Many prostate cancer patients dont need surgery. But for young patients with prostate cancer that hasnt spread, a surgery called a prostatectomy can help them become cancer-free and put prostate cancer treatment behind them. John Davis, M.D., a urologist who performs hundreds of prostatectomies each year, explains what patients undergoing this procedure can expect, including prostatectomy side effects and risks.
What is a prostatectomy?
A prostatectomy is the surgical removal of all or part of the prostate gland. Prostate cancer surgery types include:
- Robotic prostate cancer surgery: In this procedure, called robotic radical prostatectomy, a surgeon makes several small incisions in the lower abdomen and uses a robotic device to remove the prostate.
- Open radical prostatectomy: The surgeon makes one large incision in the lower abdomen to remove the prostate.
Robotic radical prostatectomies have become increasingly common over the years, and most surgeons prefer to conduct the procedure this way because its a little easier on them. But its important to know that both methods are safe.
Who needs a prostatectomy?
For some patients, prostatectomies will be the only treatment they undergo. Others who may have a difficult time recovering from surgery or have more advanced cancer may also have chemotherapy or hormone therapy treatment as well.

Will I Be Incontinent
Some patients have full urinary control as soon as the catheter is removed. Most patients experience some urinary leakage, which clears up in an average of three weeks but can take as long as 18 months. Small incontinence pads may be worn during this time to absorb any leaking urine and protect your clothing. For one out of 20 men, leakage may persist longer or even be permanent.
We define incontinence as needing more than two pads a day, says Dr. Mohler. Four out of five men wont need any pads at all. One in five will need one to two mini pads per day. One in 100 will end up needing more than two pads per day. This has a devastating effect on quality of life for these men. We dont fully understand why this happens, and we cannot predict who will have this experience. Strengthening the pelvic floor muscles with Kegel exercises before and after your operation may help you avoid or recover from problems with urinary control.

What To Expect After Radical Prostatectomy
Most men stay in the hospital for one to three days after radical prostatectomy. A urinary catheter is inserted during the surgery, and some men may need to wear the catheter home for a few days to a few weeks. Another catheter inserted through the skin also may need to stay in place for a few days after returning home.
Pain after radical prostatectomy can generally be controlled with prescription pain medicines. It can take weeks or months for urinary and sexual function to return to their maximum levels.
After radical prostatectomy, regular follow-up is essential to make sure prostate cancer does not return.

Types Of Radical Prostatectomy
The prostate gland lies just under the bladder, in front of the rectum. Surgeons choose from two different approaches to reach and remove the prostate during a radical prostatectomy. One is a traditional approach known as open prostatectomy. The other, more recent approach is minimally invasive. There are two minimally invasive procedures used in radical prostatectomy: laparoscopic prostatectomy and robot-assisted laparoscopic prostatectomy.
Open prostatectomy
In this traditional method of radical prostatectomy, the surgeon makes a vertical 8- to 10-inch incision below the belly button. Radical prostatectomy is performed through this incision. In rare cases, the incision is made in the perineum, the space between the scrotum and .
Laparoscopic prostatectomy

Small incisions are made in the belly, as in regular laparoscopic prostatectomy. A surgeon controls an advanced robotic system of surgical tools from outside the body. A high-tech interface lets the surgeon use natural wrist movements and a 3-D screen during radical prostatectomy.

Immediately After A Prostatectomy
- You will stay in hospital for two to five days.
- Nurses will monitor your vital signs.
- Your pain will be managed with medication.
- You may be given antibiotics to reduce the risk of infection.
- You may have a drip inserted into your arm or hand for a few days.
- You will most likely have a drain tube out of your abdomen that will be removed in the first day or two after the surgery.
- In most cases, you will have to go home still wearing the catheter. You will be taught how to care for it.
Urination And Incontinence In The Postoperative Period
People who have undergone a prostatectomy will be taught how to operate the catheter before they leave hospital after the operation. The operation of most catheters is relatively similar and involves collecting the urine in a drainage bag, which can then be emptied into the lavatory. The drainage bag must be changed at appropriate intervals.
All men normally experience some level of incontinence after the catheter has been removed, particularly after involuntary bodily actions such as sneezing or laughing. It is also common to experience sudden impulses to urinate while the catheter is in place. This is called a bladder spasm. Bladder spasms should ideally not be particularly painful or occur very often. Therefore, it is wise to consult the doctor if they occur frequently enough to be bothersome or are accompanied by acute pain.
For most men, urinary problems are temporary and are effectively resolved by practicing pelvic floor and Kegel exercises regularly, as recommended by the doctor. Pads should be worn whilst urinary leakage is occurring and changed throughout the day as needed.

How To Treat Erectile Dysfunction After Prostate Surgery

During Your Radiation Treatments
When its time for your treatment, your radiation therapists will bring you to the treatment room and help you onto the treatment table. Youll be positioned exactly how you were during your simulation. Your radiation therapists will do everything they can to make sure youre comfortable during your treatment. Then, theyll leave the room and start your treatment.
Breathe normally during your treatment, but dont move. Your radiation therapists will be able to see you on a monitor and hear you through an intercom during your whole treatment. Tell them if youre uncomfortable or need help.
Neither you nor your clothes will be radioactive during or after treatment. Its safe for you to be around other people.

Does Incontinence Go Away On Its Own After Prostate Surgery
For most men, urinary incontinence will go away within about 1 year. The time frame varies depending on how severe your incontinence is, and what types of treatments youre using to manage it. Some men may recover from leaks within weeks, whereas others will take months for the bladder leaks to subside. Performing pelvic floor exercises, also known as Kegels, which help strengthen the muscles that are located in the base of the pelvis between the pubic bone may help to speed the recovery process along.
How Fast Will I Become Totally Continent After Prostate Surgery
During surgery for prostate cancer, the urethra is also affected and, depending on the experience of the surgeon, more or less of the continence function will be preserved. Due to the high precision of robotic surgery, the patient will have a catheter for roughly one week and the trauma to the urinary function will be minimum.
The discomfort that patients feel during this time is minimum, but you should expect the removal to make you feel uneasy. The catheter will be removed during a visit to your doctor, so do not try to do this at home, as it could cause infections. The degree of which the urinary function will be affected depends on how normal the function was before surgery, age and weight.
It is worth noting that most men will experience some degree of incontinence after prostate surgery, but control can be regained within several weeks or months to a year.

Caring For The Incision
You will be able to take a shower the second day after your surgery. You may continue to have some discharge at the drain site for three to five days. Once you leave the hospital, the key words on caring for the drain site and incisions are clean and dry. Showering once a day and gently patting the area with a clean towel should be sufficient.

Things To Do After Prostate Surgery
After prostate surgery, you will not be allowed to stand up, as a requirement of postoperative care. During this period, a washing solution in the form of serum will be applied to your bladder and it will be continuously drained through a catheter. This solution is intended to prevent coagulation of minor bleeding that can develop after surgery, and keep the urinary tract open.
Your doctor may end this procedure the next day or the same night, depending on his/her preferences. This process will be ended once you stand up and walk. From that moment on, the water you drink and the urine you pass will take over the task of this washing solution, and will protect you from coagulation that can occur after surgery.
Diet Fluids And Bowel Movements
Constipation is a common side effect of pain medications and surgery. You should have received prescriptions for an oral stool softener and a laxative.
You should start drinking fluids as soon as you are comfortable after surgery, and you can resume your normal diet the first day after surgery. But while waiting for normal bowel function to return, you should avoid large meals in favor of several small meals a day. To prevent constipation, we recommend drinking at least eight to 10 glasses of fluids each day and eating lots of fruits and vegetables. Avoid carbonated beverages and cruciferous vegetables such as broccoli, cauliflower, brussels sprouts and cabbage for approximately two weeks, as they frequently cause gassy discomfort and distention.
Take your stool softener and laxative as prescribed. Normal trajectory for return of bowel function is one to two days to pass gas, three to five days for the first bowel movement.
If you haven’t had a bowel movement by day three after your surgery, take oral Miralax , an over-the-counter laxative. You can combine Miralax with the prescribed stool softener and laxative. Follow the instructions on the box. Do not use any enemas or take stronger laxatives, such as magnesium citrate. Contact the clinic if you still haven’t had a bowel movement by day five.

Urinary Problems After Surgery
Leaking urine
Most men cant control their bladder properly when their catheter is first removed. This is because surgery can damage the muscles and nerves that control when you urinate.You might just leak a few drops if you exercise, cough or sneeze . Or you might leak more and need to wear absorbent pads, especially in the weeks after your surgery.Leaking urine usually improves with time. Most men start to see an improvement one to six months after surgery. Some men leak urine for a year or more and others never fully recover, but there are things that can help and ways you can manage it.
Difficulty urinating
A few men may find it difficult to urinate after surgery . This can be caused by scarring around the opening of the bladder or the urethra .Some men find they suddenly and painfully cant urinate. This is called acute urine retention and it needs treating quickly to prevent further problems. If this happens, call your doctor or nurse, or go to your nearest accident and emergency department.

Sexual problems after surgery
Vitamins And Dietary Supplements
Its OK to take a multivitamin during your radiation therapy. Do not take more than the recommended daily allowance of any vitamin or mineral.
Dont take any other dietary supplements without talking with a member of your care team. Vitamins, minerals, and herbal or botanical supplements are examples of dietary supplements.

Radical Prostatectomy Perineal Approach
Youll be placed in a supine position in which the hips and knees will be fully bent with the legs spread apart and elevated with the feet resting on straps. Stirrups will be placed under your legs for support.
An upside-down, U-shaped incision will be made in the perineal area .
The doctor will try to minimize any trauma to the nerve bundles in the prostate area.
