This pilot clinical trial studies how well magnetic resonance spectroscopic imaging with hyperpolarized carbon C13 pyruvate works in finding prostate cancer that exhibits poorly differentiated or undifferentiated cells and that is restricted to the site of origin, without evidence of spread in patients undergoing radical prostatectomy. Diagnostic procedures, such as MRSI with hyperpolarized carbon C13 pyruvate, may aid in the diagnosis of prostate cancer and in discriminating high-grade from low-grade prostate cancer and benign adjacent prostate tissue

Prostate Radiation Therapy Or Short

RATIONALE: Radiation therapy uses high-energy x-rays to kill tumor cells. Androgens can cause the growth of prostate cancer cells. Antihormone therapy, such as flutamide, bicalutamide, and luteinizing hormone-releasing hormone agonist, may lessen the amount of androgens made by the body. It is not yet known which regimen of radiation therapy with or without androgen-deprivation therapy is more effective for prostate cancer.

PURPOSE: This randomized phase III trial is studying prostate radiation therapy to see how well it works compared with short-term androgen deprivation therapy given together with pelvic lymph node radiation therapy with or without prostate radiation therapy in treating patients with a rising PSA after surgery for prostate cancer.

A Study Of Salvage Radiotherapy With Or Without Enzalutamide In Recurrent Prostate Cancer Following Surgery

Patients with post-prostatectomy PSA recurrences with aggressive disease features will receive salvage radiation therapy and standard androgen deprivation therapy or enhanced ADT to determine if there is any improvement in progression-free survival when enhanced ADT is used compared to standard ADT.

Who Is At Risk For Advanced Prostate Cancer

The exact cause of prostate cancer isnt clear. Your risk of developing this particular cancer increases after you reach age 50.

Certain groups are more likely to develop aggressive forms of prostate cancer, including African-American men and men who carry certain inherited genetic mutations such as BRCA1, BRCA2, and HOXB13.

Most men with prostate cancer dont always have a family history of the disease. But having a father or brother with prostate cancer more than

You probably wont need all of these tests. Your doctor will choose the tests based on your symptoms and physical exam.

If any of the images reveal abnormalities, it doesnt necessarily mean that you have cancer. Additional testing may be necessary. If they find a mass, your doctor will probably order a biopsy.

For a biopsy, your doctor will use a needle to remove samples from the suspicious area. A pathologist will then analyze the removed cells under a microscope to see if theyre cancerous. The pathologist can also determine if you have an aggressive form of prostate cancer.

A total of 15 of 30 patients have been enrolled on the phase 1 PROPELLER trial , which seeks to assess the diagnostic value of 64Cu SAR-bisPSMA in those with untreated and confirmed prostate cancer who are scheduled to undergo radical prostatectomy.5

We are excited to have quickly and successfully recruited half of the patients planned for the PROPELLER trial with all 3 sites actively recruiting and imaging prostate cancer patients across Australia. We have been able to not only generate strong preliminary clinical data since the trial commencement in July 2021, but also validate our on-demand distribution model where the 64Cu-SAR-bisPSMA has been shipped to the trial sites across Australia from a central manufacturing facility with minimal delays or interruptions, Alan Taylor, PhD, executive chairperson at Clarity Pharmaceuticals, said in a press release.

The multicenter, blinded review, dose-ranging, non-randomized PET imaging trial will administer 64Cu-SAR-bisPSMA to patients prior to undergoing surgery. The study has 4 particular goals:

  • Identify ability of 64Cu SAR-bisPSMA to detect primary disease

    This phase IIa trial investigates how well apalutamide before surgery works in treating patients with prostate cancer that is confined to the prostate gland. Testosterone can cause the growth of prostate cancer cells. Apalutamide blocks the use of testosterone by the tumor cells. Giving low dose apalutamide before prostate surgery may lead to lowered PSA levels in men with prostate cancer that is confined to the prostate gland.

    What Is Prostate Cancer

    Prostate cancer is cancer that affects the prostate gland in men. Prostate cancer is the second-leading cause of cancer deaths for men in the US.

    The prostate lies below the bladder and in front of the rectum. In men, the size of the prostate increases with increasing age. In younger men, it is about the size of a walnut. Its primary function is to make fluid to nourish the semen.

    Growth in the prostate can be of two types

  • Benign growths: These are noncancerous growths and are rarely a threat to life. For example, benign prostatic hyperplasia.
  • Malignant growths: These are cancerous growths that can sometimes be life-threatening.
  • Prostate cancer starts in the prostate gland and may spread to the nearby areas: lymph nodes, organs, or bones in other parts of the body.

    Intermediate Risk Group Treatment Options

    Intermediate-risk prostate cancer refers to tumors that havent spread outside the prostate, but have a Gleason score of 7 and medium PSA levels . Treatment aims to reduce the risk of metastasis to other parts of the body. In this risk group, treatment options include:

    • Radiation therapy or brachytherapy, often with ADT
    • Radical prostatectomy with pelvic lymph node dissection
    • Active surveillance, though the risk for cancer spreading is higher than in very low and low risk groups, so many doctors discourage this option

    Patients will be monitored closely with PSA testing if levels rise, radiation therapy may be recommended.

    This is a tissue and blood collection protocol requiring image-guided biopsies of metastatic prostate cancer and other genitourinary malignancies including renal cell carcinoma and urothelial carcinoma. Whenever possible, a new bone lesion or new/progressing soft tissue lesion will be chosen for biopsy as opposed to radiographically stable lesion. Patients will be enrolled in into one of several parallel cohorts based upon disease status or type and the planned systemic therapy following baseline tumor biopsy: Androgen signaling inhibition, Immunotherapy, Radiotherapy, Targeted Therapy/Investigational therapeutic, DNA damage response pathway, Aggressive variant disease, Castration-sensitive ADT naïve and ADT < 3 months), or Castration-sensitive pre-treated with sub-optimal PSA nadir > 0.2 ng/ml, metastatic renal cell carcinoma and metastatic and urothelial carcinoma.

    Apalutamide for High-Risk Prostate Cancer | Learn About Clinical Trials

    Doctors use the term stage to characterize the characteristics of the original tumor itself, such as its size and how far prostate cancer has spread when it is discovered, as they do with all malignancies.

    Staging systems are difficult to understand. Most malignancies, including prostate cancer, are staged using three different elements of tumor growth and dissemination. The TNM system stands for tumor, nodes, and metastasis:

    • T, for tumor describes the size of the main area of prostate cancer.
    • N, for nodes, describes whether prostate cancer has spread to any lymph nodes, and how many and in what locations.
    • M, for metastasis, means distant spread of prostate cancer, for example, to the bones or liver.

    The TNM approach allows each mans prostate cancer to be characterized in depth and compared to the prostate cancers of other men. Doctors utilize this information to conduct research and make treatment decisions.

    However, in terms of prostate cancer survival statistics, the staging method is straightforward. As previously stated, males with prostate cancer can be split into two groups in terms of survival rates:

    Newer Treatments For Early

    Researchers are looking at newer forms of treatment for early-stage prostate cancer. These new treatments could be used either as the first type of treatment or after unsuccessful radiation therapy.

    One treatment, known as high-intensity focused ultrasound , destroys cancer cells by heating them with highly focused ultrasonic beams. This treatment has been used in some countries for a while, and is now available in the United States. Its safety and effectiveness are now being studied, although most doctors in the US dont consider it to be a proven first-line treatment for prostate cancer at this time.

    What Happens In A Clinical Trial

    Types of clinical trials

    There are different ways of gathering evidence in cancer research. There are two main types of trials observational studies and intervention studies.

    • In an observational study, researchers collect information about you that might tell them more about the risk of getting cancer, or the effects of cancer or its treatment. For example, they might select a large group of people and take blood samples, or ask them to answer questions about their lifestyle and diet at the start of the study. Then many years later, the researchers would look at whether or not those people developed cancer. They would then be able to see if there were any differences between the people who developed cancer and those who didnt.
    • In an intervention study, researchers give a group of people a treatment or test and follow them to see how well it works. One type of intervention study is called a randomised controlled trial this is normally used to test new treatments.

    This is a prospective imaging study evaluating the utility of baseline metabolic MR imaging as a diagnostic and response monitoring tool in patients with advanced prostate cancer. Preliminary pre-clinical and clinical data demonstrates the ability of HP C-13 pyruvate/metabolic MR imaging to detect high-grade prostate cancer, including cancer with neuroendocrine differentiation, as well as provide early evidence of metabolic response and resistance following application of systemic therapies for the treatment of advanced prostate cancer patients. In the proposed study, the investigators aim is to extend the initial clinical results and further develop HP C-13 MRI as an imaging modality in advanced prostate cancer.

    Study Of Docetaxel Combined With Cirmtuzumab In Metastatic Castration Resistant Prostate Cancer

    The purpose of this study is to examine the safety and efficacy of cirmtuzumab in combination with standard of care docetaxel in patients with metastatic castration resistant prostate cancer. Docetaxel is a taxane chemotherapy which has been shown to prolong survival in men with castration resistant prostate cancer. Cirmtuzumab is a monoclonal antibody that targets the receptor called ROR1 of the non-canonical Wnt pathway and is suspected to contribute to prostate cancer growth and progression.
