Blood In The Urine For Two Weeks After Prostate Biopsy

  • #1 08-23-2018 08:28 AM by efangI did a prostate biopsy on August 9. It has been for two weeks , but there is still dark blood in my urine. While I start to pee, the dark blood will come out but the amount is pretty small, but I wonder why it is not every time urine has blood, I think if there are one or several wounds that are not growing well, the bleeding should be happened at each urine ?. My feeling is that if there is a lot of urine , there will be almost no blood. If the amount of urine is small, but I still want to pee, it is likely to have blood, so I try to avoid this kind of pee. When I asked my Urologist , he said that it would be fine, just take time, I don’t know what I can do, if blooding continues for one or several months , is there any medicine can help ? Why doesn’t online information mention about using medication, just said it will be cured if takes a long time.Thanks.

What Does A Prostate Biopsy Involve

If you decide to have a biopsy, youll either be given an appointment to come back to the hospital at a later date or offered the biopsy straight away.

Before the biopsy you should tell your doctor or nurse if youre taking any medicines, particularly antibiotics or medicines that thin the blood.

You may be given some antibiotics to take before your biopsy, either as tablets or an injection, to help prevent infection. You might also be given some antibiotic tablets to take at home after your biopsy. Its important to take them all so that they work properly.

A doctor, nurse or radiologist will do the biopsy. There are two main types of biopsy:

  • a trans-rectal ultrasound guided biopsy, where the needle goes through the wall of the back passage
  • a transperineal biopsy, where the needle goes through the skin between the testicles and the back passage .

Another type of prostate issue is chronic prostatitis, or chronic pelvic pain syndrome. This condition causes pain in the lower back and groin area, and may cause urinary retention. Symptoms include leaking and discomfort. In severe cases, a catheter may be required to relieve the symptoms. If the problem is unresponsive to other treatments, your doctor may suggest a surgical procedure. If these do not work, your symptoms could progress and become chronic.

An acute bacterial infection can cause a burning sensation. Inflammation of the prostate can affect the bladder and result in discomfort and other symptoms. This is the most common urinary tract problem in men under 50, and the third most common in men over 65. The symptoms of acute bacterial prostatitis are similar to those of CPPS. Post Prostate Biopsy Symptoms

I couldn’t find an existing thread on this topic, but I wanted to share what I’m seeing so far, and compare with other’s results.

I’m 58 years old. After discovering a 5+ PSA result in a routine adult physical, I was scheduled for and received both a MRI, and a Transperineal biopsy. In my case, they took 16 samples. I do not yet have the pathology results.

I was not prepared for what happened though. Four days after the biopsy, and to my horror, I found my expectations were way underestimated. It was not “streaks or spots” of blood, nor was it red-tinted. It looked to be pure blood. The volume was probably double what I would have expected. Hoping that the next day would be more normal, I again tested the waters.. It was nearly as bad, however, the color seemed to be a cross between pure blood and fresh motor oil.

I’ve got a message into my care team, and I’m awaiting a response while I also await pathology. I’m a bit nervous.

Has this been other’s experience? If it is common, why understate it in the publications I was provided?

I have not noticed blood in my urine, however, it tends to be dark-ish as I tend to under-hydrate.

What Happens After A Prostate Ultrasound And Biopsy

When the procedure is finished, you may resume your normal meals and daily activities, unless otherwise instructed. Some urologists may prescribe an antibiotic after the biopsy to prevent any infections, but given antibiotics only prior to the biopsy. Why Is A Prostate Biopsy Done

A prostate biopsy is done to screen for prostate cancers. This is to confirm whether cancer is present, or if a prostate cancer diagnosis is aggressive. In a prostate biopsy, small samples of the prostate are removed and then observed under the microscope. Doctors usually recommend a biopsy of the prostate gland based on certain findings.

  • If prostate-specific antigen blood test results are higher than the average age
  • If the doctor detects signs of a prostate problem during a digital rectal exam
  • If a biopsy is the only method to confirm the cancer

A prostate biopsy involves:

  • Collecting minute samples of the prostate gland. A computed tomography or magnetic resonance imaging scan is also used to guide them through the procedure.
  • A prostate biopsy takes about 10 minutes and is usually done in the doctors office. The samples will be sent to a lab and will be looked at under a microscope to see if they contain cancer cells.
  • If cancer is detected in the patient, it will also be assigned a grade. The results are available after one to three days, but it can sometimes take longer.

However, recent reviews of the evidence for using saw palmetto as a treatment for mild or moderate urinary symptoms did not show any improvement, compared to no treatment, in men with BPH.

How Long Do Prostate Biopsy Results Take

Generally, you can expect to receive your prostate biopsy results in 1-3 days. After that, pathologists would have a look at the tissue under the microscope, and in the report, they will define the kind of cells they could see.

Additionally, doctors would also describe the cancer score. For this, they use the Gleason scoring system that ranges from 2 to 10.

There is a complex way of calculating this score, and doctors do not use a score of 5 or below. It means that a score of 5 or below means that cancer could not be confirmed. 6 means it is starting to appear, and 7 means already some early sign.

Scores of 8,9,10 are almost confirmatory of cancer, indicating that cells are well differentiated from normal healthy cells.

How To Get The Best Results

Most men do not find prostate biopsy excessively painful or uncomfortable, and the complications are usually not seriousbut can be. Certain steps taken before, during, and after the procedure can improve the outcome:

Take antibiotics. Taking preventive antibioticsbefore and after the procedurecuts the risk of infection substantially. Most infections are not dangerous but could become so if they get out of control. The overall chance of being hospitalized with an infection after prostate biopsy is 1% to 3%.

Review medications. Before the biopsy, your doctor may advise you to stop taking daily low-dose aspirin or an anticoagulant such as warfarin , dabigatran , edoxaban , rivaroxaban , or apixaban . These drugs reduce the blood’s ability to clot. Your doctor will weigh the chance of bleeding against the need for anticoagulants to prevent heart problems or stroke.

Expect anesthesia. Get local anesthesia for the biopsy. This means an injection of a numbing drug into the prostate gland to reduce pain during the biopsy.

Further Treatments To Control The Cancer

Your first treatment may help keep your cancer under control. But over time, the cancer may change and start to grow again. If this happens you might be offered other treatments, including:

  • more hormone therapy
  • clinical trials

More hormone therapy

Your prostate cancer may respond well to other types of hormone therapy, such as abiraterone , enzalutamide , steroids or oestrogens, or to a combination of treatments.

More chemotherapy

If youve had hormone therapy on its own as a first treatment, you might be offered a chemotherapy drug called docetaxel . This may help some men to live longer, and can help to improve and delay symptoms. If youve already had docetaxel, you might be offered more docetaxel or another chemotherapy drug called cabazitaxel .

Radium-223

This is a type of internal radiotherapy that may be an option if your cancer has spread to your bones and is causing pain. A radioactive liquid is injected into your arm and collects in bones that have been damaged by the cancer. It kills cancer cells in the bones and helps some men to live longer. Are There Any Problems To Expect After A Prostate Ultrasound And Biopsy

In some cases, men can develop a urinary tract infection or an infection in the prostate. These infections are rare and easy to treat with prescribed antibiotics.

Some men may also have trouble urinating after the procedure. Most problems are minor and go away on their own after a few days. If you arent able to urinate at all, call your healthcare provider or go to the emergency room. Make sure that you tell them that you just had the ultrasound and biopsy.

  • A fever of 100 degrees F or higher.
  • Shaking or chills.

Day After The Prostate Biopsy

After your biopsy, you will have to drink many fluids so that you may provide us with two separate urine samples from two separate voids. The most common complication is blood in the urine after the biopsy. We would like to make sure that your urine is in the process of clearing before we send you home for the day. It is normal to see blood in the urine and bowel movements for 1-2 weeks after the procedure. It should progressively diminish over that time-period. If you find that, you cannot urinate or if you are passing a large number of blood clots, then report to the office or nearest emergency room for evaluation. A physician may have to insert a catheter into the bladder for a few days to allow you urine to be drained. You should continue to drink more than your usual intake of fluids to keep your urine diluted and to prevent formation of blood clots within the bladder.

If you have fever or chills, please take your temperature with an oral thermometer and call your physician if it reads above 100.8 F.

Follow-up Your results will usually take 5-10 working days. If you have not heard from your physician after 14 days please call the office at 558-9091.

What Are The Symptoms Of Prostate Problems

The symptoms of prostate problems may include

Most of these problems are more likely to be caused by something other than prostate cancer. For example, trouble urinating is much more often caused by benign prostatic hyperplasia , a non-cancerous growth of the prostate. Still, its important to tell your health care provider if you have any of these symptoms so that the cause can be found and treated, if needed. Some men might need more tests to check for prostate cancer.
