Most cancers are graded from stage 1 to 4 in level of severity, but prostate cancer uses a different system called the Gleason score. In the Gleason system, prostate cancer is graded using numbers from 1 to 5. Often, prostate cancer has sections with different grades, so those grades are added together to calculate the Gleason score. The highest number on the Gleason score is 10. Your Gleason score helps determine what treatment options are possible.

Treatment Of Prostate Cancer Stage 4

With prostate cancer stage 4, the main type of treatment both for the primary tumor and its metastases is hormone therapy combined with radiation therapy and radiosurgery.

Hormone therapy also plays an important role in the treatment of prostate cancer stage 4, blocking the effects of testosterone, which is responsible for the rate of prostate cells division in prostate cancer stage 4. Depending on the indications, hormone therapy is used to suppress the production of the testosterone in the patients body, or to block its effects on prostate tissue.

Surgery for the treatment of prostate cancer stage 4 is used as a symptomatic therapy. The primary surgical technique of improving quality of life is minimally invasive transurethral resection of the prostate, which facilitates normal urine flow.

Transurethral resection implies inserting a special surgical instrument into the lumen of the urethra and creating a pathway for the destroyed tumor cells to leave. If the outflow of urine is obstructed but there is no tumor infiltration, a patient with prostate cancer stage 4 may undergo stenting of the urethra.

Active surveillance, radiation therapy, and surgery all have advantages and disadvantages .13,2427 A randomized controlled trial of 1,643 men in Great Britain compared active surveillance, radical prostatectomy, and external beam radiation therapy for treatment of clinically localized prostate cancer over a median of 10 years.24 There were 17 prostate cancerspecific deaths overall: eight in the active-surveillance group , five in the radical prostatectomy group , and four in the EBRT group . There were no significant differences among groups in prostate cancerspecific mortality or all-cause mortality. Surgery and radiation therapy were associated with lower incidences of disease progression than active surveillance. No trials have compared treatment outcomes by race or ethnicity.

Eight Types Of Standard Treatment Are Used:

Watchful waiting or active surveillance

Watchful waiting and active surveillance are treatments used for older men who do not have signs or symptoms or have other medical conditions and for men whose prostate cancer is found during a screening test.

Watchful waiting is closely monitoring a patients condition without giving any treatment until signs or symptoms appear or change. Treatment is given to relieve symptoms and improve quality of life.

Active surveillance is closely following a patient’s condition without giving any treatment unless there are changes in test results. It is used to find early signs that the condition is getting worse. In active surveillance, patients are given certain exams and tests, including digital rectal exam, PSA test, transrectal ultrasound, and transrectal needle biopsy, to check if the cancer is growing. When the cancer begins to grow, treatment is given to cure the cancer.

Other terms that are used to describe not giving treatment to cure prostate cancer right after diagnosis are observation, watch and wait, and expectant management.

Causes Of Prostate Cancer

Prostate cancer is the most common form of male reproductive organ cancer. There are two types of prostate cancers, glandular and non-glandular.

Glandular tumours are more aggressive, spread faster than other forms, and require surgery to remove them completely.

Non-Glandular prostate tumours grow slower but can still be a concern if theyre not monitored extensively by your physician or specialist. Different types of treatment are available for prostate cancer. You and your doctor will decide which treatment is right for you. Some common treatments are

  • Expectant management. If your doctor thinks your prostate cancer is unlikely to grow quickly, he or she may recommend that you dont treat the cancer right away. Instead, you can choose to wait and see if you get symptoms in one of two ways:
  • Active surveillance. Closely monitoring the prostate cancer by performing prostate specific antigen tests and prostate biopsies regularly, and treating the cancer only if it grows or causes symptoms.

    Who Remained Prostate Cancer Free

    Intermediate Risk patients experience a wider range of results and approaches due to the risk of disease beyond the prostate. When selecting a course of action, consultations are recommended with a urologist or surgeon, radiation oncologist and medical oncologist. Different treatments have different results and side effects. It is important to understand the potential impact each treatment can have on your quality of life after treatment.

    Intermediate Risk patients are at a higher risk for cancer relapse or recurrence than Low Risk patients. That means patients in this risk group are more likely to have their cancer return following initial treatment, which may require additional treatment.
