What Happens During The Procedure

You usually have a general anaesthetic for this procedure. If you have other long-term medical conditions, such as problems with your heart or breathing, we may recommend you have a spinal anaesthetic instead with some sedation to make you comfortable. This means you have no feeling of pain from the waist down during the operation. If this applies to you the anaesthetist will let you know.Once you are in the operating theatre: We will pass a telescopic instrument called a cystoscope into your urethra and up into your bladder to examine it. The cystoscope is 2-3 millimetres thicker than the thickness of a pencil and has a tiny video camera on one end, so we can view images of your urethra and bladder on a television screen We will vaporise the prostate tissue through the cystoscope using a high-powered laser. For men over 60, benign prostatic hyperplasia more commonly known as prostate enlargementis a common and uncomfortable condition. Most patients experience urinary frequency, urgency, and even retention. Until recently, treatments involved either medication or surgery, both of which come along with a host of side effects that include dizziness, bleeding, erectile dysfunction, and urinary incontinence.

What Happens During A Holmium Laser Enucleation Of The Prostate Procedure

  • HoLEP requires the patient to be under general anesthesia . If a patient cannot have general anesthesia for some reason, a spinal anesthetic can be usedâthis allows him to remain awake but blocks all feeling from the waist down.
  • An injection of an antibiotic is given to reduce the chances of infection.
  • The patient is placed on his back with the legs raised during the procedure.
  • A surgical instrument called a resectoscope is inserted through the urethra . The resectoscope includes a camera that allows the surgeon to view the internal structure of the prostate gland, and to see where incisions are being made during surgery.
  • The laser is inserted into the resectoscope and is used to enucleate the enlarged prostate tissue from the capsule and then to seal up any blood vessels. The tissue that has been removed is deposited in the bladder.
  • The laser is removed from the resectoscope and is replaced by a morcellator. This device is used to suction out the tissue that was left behind in the bladder.
  • When tissue removal is complete, the resectoscope is withdrawn and a urinary catheter is put in place.

Medium And Long Term Side Effects

Patients usually resume everyday activities within a month of surgery and ongoing treatment is seldom required with this long-term solution. Its safe to drive after a couple of days, although a catheter may be needed. Side effects can include the uncomfortable passing of urine/blood and the frequent urge to urinate erection strength or quality may be impacted and you should abstain from sexual activity for at least two weeks.
