One of the challenges with choosing treatment is that are numerous options available, Choi explains. However, for most patients, both surgery and RT are good options with a very high chance for cure and low risk for side effects. Nevertheless, there are some patients who are more likely to benefit from RT, which are those who have comorbidities, such as heart disease. Other patients who might benefit better are those with very aggressive disease who might undergo RT anyway even if they had surgery. It is controversial whether RT by itself is less effective than RT plus surgery he adds.
Types Of Prostate Cancer
There is only one main type of prostate cancer: adenocarcinoma. Other types, such as sarcomas and small cell and transitional cell carcinomas, are very rare. A condition known as prostatic intraepithelial neoplasia may turn into cancer, but this connection isnt proven.
Adenocarcinomas
Prostate sarcoma is a rare type of cancer that starts in the soft tissue.
Small cell carcinomas
This rare type of prostate cancer is characterized by small, round cells. It tends to grow more quickly than adenocarcinomas.
Transitional cell carcinomas
Also known as urothelial cancer, transitional cell prostate cancer is a rare cancer that usually starts in the cells lining the urethra, but may start in the prostate.
Prostatic intraepithelial neoplasia

Owing to routine prostate-specific antigen screening, prostate cancer is increasingly detected at early stages, leading to a 5-year survival rate of nearly 100%. Although many screening-detected prostate cancers are indolent, about 90% of men with localized prostate cancer receive upfront aggressive treatments that often cause significant morbidity. Conversely, some patients with potentially aggressive prostate cancer who would benefit from early intervention may choose to delay treatment. This dilemma is particularly acute for patients with clinically defined intermediate-risk prostate cancer. Clinical variables alone are not sufficient to accurately differentiate aggressive and indolent diseases. Biomarkers are urgently needed to refine risk stratification. In this project, we will focus on three promising biomarkers: mitochondrial DNA , microRNA , and metabolites. These multi-functional and interconnected molecules are related to obesity, an established risk factor for aggressive prostate cancer. Leveraging two of the largest prostate cancer patient cohorts in the United States, we will perform integrative analyses of these biomarkers with clinical variables to more precisely define aggressive prostate cancer. We will use knowledge gained from comparing extreme phenotypes at diagnosis to better stratify patients with clinically defined intermediate risk profiles. There are four specific aims:

Project : Targeting Androgen Receptor And Parp For Synthetic Lethality In Crpc

Aim 1. Characterize the HR/HRM gene signature in bone marrow biopsy specimens from men with mCRPC treated with novel inhibitors of androgen signaling .
Aim 2. Use preclinical models to further characterize the synergistic potential of and identify biomarkers predictive of response to combination therapies that co-target AR and PARP .
Aim 3. Conduct a clinical trial of ENZ followed by OLA in CRPC patients to achieve greater therapeutic response and correlate an ENZ-regulated HR/HRM gene signature to the therapeutic responses.

Metastatic Prostate Cancer Treatment
If a patients prostate cancer has spread beyond the prostate and the surrounding area, he is given systemic therapies like hormone therapy and possibly chemotherapy. While cancer responds to hormone therapy, it is called castrate-sensitive disease. Over time, the disease may become less responsive to hormone therapy and start growing again. This is called castrate-resistant disease. Patients with castrate-resistant disease can be treated with a number of additional therapies. Many are eligible for clinical trials with newer drugs or drug combinations, including immunotherapy.

Cooper Prostate Cancer Registry
The Cooper Prostate Cancer Registry is the only one of its kind in New Jersey. It collects data, blood, and tissue samples from prostate cancer patients and their families. Developed and administered in collaboration with the Coriell Institute for Medical Research, the registry is playing an important role in helping doctors to understand the genetic factors behind prostate cancer. It is also contributing significantly to the development of improved diagnostic procedures and new tumor-specific treatments.
In addition, the Prostate Cancer Center provides access to cancer prevention and treatment clinical trials. For example, we are currently engaged in studies to determine the effectiveness of selenium and/or vitamin E in the prevention of prostate cancer. Why I Traveled Across The World For Prostate Cancer Treatment
When I was diagnosed with prostate cancer, I was determined to find the best care and treatment. As an orthopedic surgeon, I have traveled the world delivering lectures. After receiving my diagnosis, I knew that I would have to make a decision about undergoing surgery or radiation therapy.
I consulted some of the best surgeons and oncologists in the world but wanted to do my own research to see what was best for me.
