherapy isnt suitable for all men with advanced prostate cancer.
If you live in Scotland, you may also be offered a type of hormone therapy called abiraterone acetate together with standard hormone therapy. In the rest of the UK, abiraterone is currently only given to men with advanced prostate cancer that has stopped responding to other types of hormone therapy. The National Institute for Health and Care Excellence is currently deciding whether to make it available for men who have just been diagnosed with advanced prostate cancer.

Treatments For Prostate Cancer Spread To Bones
If prostate cancer spreads to other parts of the body, it nearly always goes to the bones first. Bone metastasis can be painful and can cause other problems, such as fractures , spinal cord compression , or high blood calcium levels, which can be dangerous or even life threatening.

A diagnosis of pancreatic or colon cancer often sparks dread about the disease’s likely next destination: the liver. That’s because liver metastasis is a leading cause of death in these patients.
A Cedars-Sinai scientific team has been awarded a $9.1 million grant by the National Cancer Institute to study this often-fatal process, with the goal of understanding how cancer spreads, or metastasizes, to the liver and finding ways to block it. The team will focus on the interplay between dietary fat and fatty liver disease which is commonly associated with obesity and the mechanisms that allow cancer to spread to the liver.
The liver is the second-most common site, after lymph nodes, to which cancer spreads. The majority of patients that die of pancreatic, colon or prostate cancer develop liver metastases at the time of death, said co-lead investigator Neil Bhowmick, PhD, director of the Cancer Biology Program at Cedars-Sinai. While the liver is the primary organ of metastasis from pancreatic and colon cancers, the relatively rare incidence of prostate cancer metastasis to the liver has the worst outcome. The liver whose primary function is to filter the blood coming from the digestive tract, detoxify chemicals, metabolize drugs and excrete bile plays a role in other cancers as well.
Most liver metastases originate in the colon, rectum, pancreas, stomach, esophagus, breast, lung and skin.

Distant Metastatic Prostate Cancer
Distant metastatic prostate cancer is more advanced, having spread to areas outside the pelvic region. The most common distant places where prostate cancer spreads include the:
- Bones
- Lungs
- Liver
Less commonly, prostate cancer may spread to the adrenal glands, kidneys, brain, pancreas or another organ. It is important to note that it is still considered prostate cancer when it spreads to other regions, as the cancer cells in distant areas are the same as the original cancer cells in the prostate gland, even if the prostate gland was previously removed. A biopsy may be recommended to confirm this.
Some Colorectal Tumors Are Born To Be Bad
To get a better handle on the timing of metastasis, the team developed a computer model to simulate the evolution of millions of virtual tumors under different conditions.
We could then compare the simulated data with the observed patient data using various statistical techniques, Dr. Curtis explained.
The modeling data indicated that metastasis begins very early during tumor developmentwhen the primary tumor is smaller than the tip of a sharpened pencil.
In other words, Dr. Curtis said, some tumors may be “born to be bad.
The team next investigated whether the results from their small set of paired samples of primary and metastatic tumors from patients could be extended to a larger number of patients.
They analyzed publicly available data from 938 people with metastatic colorectal cancer and 1,813 people with early-stage, nonmetastatic colorectal cancer whose medical histories were known. The primary tumors of these patients had been profiled to identify genetic changes in known cancer-associated genes.
This analysis showed that certain genetic changes tend to occur together, and that specific combinations of three to five mutations were more common in patients with metastatic cancer than in those with nonmetastatic disease, suggesting that specific combinations of early genetic changes confer the potential for tumor cells to spread.
If confirmed in future studies, this information could potentially be used to guide treatment decisions, she said.
