A Minimally Invasive Approach

After receiving a diagnosis of prostate cancer, I met with Dr. Eric Horwitz, a radiation oncologist, in January 2020, and we walked through all the processes and he did more blood tests. At that point, it looked like radiation was the best way to go. We scheduled a minimally invasive SpaceOAR procedure for February with surgical oncologist Dr. Richard Greenberg.

SpaceOAROAR stands for organ at riskis a gel that is injected under anesthesia into the space between the rectal wall and the prostate. The idea is to protect the rectum from the high doses of radiation used to treat the prostate. Exposing the rectum to radiation can lead to a disruption of bowel function in some patients. After the procedure, I was scheduled for 23 external beam radiation treatments.

How Prostate Cancer Is Diagnosed

The experienced urologists and surgeons at Philadelphias Jefferson Multidisciplinary Genitourinary Oncology Center and Sidney Kimmel Cancer Center Jefferson Health work as a team to confirm a prostate cancer diagnosis. Our thorough examination begins with a discussion about your symptoms and a review of your medical history.

Jeffersons experts will conduct a series of physical exams to feel for prostate gland abnormalities. Our urologists and our prostate cancer specialists will conduct a Prostate Specific Antigen or PSA blood test as well as other prostate-related tests and diagnostic imaging tests such as the Color Doppler or 3D Ultrasound. Our urologists may also take prostate tissue samples for conducting a prostate biopsy.

Nipping It In The Bud

Although the fatality rate of my cancer was relatively low, I felt more comfortable nipping it in the bud. Presented with the same three treatment options by Fox Chase, I finally decided to undergo robotic surgery .

I could not fully enjoy life with the lingering thought of prostate cancer, so the wait-and-see approach was not really an option for me. I cannot overstate the amount of research I put into my decision to have surgery. Whether for legal decisions or diet and exercise info, thorough research has always been a major part of my life.

My wife Janet knows this better than anyone. When we shop together, she knows I will go out of my way to compare the prices and attributes of a certain appliance, and a final decision might take weeks. I went over my PSA numbers and Gleason score again and again and related them to my research.

Everyone has some anxiety about operations, but my biggest concern was urinary incontinence afterward. Thats the one thing I couldnt live with. I had endured a few days of incontinence due to a previous unrelated procedure, so I knew it was something I wanted to avoid if at all possible.

Former Philly Mayor Michael Nutter Had Surgery For Prostate Cancer He Especially Wants Black Men To Hear About His Experience

PSA screening poses a dilemma, especially for Black men, who are at higher risk of developing and dying of prostate cancer.

In December, after more than four years of medical monitoring while keeping his early-stage prostate cancer secret from his family, Michael Nutter had the walnut-size organ removed.

Now, fully recovered fully, he stressed the former Philadelphia mayor is advocating that men undergo regular screening for prostate cancer with the PSA blood test.

Get screened! he said during an interview on Thursday at the Center City offices of public relations executive Jay Devine. All you have to do is give a little blood. See where you are. Its not like theyre drilling a hole in your head.

Nutter, 63, is aware that prostate specific antigen screening is far more controversial than when he began doing it at age 50 in 2008. Back then, his doctor recommended checking his PSA level, so Nutter did it.

Fast forward to today. All the expert groups that issue guidelines, including the American Urological Association, acknowledge a dilemma. Studies show automatic, one-size fits all screening saves very few lives, and leads to diagnosis and treatment of many cancers that would never cause harm if left undetected. Treatment may cause permanent urinary or sexual problems, or both. The guidelines say men should be informed of the risks, as well as the benefits, before deciding what to do.

Nutrition And Lifestyle Changes

Many studies have looked at the possible benefits of specific nutrients in helping to treat prostate cancer, although so far none have shown a clear benefit. Some compounds being studied include extracts from pomegranate, green tea, broccoli, turmeric, flaxseed, and soy.

One study has found that men who choose not to have treatment for their localized prostate cancer may be able to slow its growth with intensive lifestyle changes. The men in the study ate a vegan diet and exercised frequently. They also took part in support groups and yoga. After one year the men saw, on average, a slight drop in their PSA level. It isnt known if this effect will last since the report only followed the men for 1 year. The regimen may also be hard for some men to follow.

It’s important for men thinking about taking any type of nutritional supplement to talk to their health care team first. They can help you decide which ones you can use safely while avoiding those that might be harmful.

Newer Treatments For Early

Researchers are looking at newer forms of treatment for early-stage prostate cancer. These new treatments could be used either as the first type of treatment or after unsuccessful radiation therapy.

One treatment, known as high-intensity focused ultrasound , destroys cancer cells by heating them with highly focused ultrasonic beams. This treatment has been used in some countries for a while, and is now available in the United States. Its safety and effectiveness are now being studied, although most doctors in the US dont consider it to be a proven first-line treatment for prostate cancer at this time.

I Didnt Want To Worry Them

In 2007, Nutter won a come-from-behind victory in a field of political heavyweights by promising to clean up City Hall and avoid the ethical scandals of his predecessor and nemesis, John F. Street. Nutter led with a data-focused approach to city management. He had an oftentimes icy relationship with City Council and labor throughout his eight years, which included the recession, the spotlight of the 2015 papal visit, and planning for the Democratic National Convention.

Since leaving office in 2016, he has been involved in education, media, public policy, political campaigns, consulting, and more.

It was in 2016 that Nutters PSA level, which had been under the threshold for concern, rose enough that his primary care doctor, Chris Hannum, sent him to urologist Laurence Belkoff in Bala Cynwyd. Belkoff is a managing partner of the MidLantic Urology group and on the faculty at Philadelphia College of Osteopathic Medicine and Drexel University College of Medicine.

Nutter underwent a biopsy, an ultrasound, a CAT scan, and a bone scan, then Belkoff gave him the bottom line: He had a tiny, relatively nonaggressive cancer confined to his prostate. The options, Belkoff said, were surgery or radiation. Or, they could monitor the cancer closely with frequent PSA tests and follow-up biopsies so-called active surveillance.

Im working and traveling and teaching, Nutter recalled. I said Id like to do active surveillance.

I didnt want to worry them, he said.
