Water Vapor Treatment For Prostate

Rezm Therapy For Bph Treatment

Suffering from Enlarged Prostate (BPH)? Try Rezm Water Vapor Therapy.

Rezm water vapour therapy is a safe and effective new BPH treatment that can offer longer term relief from symptoms without the risks and discomfort of surgery.

The therapy is a minimally invasive treatment for an enlarged prostate that uses controlled doses of hot steam to directly ablate and reduce portions of the swollen prostate that are causing obstruction of the urethra.

Rezm has undergone rigorous studies and is recommended by the Canadian Urological Association as a minimally invasive BPH symptom reduction method for men interested in retaining preserving erectile and ejaculatory function. It was also approved by the US FDA in 2015 for the treatment of the enlarged prostate.

SanoMed is the first clinic to offer Rezm in Quebec and Eastern Canada.

What Do The Studies Say About Rezum

A UK study published in 2018 reported on a 5-year follow-up of the results of Rezum treatments showed an improvement of symptoms/ symptom relief on the international scale from 19.4 to 8.6. This is pretty significant!

Retreatment rates were between 3 and 5%, but no cases of ejaculatory dysfunction were caused by the method. In fact, the scientists reported that the ejaculatory bother to score improved by 31% compared with baseline.

Twenty-seven percent of the patients studied achieved clinically relevant differences in erectile dysfunction in one year.

However, Rezum treatment wont sample the tissue of the prostate for prostate cancer. The TURP method, on the other hand, finds anywhere from 4.1 to 16.7% of the cases. Thus, Rezum is not particularly suitable for those men who have urinary retention or have a large prostate volume.

How Does Rezm Water Vapor Therapy Work

Rezm Water Vapor Therapy is a non-surgical treatment that uses the natural energy stored in water vapor, or steam, to remove excess prostate tissue that is pressing on the urethra.

Rezm Therapy is typically performed right in your urologists office, or at an outpatient surgery facility and completed in one short appointment.

During each 9-second treatment, sterile water vapor is released throughout the targeted prostate tissue. When the steam contacts the prostate tissue, all the stored energy is released into the tissue. Your doctor will determine the amount of treatments you need, based on the size of your prostate.

Over time, your bodys natural healing response absorbs the treated tissue, shrinking the prostate. With the extra tissue removed, the urethra opens, reducing BPH symptoms.

Most patients begin to experience symptom relief in as soon as two weeks, and maximum benefit may occur within three months.*1,2 Patient responses can and do vary.

Further Considerations And Research

GREAT DAY SA: Urology SA Explains Rezum Water Vapor ...

The Rezum system is an exciting and novel minimally invasive therapy for treatingBPH, which thus far has demonstrated strong evidence of clinical effectiveness witha potential wide scope of use with limited drawbacks. However, it is important toconsider limitations in the current literature when assessing this noveltreatment.

Previous trials have been supported or sponsored by NxThera which may represent a potential conflict of interest. So far outcomes up to 3yearshave been recorded, this is as part of a 5-year RCT currently ongoing. Long-termoutcomes are currently awaited so it is difficult to comment on the long-termefficacy of Rezum as a treatment for BPH and whether there will be continued successin relief of LUTS symptoms. A reduction in prostate volume was noted, although nological explanation on lack of substantial reduction in PVR was noted, which needsto be explored in the future studies.

Further evidence in a nontrial setting is needed in order to confirm theeffectiveness of Rezum in real-world applications and also to provide additionalsupport of findings published in a retrospective trial indicating the potential useof Rezum in patients with a large prostate burden and in urinary retention.

What Are The Risks Of Rezum

Like any treatment, side effects with Rezum can happen. In clinical trials, the most common side effects were temporary. They included burning with urination , blood in the urine , urinary frequency and urgency , temporary inability to urinate and urinary tract infection . All of these side effects were resolved within 3 weeks of treatment. Less common side effects include blood in the semen and decreased ejaculate volume.

Who Are The Men Who Can Benefit From Rezum Treatment Of The Prostate

  • Men who do not want to take drugs to treat BPH.
  • Men who are bothered by BPH symptoms, despite medical treatment.
  • Men who do not want surgery to treat their BPH.

Is Benign Prostatic Hyperplasia A Risk For Prostate Cancer

There is no evidence that BPH increases the risk of developing prostate cancer. However, the symptoms of BPH and prostate cancer are similar. The American Urological Association and the American Cancer Society recommend annual prostate screenings for men ages 55 to 69. Men at high risk such as African-American men and men with a family history of prostate cancer should begin screening at 40. It is important that doctors and patients discuss the screening process and possible inaccurate results while arriving at a decision to screen for prostate cancer.

Mayo Clinic Q And A: New Steam Treatment For Benign Prostatic Hyperplasia

DEAR MAYO CLINIC: I was diagnosed with benign prostatic hyperplasia four years ago. It didnt bother me at first, so my health care provider recommended watchful waiting. But now its causing quite a few problems, and were talking about surgery. I heard theres a new treatment for benign prostatic hyperplasia that involves steam. Does it work? Are there side effects?

ANSWER: There are several procedures available to treat benign prostatic hyperplasia, or BPH, including a relatively new treatment that uses steam to reduce the size of the prostate and ease symptoms. This treatment has been shown to effectively eliminate excess prostate tissue, while carrying a low risk of side effects.

Benign prostatic hyperplasia is enlargement of the prostate gland thats not related to cancer. Its a common condition in men as they age. The prostate gland is beneath the bladder. The tube that transports urine from the bladder, called the urethra, passes through the center of the prostate. When it first starts, benign prostatic hyperplasia may cause few, if any, symptoms. But, as the prostate grows larger over time, it can block urine flow through the urethra, and that can cause problems.

Common benign prostatic hyperplasia symptoms include a frequent or urgent need to urinate, difficulty starting urination, weak urine stream, a stream that stops and starts, increased urination at night or inability to completely empty the bladder.

What Are The Risks Of Benign Prostatic Hyperplasia Surgery

Surgery for benign prostatic hyperplasia is common and generally very safe. However, there are risks with any surgery. After the procedure you might have the following issues:

  • Frequent urination or difficulty urinating, painful urination.
  • Blood in the urine and/or in semen.
  • An inability to hold urine .
  • Infection or symptoms resembling a urinary tract infection.
  • Sexual dysfunction.
  • Scar tissue.

In general, side effects from vapor ablation are milder than with other forms of surgery to correct BPH.

How Water Vapor Therapy Works

Convective Radiofrequency Water Vapor Thermal Therapy with ...

This minimally invasive procedure to remove excess prostate tissue is conducted in your doctors office, under local anesthesia. Your urologist will use a special instrument inserted through the urethra. It quickly turns water into steam, and its this thermal energy that shrinks enlarged prostate tissue. The tissue is destroyed in mere seconds and the process does so without harming healthy nearby tissue. During your bodys natural healing response to the procedure, the destroyed tissue is removed and eliminated, allowing the urethra to more fully open, and thus, eliminating your BPH symptoms.

How Is Rezm Performed

Dr Clavell performs the Rezm therapy in the clinic or outpatient setting. The doctor performs a cystoscopy to evaluate the inside of the urinary channel. With a hand-held device, Rezm applies radio-frequency energy to a few drops of water to create vapor . That water vapor is then injected into the prostate tissue that is blocking the flow of urine from the bladder. The steam releases the energy into the cell membranes and will gently and immediately damage those cells by causing cell death. Over time, your body will absorb the treated prostate tissue and the urinary channel will re-open, relieving you of your urinary symptoms.

What To Expect After Rezum

Patient Testimonial: Rezm Water Vapor Therapy for Enlarged Prostate

After the Rezum procedure, the body creates an inflammatory response, which takes two to four months to completely resolve. Initially, there will be swelling in the prostate, which can cause restriction of flow and cause more difficulty and frequency of urination.

Symptoms will get worse before they get better. Generally, at two weeks symptoms begin to stabilize. At four weeks, patients are back where they were before the procedure, and at six weeks patients start to get off medication.

Erections are not affected by the Rezum procedure. There is a one to two percent chance of retrograde ejaculation, which occurs when semen enters the bladder instead of the penis, causing very little to no semen to be ejaculated. This is a significantly lower percentage than in other prostate procedures.

How Does It Work

Rezm uses radiofrequency to heat water into vapor , which is then injected directly into prostate. Vapor turns into water on contact with the prostate tissue, creating controlled damage to the cells that cause obstruction, discomfort, and symptoms associated with BPH. Over time , your body will absorb and remove the treated tissue through the natural healing response.

The Day After Surgery And For The First Week:

  • You can walk, but go gradually by alternating periods of activity of 15-30 minutes with rest periods.
  • Avoid lifting heavy objects for ONE week.
  • Avoid intense exercise or physical activity for ONE week.
  • Avoid having sexual activities for ONE week.
  • Drink plenty of water, to allow small clots to drain well from the bladder and to have clear urine about 8 ounces every 2 hours during the day and stop 2-3 hours before bedtime.
  • The catheter is usually removed after approximately 7 days at the clinic or at the CLSC.
  • A follow-up appointment is planned after 2 months, with an ultrasound of the bladder.

Risks And Benefits Of Rezm Water Vapor Therapy

How Does Rezm Water Vapor Therapy Work

The goal of Rezm is similar to other BPH treatments the removal of excess tissues and the use of hot vapour as a tool can offer the desired results with minimal side effects and a relatively low risk of complications.

The procedure can be performed in our clinic during a short visit. No general anaesthesia is necessary and most men are able to return to regular activities within a few days.

BPH symptom reduction is expected within a month. Longer term studies have demonstrated a sustained symptom relief of over up to five years.

What Are The Alternatives To Rezum

Home treatment, such as double-voiding can help with BPH.

Alcohol and caffeine can make the body want to get rid of excess water, resulting in more frequent urination.

Some medications, such as over the counter decongestants and nasal sprays, antihistamines, and allergy medication can make it more difficult to urinate.

You can check with your doctor or pharmacy if any of those are affecting you and if there is an alternative.

If home treatment doesnt help, you might like to try enlarged prostate medication to reduce your symptoms.

If you stop the medication though, your symptoms will return. Finally there is the option of surgery which will remove some of your prostate.

Not many men have BPH of this magnitude, meaning Rezum could very well be the perfect cure for most incidences of BPH.

Limitations And Further Limitations

While there has been a recent surge in studies on Rezum, which all deliver promising early results, there is a lack of randomized trial data. Future studies, which compare against other standard BPH surgeries are warranted as at present only one sham study exists in world literature. The Comparing uroLift Experience Against Rezum study, a randomized trial, is currently ongoing. Caution should also be noted when interpreting these early results as in the setting of both pilot studies and clinical trials, inclusion/exclusion criteria can be very strict when in reality the demographics of patients who present in clinic and require surgical intervention for BPH are typically much broader. Standardized evaluation parameters will aid future evaluation of Rezums efficacy. To date, no studies have formally assessed the learning curve for the procedure. However, expert opinion has highlighted that it is straight forward for those with sufficient endoscopic experience.
