Active Surveillance For Prostate Cancer

HIFU prostate cancer treatment offers non-invasive alternative

Our active surveillance process uses sophisticated prostate MRI imaging to analyze the makeup of your tumor and provide a personalized risk assessment. This process is used for small, nonaggressive tumors that have a low probability of impacting your health and are not likely to grow or spread.

Your doctor may recommend active surveillance and periodic imaging tests to monitor your prostate cancer and determine whether treatment is needed. Active surveillance includes a prostate-specific antigen test every six-to-12 months and a prostate MRI every one or two years. A biopsy may be taken at the time of your prostate MRI as well.

About Advanced Prostate Cancer

Prostate Cancer is the most common type of cancer among men. While many cases of prostate cancer can be treated and cured upon early detection, sometimes its not that easy.

If your prostate cancer was detected late or has returned after receiving treatment, we at Urology Specialists of the Carolinas can provide uninterrupted, high-quality treatment and care through our Advanced Prostate Cancer Clinic.

What Exactly Is Hifu

High-intensity focused ultrasound is a treatment that involves killing cancer cells by heating them to temperatures at which they cannot survive. Its not exactly surgery and it doesnt involve radiation. The HIFU beam specifically focuses on the cancer tissue and kills it without affecting areas outside the prostate and without causing damage to the healthy tissue of the prostate.

HIFU is a process that involves putting a transrectal probe up the rectum, from which the ultrasound waves can be aimed at the prostate. Your surgeon can see on a screen precisely where he or she is aiming the beam, which will be at the cancerous tissue. This treatment may be a good option for those who have a lower grade of prostate cancer and one that has not spread outside the prostate.

Genetic Counseling For Prostate Cancer

While doctors dont know exactly what causes prostate cancer, certain factors are linked to increased risk. About 5 to 10% of prostate cancers are hereditary, meaning genes that run in your family can lead to cancer.

Genetic counseling can help you find out if your genes raise your risk of prostate cancer and make informed decisions about your health. If you have prostate cancer, genetic tests may change or influence your treatment plan.

Prostate Cancer Patients Are Paying Hefty Price For Controversial New Treatment

Prostate cancer surgery: About the early stage

Men hoping to avoid some side effects of prostate cancer treatment are shelling out tens of thousands of dollars for a procedure whose long-term effects are unknown and insurers, including Medicare, wont pay for.

Proponents say high-intensity focused ultrasound can have fewer negative side effects than surgery or radiation, while giving some patients another option between actively watching their cancer and those more aggressive steps. Critics, however, say the procedure is being oversold, leading some patients to get a treatment they dont need.

Device makers are busy selling the $500,000-and-up machines to doctors around the country and offering training courses. Billboards advertising this new non-invasive treatment for prostate cancer are springing up, while treatment center websites promise a safer method with benefits such as no erectile dysfunction and no incontinence, although studies show those side effects can occur, but less often than with other types of more aggressive treatments. The treatment can range in cost from $15,000 to $25,000.

HIFU is the latest treatment to prompt concerns over whether there should be limits such as requiring tracking of results placed on expensive new technology while additional data is gathered.

In the U.S., advisory committees to the Food and Drug Administration twice turned down applications from manufacturers to market HIFU devices as a treatment for prostate cancer, citing not enough long-term evidence.

New Prostate Cancer Treatment Available In Charlotte

CHARLOTTE, NC – A new tool that’s considered one of the most significant advancements in prostate cancer treatment is now being used in Charlotte. It could literally mean the difference between life and death for many men.

The tool is called Prostate MRI and it’s much like a mammogram used to diagnose breast cancer in women.

According to Dr. Chris Teigland of Charlotte’s Levine Cancer Institute, ” Where we’re actually imaging prostates like mammograms used to image breasts. We’re imaging prostates and we’re only biopsying those lesions where they look like they’re going to be clinically significant cancers.”

Dr. Teigland is considered one of the leading prostate cancer experts in the United States.

He says the new trend in prostate cancer has moved toward trying to identify those patients who have clinically significant disease that needs to be treated, and trying not to treat those who don’t need to be treated.

“The reason for that is that we’ve learned that a lot of the early low grade cancers that we were diagnosing in the 90s and 2000s really could be watched,” Teigland said. “So we no longer operate on those very low risk or low risk patients. So we’re only operating on more clinically significant cancers and watching the low risk cancers.”

In another significant development, newly developed genetic analysis is allowing doctors to determine if a cancer is clinically significant.

It is recommended that any man 40 years or older have his prostate checked.

Dr Peretsman And Urology Specialists Of The Carolinas

Charlotte has played an important role in advocating for the approval of HIFU for more than ten years. An HIFU device called Sonablate was made with the collaboration of Charlottes Steve Puckett Sr. from SonaCare Medical and an Indiana-based company. Together, they were able to start clinics to perform HIFU outside of the country. Formerly, patients could only get HIFU done in clinical trials in the United States, which SonaCare Medical paid for.

With the FDAs approval, SonaCare Medical cant specifically advertise that HIFU is for prostate cancer treatment, but surgeons are allowed to conduct HIFU treatments for prostate cancer. Sonablate is officially FDA-approved and devices are already being installed around the country. Charlotte urologists including Dr. Peretsmans Urology Specialists of the Carolinas group own part of the HIFU FDA-approved device at a clinic in Huntersville, just north of Charlotte.

Dr. Peretsman learned about HIFU nearly ten years ago and learned the procedure so that he could better advise his patients about it. Many prostate cancer patients were traveling out of the country to have the procedure done, but upon returning home, couldnt find doctors with enough knowledge about the procedure to have adequate follow-up care. Dr. Peretsman is now among the most skilled and knowledgeable doctors to perform HIFU, and will even conduct trainings for other doctors in the country on how to perform the procedure.

How Long Is Radiation Treatment For Prostate Cancer

Radiation treatment for prostate cancer generally lasts for one to two months, at a frequency of about five days per week. External beam radiation usually takes less than one hour per session. The total number of treatments needed depends on individual factors, including how large the prostate cancer is, whether the cancer has spread to other parts of the body, and the patients overall health.

Hifu Treatment For Localized Prostate Cancer Now Available In Encino Ca

HIFU, or High Intensity Focused Ultrasound, is a minimally invasive, outpatient treatment for prostate cancer

CHARLOTTE, NC / ACCESSWIRE / December 6, 2019 / HIFU Prostate Services, LLC, the leading provider of HIFU technology to physician practices in the United States, has partnered with seven urologists in the greater Los Angeles areas to bring HIFU for localized prostate cancer to Encino, CA.

The doctors bringing this technology to the area are Richard David, MD Ganeshalingam Devendra, MD Michael Hyman, MD S.Daniel Niku, MD Shashi Reddy, MD Mark Sender, MD and Richard Shapiro, MD.

HIFU, cleared by the FDA for prostate tissue ablation in October 2015, is a non-invasive treatment that uses sound waves to heat and destroy cancerous tissue in the prostate. It is a one-time, outpatient procedure that allows men to return to their normal lifestyle within a few days without the complications commonly seen with surgery and radiation.

The physicians are offering HIFU at Encino Plaza Surgical Center and Dr. Davendra treated two patients with HIFU earlier this week.

During HIFU, ultrasound energy is emitted through a transrectal probe that focuses the energy to a focal point in the prostate to rapidly heat and destroy tissue. HIFU is very accurate and therefore, areas outside the targeted tissue are undamaged which greatly reduces the risk of side effects such as urinary incontinence and erectile dysfunction.

While he did so, the United States held off on legalizing this treatment, waiting to see its results in the more than forty other countries that had already approved HIFU for cancer treatment.

However, in October 2015, HIFU was approved by the FDA for use. Dr. Peretsman, a urologist in Charlotte, was among the first to perform the surgery on a prostate cancer patient in early December in the United States.
