ROCHESTER, Minn. — Two Mayo Clinic patients whose prostate cancer had been considered inoperable are now cancer free thanks in part to an experimental drug therapy that was used in combination with standardized hormone treatment and radiation therapy. The men were participating in a clinical trial of an immunotherapeutic agent called MDX-010 or ipilimumab. In these two cases, physicians say the approach initiated the death of a majority of cancer cells and caused the tumors to shrink dramatically, allowing surgery. In both cases, the aggressive tumors had grown well beyond the prostate into the abdominal areas.
“The goal of the study was to see if we could modestly improve upon current treatments for advanced prostate cancer,” says Eugene Kwon, M.D., Mayo Clinic urologist and leader of the clinical trial. “The candidates for this study were people who didn’t have a lot of other options. However, we were startled to see responses that far exceeded any of our expectations.”
“The tumors had shrunk dramatically,” says Michael Blute, M.D., Mayo urologist, co-investigator and surgeon, who operated on both men. “I had never seen anything like this before. I had a hard time finding the cancer. At one point the pathologist asked if we were sending him samples from the same patient.”

Analysis: As Mayo Clinic Bets Big On Protons Technology Still Lacking Strongest Evidence
ROCHESTER, Minn. As Mayo Clinic invests hundreds of millions of dollars into proton beams, the wait continues for decisive evidence that the treatments are superior to existing methods at reducing tissue damage following radiation of tumors.
The lack of highest-level trial data in support of the sophisticated cancer treatments underscores a persistent dichotomy in medicine today, one in which calls to “follow the data” rise alongside projects reflecting the intangible, enduring fascination with technology.
Otherwise known as demand.
Mayo cited the demand for proton beam therapy when it announced plans earlier this month to build a $200-million, 110,000-foot addition to its Robert O. Jacobson Building, a $188 million center launched in 2015.
When it opens in 2025, the expansion will increase capacity for the high-tech treatments on the Rochester campus by 50%, allowing the care of an extra 900 patients annually.
At six rooms, the Jacobson Building will become one of largest proton beam centers in the nation.
It will join a trio of planned or completed proton centers under the Mayo banner, an infrastructure stockpile that includes a $182 million structure in Phoenix, and a $233 million building in Jacksonville, Florida, slated for 2026.
The arsenal will give Mayo more proton resources than those currently held within the entire United Kingdom.

Prostate Cancer Treated With Robotic Surgical System
Dr. Randall Minion is active in his medical practice after having robotic surgery to treat his prostate cancer.
Randall Minion, M.D., was helping out a newly hired lab technician when he had his prostate specific antigen tested. When he volunteered his blood sample for training purposes at his private practice, he never dreamed it would lead to the serendipitous early detection and treatment of prostate cancer.
“I was 49, so I was half-joking, but it was an appropriate test to run because I would be turning 50 in a few months,” says Dr. Minion, a family medicine physician in Fort Dodge, Iowa. “When my PSA was elevated, I visited my local urologist, who did a biopsy.” It came back positive for cancer and Minion’s doctor discussed treatment options, which included a new robotic surgery procedure.
A physician himself, Dr. Minion already knew the possible side effects of radical prostatectomy, which include urinary incontinence and impotence. He did extensive research on treatment options. “I chose surgery to avoid the many trips required for radiation therapy,” he says. “It also offered me the best chance for a cure.”
After learning more about robotic surgery and its benefits, Dr. Minion scheduled an appointment at Mayo Clinic, whose urology practice is ranked among the top centers for Urology by U.S. News and World Report. One doctor suggested a prostatectomy, but my husband finds ablation appealing because of quality of life advantages. I’ve been reading about ablations and came across information about different techniques. I am wondering if one technique or process is better, or if surgery is more successful?
ANSWER: I’m sorry to hear that your husband is dealing with prostate cancer, which is the most common cancer in American men. Approximately 192,000 men will be diagnosed with prostate cancer in 2020, according to the American Cancer Society.
Prostate cancer is more likely to develop in older men, typically those over 60, and it is more common in African American men. Given advances in care, most men will not die from prostate cancer.
There are many options for treating prostate cancer, depending on the grade and stage of the cancer. In recent years, the guidelines for treating prostate cancer have changed. Also, technological advances in imaging and treatment have made it possible for some men to avoid radical surgery that may come with adverse side effects. In recent years, surgeons are finding that ablation is a viable alternative to traditional surgery.
The ability to control the application of the heat or cold, and get the energy to stay within an intended boundary and not spread even 1 or 2 millimeters beyond, has proven difficult.

Prostate Cancer Experiences Of Black Men: The Good The Bad And The Ugly

Why did we do the study? On average, about 30,000 Black men will hear the words You have prostate cancer annually and over 5,000 Black men will die of prostate cancer annually. The good news is that deaths from prostate cancer has dropped to about half in Black men since 1993. The bad news is that Black men continue to be affected by prostate cancer compared to any other racial or ethnic group. Fighting prostate cancer in the Black community is difficult because many factors contribute to the problem. To conquer prostate cancer, we will need to pull together all our knowledge, experiences, and resources and include all stakeholders. Of significant importance is our knowledge of the prostate cancer experiences of Black men.
What was the goal of the study? With funding from the US Department of Defense, our research team set out to study Florida Black prostate cancer survivors in order to explain the experiences of Black men across the prostate cancer care continuum, including prostate cancer prevention, screening, detection, treatment and survivorship. Our goal was to develop what we call the prostate cancer care and survivorship model.
