Prostate Cancer Stages And Other Ways To Assess Risk

After a man is diagnosed with prostate cancer, doctors will try to figure out if it has spread, and if so, how far. This process is called staging. The stage of a prostate cancer describes how much cancer is in the body. It helps determine how serious the cancer is and how best to treat it. Doctors also use a cancer’s stage when talking about survival statistics.

The stage is based on tests described in Tests to Diagnose and Stage Prostate Cancer, including the blood PSA level and prostate biopsy results.

Updates In Bladder Cancer Staging

Urothelial carcinoma that concurrently involves the prostate and bladder can be staged in 2 different ways. First, if there is a bladder primary with direct involvement of the prostatic stroma through the bladder wall, it is staged as T4 . Second, if there is bladder urothelial carcinoma as well as concurrent involvement of the prostatic urethra, typically by extension of additional independent primary urethral lesions with prostatic stromal invasion, this would be staged as both a bladder and prostatic urethral primary. Therefore, a cystoprostatectomy performed for muscle invasive high-grade urothelial carcinoma, found to also contain involvement of the prostatic urethra and prostatic ducts with stromal invasion , would be staged based on the AJCC 7th edition as pT4aNXMX and based on the 8th edition as bladder: pT2NXMX and prostatic urethra: pT2 NX MX ., Hence, both thorough microscopic and gross examinations are necessary to appropriately evaluate the pathologic stage.

Perivesical lymph nodes were added as a primary drainage site for bladder urothelial carcinoma as involvement correlates with a worse prognosis ., Therefore, pathologic evaluation of the perivesical soft tissue for lymph nodes are prudent.

However As With Other Types Of Cancer

It may grow slowly and it’s typically treatable. But hearing the words can still be scary. One in seven men in the united states will receive a prostate cancer diagnosis during his lifetime. Prostate cancer stays “localized” when cancer cells are found only in the prostate or even a little bit beyond it, but do not move to other parts of the body. The earlier the detection of prostate cancer, the better the patient’s chance of survival is.

A Conversation With Frederick L Greene Md Facs

CA Cancer Journal on Twitter: " Prostate cancer staging ...

This year marks the 60th anniversary of the creation of the American Joint Committee on Cancer , whose founding mission was to standardize the anatomic staging of cancer based on the size of a tumor and any spread to nearby tissue the spread of the cancer to nearby lymph nodes and the spread of cancer to other parts of the body . The intent was to better understand the clinical behavior of specific cancers, determine prognosis, and help physicians compare outcomes of similar groups of patients.

Developed in the 1940s and 1950s by Pierre Denoix, MD, a surgical oncologist at the Institut Gustav-Roussy in Paris, the TNM staging system was primarily used for the clinical classification of breast cancer. In 1987, it became accepted worldwide as the anatomic staging system for most solid tumors.1

The AJCC was launched on January 9, 1959, to provide cancer classification that not only incorporated the international TNM staging system, but that was also relevantand acceptableto American oncologists. Initially composed of a coalition of 6 medical organizations, including the ACS, ACR, American Cancer Society, College of American Pathologists, American College of Physicians, and National Cancer Institute, today there are 22 sponsoring organizations of the AJCC, including the National Comprehensive Cancer Network® and ASCO.

We read with interest the article by Herden and colleagues regarding the validation of the UICC and AJCC 8th edition tumor-nodes-metastasis system on a German community-based dataset. In this study, the authors analyzed data derived from the HAROW study , a prospective, non-interventional, health service research study examining various treatment options for patients with localized prostate cancer . Within their study, Herden and colleagues selected all patients that underwent radical prostatectomy with available pathologic information and follow-up data . For validation purpose, the authors stratified the patients in either cT1 or cT2, S I or S II , and prognostic stage group I, PSG IIA, PSG IIB or PSG IIC according to the TNM classification and World Health Organization grading. All groups were compared regarding proportion of organ confined disease and extraprostatic extension after RP, as well as favorable and unfavorable WHO grade group, or favorable and unfavorable Gleason Score after RP, respectively. Finally, biochemical recurrence free-survival was examined and compared between groups in each classification systems.

Shifting From Anatomic To Prognostic Staging

Since the 1940s, clinicians have used tumors , lymph nodes , and distant metastases to determine a cancers anatomic stage. However, in recent years, new technologies such as genomic profiles and molecular targets have led to new classification called prognostic staging that builds a more complex picture of cancer on top of anatomic stage.

Edge said that prognostic markers like genetic testing explain why some patients with early-stage cancer have poor response to treatment whereas other patients with advanced cancer respond well to treatment. Prognostic staging incorporates these markers into cancer classification to provide a more tailored picture of each patients prognosis.

Additionally, Edge explained, the new AJCC prognostic staging system is a searchable and updateable database that will better aid researchers and clinicians in interpreting case data.

AJCCs manuals have used factors other than T, N, and M in staging for some cancers in previous editions , but the 2018 8th edition shows a marked increase. However, Edge said that not all cancer types have validated nonatomic factors that can be used for prognostic staging, so not every cancer will see changes in staging.

Updates In Prostate Cancer Staging

A notable change in the AJCC 8th edition is that pT2 is no longer a 3-tiered system of organ-confined disease, including T2a , T2b , and T2c disease . Numerous studies have demonstrated that the pathologic substaging lacked prognostic implications and there was justification to merge the subdivisions into a single category. In addition, there was no significant difference in cancer-specific survival among the substage groups. Therefore, localized prostate cancer is highly dependent on nonanatomic factors such as Gleason score and correlating grade groups and serum prostate-specific antigen .

New Updates To Ajcc Staging With Release Of 8th Edition

The American Joint Committee on Cancer staging system is a classification system used to describe the extent of disease progression in patients with cancer. The AJCC manual is updated approximately every 6 years, and the recently released 8th edition contains its most radical changes to date. This is exciting because we know this enables us to give our patients more accurate staging and better treatment options, said Lavinia Dobrea, MS, RN, OCN, at the AONN+ 9th Annual Navigation & Survivorship Conference.

According to Ms Dobrea, Manager of the Oncology Research and Biospecimen Program at St. Joseph Hospital/Center for Cancer Prevention and Treatment in Orange, CA, cancer staging is vital to the medical community because it provides a common language for physicians around the world to talk about a cancer with no ambiguity or misunderstanding, and to thereby further medical advancements.

AJCC staging is a formalized process of determining how much cancer is in the body by classifying the location and size of the primary tumor, as well as its lymph node involvement and the extent to which it has spread. By understanding the staging, we can determine what were dealing with, including the prognosis and treatment options, she said. And that includes treatment options on clinical trials.

Areas For Further Research

There are several promising imaging methods on the horizon that may improve the staging of prostate cancer in the near future. Prime among these is the prostate-specific membrane antigen PET-CT. A series of high-affinity gallium-68âlabeled and fluorine-18 âlabeled ligands have been developed that bind PSMA, an antigen expressed in prostate cancers and their metastases. Thus, PSMA PET-CT is a promising method for both lymph node and bony staging. Other agents in the same category include F-18 choline and anti-1-amino-3-fluorocyclobutane-1-carboxylic acid , both of which target prostate cancer, although they do not appear to be as sensitive in head-to-head trials as PSMA PET-CT. However, because none of these are yet widely available, they cannot be recommended as part of the current standard of care. Similarly, progress has been made in imaging lymph nodes with ultrasmall particles of iron oxide combined with MRI, as normal lymph nodes will take up the agent and turn dark, but abnormal lymph nodes will not. However, this technique, termed magnetic resonance lymphography, although able to depict lymph nodes outside of the normal surgical template, is also still investigational.
