What Are The Types Of Prostate Surgery For Benign Prostatic Hyperplasia

Treatment decisions for prostate cancer in 2021.

Several minimally invasive surgeries may be used for benign prostatic hyperplasia. These include:

GreenLight laser: Photoselective vaporization of the prostate is a treatment that vaporizes prostate tissue to create a channel in the urethra for free urination. This is surgical treatment performed in the operating room under general anesthesia.

Plasma button electrovaporization: Prostate tissue is removed using low temperature plasma energy. Tissue is vaporized. This is an operating room-based therapy and requires an anesthetic.

Water vapor therapy : Water vapor is directly delivered to the prostate tissue. Over a 3-month period the tissue is destroyed and reabsorbed by the body. This is an office-based therapy and is performed under a local nerve block.

UroLift®: A mechanical approach that places implants to pin the lateral prostate lobes out of the way to reduce obstruction. This is an office-based procedure performed under a local nerve block.

Prostatic artery embolization: This approach uses catheters to deliver agents that block blood flow to the prostatic artery to reduce symptoms of BPH by shrinking tissue. This prostate procedure uses local anesthesia and doesnt require a hospital stay.

Choosing To Stop Treatment Or Choosing No Treatment At All

For some people, when treatments have been tried and are no longer controlling the cancer, it could be time to weigh the benefits and risks of continuing to try new treatments. Whether or not you continue treatment, there are still things you can do to help maintain or improve your quality of life.

Some people, especially if the cancer is advanced, might not want to be treated at all. There are many reasons you might decide not to get cancer treatment, but its important to talk to your doctors and you make that decision. Remember that even if you choose not to treat the cancer, you can still get supportive care to help with pain or other symptoms.

The PAE procedure is best for candidates who are either ineligible due to pre-existing health conditions or not interested in traditional surgery. Entadfi Approved For Benign Prostatic Hyperplasia

The Food and Drug Administration has approved Entadfi for the treatment of the signs and symptoms of benign prostatic hyperplasia in men with an enlarged prostate for up to 26 weeks.

Entadfi is a fixed-dose combination of finasteride, a 5-reductase inhibitor, and tadalafil, a phosphodiesterase 5 inhibitor. The approval was based on a randomized, double-blind, placebo-controlled phase 3 study that evaluated the efficacy and safety of tadalafil plus finasteride in 696 men.

Patients were randomly assigned to receive tadalafil 5mg with finasteride 5mg or placebo with finasteride 5mg. The primary endpoint was the change from baseline to week 12 in total International Prostate Symptom Score .

Findings showed a statistically significant improvement in the signs and symptoms of BPH when tadalafil was administered with finasteride vs placebo plus finasteride . Improvement in total IPSS was observed starting at week 4 and continued through week 26 .

Entadfi is not recommended for more than 26 weeks because the incremental benefit of tadalafil decreases from 4 weeks until 26 weeks and the incremental benefit beyond 26 weeks is unknown.

Entadfi is supplied as a capsule containing a combination of finasteride 5mg and tadalafil 5mg in 30- or 90-count bottles. The product is expected to be available in early 2022.

    The Issue With Supplements

Dr. Klotz – What’s New in Active Surveillance for Prostate Cancer in 2021

Dietary supplements go through fewer regulatory hurdles than drugs. Drugs must undergo clinical trials to demonstrate to the FDA that they are safe and effective.

The FDA can then approve the new drug, so it can be marketed and sold in the US. However, safety issues are detected with a third of medicines after market approval.

Dietary supplements are considered safe until proven otherwise. Manufacturers are not obligated to conduct clinical trials to prove the efficacy and safety before entering the market.

Brands are free to make a number of health claims related to addressing nutrient deficiencies, supporting general health or supporting a particular body function .

The manufacturers need to follow the claim with the following message: This statement has not been evaluated by the Food and Drug Administration. This product is not intended to diagnose, treat, cure, or prevent any disease.

The options for laser therapy include:

Ablative proceduresThese procedures vaporize obstructive prostate tissue to increase urine flow. Examples include photoselective vaporization of the prostate and holmium laser ablation of the prostate . Benign prostatic hyperplasia surgery has a very high success rate. However, for the best results from treatment, you should seek medical help at the earliest signs of a problem. Also, talk with a doctor if you are experiencing symptoms of BPH after surgery.

Treatment Option For Bph: The Urolift System

Urologists at UCLA Urology are now offering UroLift, a new treatment option for benign prostatic hyperplasia .

    UroLift Offers Relief for BPH

When Jeff Brenner first began experiencing bothersome urinary symptoms in his 40s, he attributed it to middle age and figured it was something he could live with.

An enlarged prostate compresses on the urethra, making it difficult for urine to flow. The UroLift Device is placed through the obstructed urethra to access the enlarged prostate. The device compresses the tissue and delivers tiny implants to lift and hold prostate tissue out of the way, thus opening the urethra. The permanent implants keep the tissue in place, like tiebacks on a window curtain. The UroLift Device is removed, leaving an open urethra for urine to flow.

New Treatment For Enlarged Prostate Shows Fewer Side Effects

A noninvasive method could offer men with an enlarged prostate effective treatment with fewer complications.

A new treatment could provide men with relief from symptoms of an enlarged prostate with fewer side effects than traditional methods.

The minimally invasive technique isnt entirely new, but a study presented this week at the Society of Interventional Radiologys Annual Scientific Meeting shows that the method could be an alternative to commonly used treatments for benign prostatic hyperplasia .

This innovative treatment offers less risk, less pain and less recovery time than traditional surgery, said Dr. Sandeep Bagla, the studys lead researcher and an interventional radiologist at Inova Alexandria Hospital in Virginia, in a press release. We are hopeful that further research will confirm it to be an effective therapy for BPH.

BPH affects about one-third of men 50 years or older, and 90 percent of men 85 years or older. According to Medscape, up to 14 million men in the United States experience symptoms due to an enlarged prostate.

Researchers looked at the medical records of 78 men who were treated for an enlarged prostate using the new technique prostate artery embolization. Doctors performed the procedure as part of their routine clinical practice, not through a clinical study.

    Considering Complementary And Alternative Methods

Vitamins To Take After Prostate Surgery

You may hear about alternative or complementary methods that your doctor hasnt mentioned to treat your cancer or relieve symptoms. These methods can include vitamins, herbs, and special diets, or other methods such as acupuncture or massage, to name a few.

Complementary methods refer to treatments that are used along with your regular medical care. Alternative treatments are used instead of a doctors medical treatment. Although some of these methods might be helpful in relieving symptoms or helping you feel better, many have not been proven to work. Some might even be harmful.

Be sure to talk to your cancer care team about any method you are thinking about using. They can help you learn what is known about the method, which can help you make an informed decision.

    How Effective Is Pae

PAE is a new and promising procedure. In a study published in 2016, 630 patients underwent a prostate artery embolization procedure. The study found that the procedure had a positive effect on urinary symptoms as well as overall quality of life. This positive effect lasted 1-3 years in 82% of the patients and lasted 3-7 years in 76% of the patients. Additionally, there was no urinary incontinence or sexual dysfunction reported.

With the high success rate of PAE, men ineligible for surgery are able improve their quality of life by eliminating or reducing uncomfortable BPH symptoms. Be sure to speak to your urologist to help determine the best approach for you.
