In most cases, prostate cancer is silent. Due to the location of the prostate gland deep within the groin, a growing tumor would not be close enough to painfully press on any nearby organs or tissues. With that said, some men experience:

  • Burning sensations during urination
  • Difficulty starting and stopping urination
  • A weak urine stream
  • More frequent urination, especially at night
  • Bloody urine
  • Pain during ejaculation
  • Bloody semen

More often, the cause of these symptoms is unrelated to prostate cancer, so its important to discuss them with a physician before drawing any conclusions. As part of the diagnostic process, a physician may perform a digital rectum exam to manually check the prostate and order a prostate-specific antigen test to measure the level of PSA in the blood.

Although an elevated PSA level may be a sign of prostate cancer, it could also indicate a noncancerous condition, such as prostate inflammation or enlargement . Therefore, if a PSA test reveals an elevated PSA level, a physician will typically order a biopsy to confirm or rule out a diagnosis of prostate cancer.

Signs Of Prostate Cancer Include A Weak Flow Of Urine Or Frequent Urination

These and other signs and symptoms may be caused by prostate cancer or by other conditions. Check with your doctor if you have any of the following:

  • Weak or interrupted flow of urine.
  • Sudden urge to urinate.
  • Frequent urination .
  • Trouble starting the flow of urine.
  • Trouble emptying the bladder completely.
  • Pain or burning while urinating.
  • Blood in the urine or semen.
  • A pain in the back, hips, orpelvis that doesn’t go away.
  • Shortness of breath, feeling very tired, fast heartbeat, dizziness, or pale skin caused by anemia.

Other conditions may cause the same symptoms. As men age, the prostate may get bigger and block the urethra or bladder. This may cause trouble urinating or sexual problems. The condition is called benign prostatic hyperplasia , and although it is not cancer, surgery may be needed. The symptoms of benign prostatic hyperplasia or of other problems in the prostate may be like symptoms of prostate cancer.

What Are The Types Of Prostate Cancer

The most common type of prostate cancer, diagnosed in nearly all cases, is prostatic adenocarcinomas. All other prostate cancer types comprise an estimated 1-5% of cases. Prostatic adenocarcinoma develops in the glandular epithelial cells that line the prostate and can be categorized into two subtypes:

  • Acinar adenocarcinoma Most prostate cancers are diagnosed as acinar adenocarcinomas. It produces elevated PSA levels and, since the cells grow in the back of the prostate near the rectum, the cancerous cells can sometimes be felt during a digital rectal exam, making this subtype easier to detect.
  • Prostatic ductal adenocarcinoma A rarer, more aggressive form of prostatic adenocarcinoma, this subtype develops in the cells lining the tubes and ducts of the prostate gland. Its harder to detect and often develops alongside acinar adenocarcinoma.

Other, less common types of prostate cancer include urothelial cancer, neuroendocrine tumors, small cell carcinoma, squamous cell carcinoma and soft-tissue prostate cancer.

Treatment Of Prostate Cancer Stage 4

High-Risk Prostate Cancer Treatment – MUSC Hollings

With prostate cancer stage 4, the main type of treatment both for the primary tumor and its metastases is hormone therapy combined with radiation therapy and radiosurgery.

Hormone therapy also plays an important role in the treatment of prostate cancer stage 4, blocking the effects of testosterone, which is responsible for the rate of prostate cells division in prostate cancer stage 4. Depending on the indications, hormone therapy is used to suppress the production of the testosterone in the patient’s body, or to block its effects on prostate tissue.

Surgery for the treatment of prostate cancer stage 4 is used as a symptomatic therapy. The primary surgical technique of improving quality of life is minimally invasive transurethral resection of the prostate, which facilitates normal urine flow.

Transurethral resection implies inserting a special surgical instrument into the lumen of the urethra and creating a “pathway” for the destroyed tumor cells to leave. If the outflow of urine is obstructed but there is no tumor infiltration, a patient with prostate cancer stage 4 may undergo stenting of the urethra.

A Biopsy Is Done To Diagnose Prostate Cancer And Find Out The Grade Of The Cancer

A transrectal biopsy is used to diagnose prostate cancer. A transrectal biopsy is the removal of tissue from the prostate by inserting a thin needle through the rectum and into the prostate. This procedure may be done using transrectal ultrasound or transrectal MRI to help guide where samples of tissue are taken from. A pathologist views the tissue under a microscope to look for cancer cells.

Sometimes a biopsy is done using a sample of tissue that was removed during a transurethral resection of the prostate to treat benign prostatic hyperplasia.

If cancer is found, the pathologist will give the cancer a grade. The grade of the cancer describes how abnormal the cancer cells look under a microscope and how quickly the cancer is likely to grow and spread. The grade of the cancer is called the Gleason score.

To give the cancer a grade, the pathologist checks the prostate tissue samples to see how much the tumor tissue is like the normal prostate tissue and to find the two main cell patterns. The primary pattern describes the most common tissue pattern, and the secondary pattern describes the next most common pattern. Each pattern is given a grade from 3 to 5, with grade 3 looking the most like normal prostate tissue and grade 5 looking the most abnormal. The two grades are then added to get a Gleason score.

Most men begin to suspect they have prostate cancer after obtaining an elevated PSA level or abnormal digital rectal examination. The PSA blood test measures the level of prostate-specific antigen in the blood.
