Only Facility In The World With Same Day Diagnostics

UH Seidman Cancer Center is the only facility in the world to offer same day diagnostics, including prostate cancer screening, biopsy and plan of care. Our patients with prostate cancer symptoms can receive an accurate diagnosis and plan on the same day, eliminating the uncertainty that sometimes lasts for weeks.

We use our state-of-the-art MRI and patented MRI fingerprinting technology along with advanced image analytics and the support of biomedical engineering to reduce unnecessary biopsies and treatments for low-grade prostate cancer.

Our prostate cancer specialists are also among the first in the country to offer in-gantry function, or within the circular components of the MRI machine, for MRI-guided prostate biopsy. This advanced technique reveals the specific characteristics that distinguish between normal and diseased tissue.

We also fuse an MRI to an ultrasound to create an MRI ultrasound fusion biopsy. The MRI ultrasound fusion biopsy allows for more targeted biopsies and only two to four biopsy samples are needed rather than the 12 samples necessary with the standard technique.

Chemotherapy For Prostate Cancer

Surgery for Prostate Cancer

Chemotherapy is a treatment method of choice for many types of oncology. However, the effectiveness of chemotherapy for prostate cancer is rather poor.

Prostate cancer is most sensitive to hormonal medications because it is the male hormones that stimulate atypical cellular changes and malignant growth. Although, in some cases, the positive result is seen with several chemotherapy courses after surgical intervention.

Usually, chemotherapy for prostate cancer is indicated in cases of:

  • Extensive spreading of the malignant process
  • Resistance of prostate cancer to hormonal therapy
  • Development of the postoperative recurrence

Administration of chemotherapeutic agents is not a way to cure prostate cancer, but it can inhibit malignant growth and contribute to a significant reduction in the size of the tumor.

What Are The Different Types Of Prostate Cancer Surgery

The most common one is called a radical prostatectomy. In this procedure, the entire prostate gland is removed, along with some of the surrounding tissue. In some cases, nearby lymph nodes are removed as well. There are several surgical approaches for this, including traditional open surgery. We can also use minimally invasive procedures, such as laparoscopic surgery and robot-assisted laparoscopic surgery.

A critical aspect of a radical prostatectomy is tailoring it to the individual features of each mans cancer. One size does not fit all. This means the exact same procedure is not appropriate for every person. The location, size, and other features of the cancer are considered to design an operation that is appropriate for the person.

In a laparoscopic radical prostatectomy, a surgeon inserts a tiny camera called a laparoscope through a small cut in the abdomen. The camera gives doctors a magnified, high-definition picture of the prostate gland. With that image as a guide, the surgeon can remove the prostate, seminal vesicles, and lymph nodes using special tools.

To do a robot-assisted procedure, a surgeon sits at a console that has a screen as well as hand, finger, and foot controls. The surgeons hand, wrist, finger, and foot movements control the robotic instruments inside the patient in real time.

Some Side Effects Of Radical Prostatectomy Reduced If Surgeon Performs Many Procedures

The risk of some side effects caused by a radical prostatectomy appear to be reduced if a patient is treated at a high-volume hospital and by a surgeon who performs a high volume of procedures annually

Recently, investigators from the Health Outcomes Research Group, Memorial Sloan-Kettering Cancer Center and the Applied Research Branch of the National Cancer Institute evaluated data from the Surveillance, Epidemiology, and End Results -Medicare database to evaluate the incidence of postoperative complications and death in relation to the volume of patients treated at a hospital or the volume of procedures performed by a surgeon. Four separate categories were evaluated: postoperative complications, including life-threatening events during the 30 days after surgery involving cardiac, respiratory, vascular, renal or bleeding events, shock or the need for reoperation late urinary complications including bladder obstructions, fistulas, strictures, pelvic abscess or early incontinence that occurred 31 to 365 days after surgery postoperative death and long-term incontinence lasting over one year following surgery.

This data appears to indicate that patients undergoing a radical prostatectomy may have a reduced risk for developing some side effects if treated in a high-volume hospital and by a surgeon who performs a large number of prostatectomies annually.

Types Of Prostate Cancer

AdenocarcinomaAlmost all cases of prostate cancer begin in the gland cells that make the prostate fluid. This type of cancer is called adenocarcinoma. Other kinds of cancer can also start in the prostate. This includes small cell carcinoma, transitional cell carcinoma, and sarcoma. But these types of cancer are rare. Most men with prostate cancer have adenocarcinoma.

Other types of growth that may affect the prostate include:

Benign prostatic hyperplasia As a man ages, the prostate can grow larger. BPH is a common condition. While not prostate cancer, it can cause the prostate to press on the urethra. Pressure on the urethra causes urination issues and can be treated.

Prostatic intraepithelial neoplasia This abnormal non-cancerous prostate growth may be found with a prostate biopsy. PIN can be low-grade or high-grade. Low-grade PIN is more common as men grow older. Men who have high-grade PIN have a chance that cancer is somewhere else in the prostate.
