How Does Hifu Help Patients Get Rid Of Bone Metastases
In the fourth stage of cancer, tumour cells penetrate the bones. Because of this, the person feels a lot of pain. The Journal of Vascular and Interventional Radiology has published an article in which doctors said that 67% of patients with metastases in the bones have decreased pain. In HIFU-therapy under MRI control, 87% of patients experience complete relief from pain. This allows them to do their daily tasks and improves their quality of life.
The main advantage of HIFU over cryotherapy and radiofrequency ablation is that it does not require surgery and is under local anaesthesia. Compared to radiation therapy, HIFU does not cause problems with the gastrointestinal tract.

Ultrasound therapy destroys cancer cells in the prostate
Using high energy ultrasound beams to destroy prostate cancer tumours may be as effective as surgery or radiotherapy, but with fewer side effects.
A new study, carried out at six hospitals across the UK, tracked 625 men with prostate cancer who received a type of treatment called high-intensity focused ultrasound .
The research, published in the journal European Urology, is the largest ever study of HIFU treatment used to target prostate tumours. The treatment is similar to a lumpectomy for other cancers where doctors remove only tumour cells, leaving as much healthy tissue as possible.
The findings, from a number of institutions including Imperial College London, Imperial College Healthcare Trust and University College London, found that after five years the cancer survival rate from HIFU was 100 per cent. Approximately, 1 in 10 men needed further treatment. The cancer survival rate from surgery and radiotherapy is also 100 per cent at five years.

The research also showed the risk of side effects of HIFU, such as urinary incontinence and erectile dysfunction, were lower than other treatment options, at 2 per cent and 15 per cent respectively.
Prostate cancer is the most common cancer in men in the UK, with around 47,000 cases every year.

Continence And Sexual Function Preservation
Multicenter study conducted by the French Association of Urology showed optimal preservation of continence for 97% of patients and erectile function for more than 78% of them. In another study carried out by a medical team from Institut Bordet in Brussels , 80% of patients retained their sexual function, while more than 94% maintained perfect continence.

Does Hifu Help Cure Primary And Secondary Liver Cancer
HIFU-therapy is recognized as an effective method for treating inoperable liver cancer. 44% of patients live for 5 years, if the cancer process was diagnosed at the 1st or 2nd stage, provided that the liver cancer was treated with chemotherapy or radiotherapy. The researchers at the Journal of Cancer found that 79% of patients successfully undergo HIFU sessions, after which the tumour begins to shrink. In 30% of cancer patients, the tumour is destroyed.
The disadvantage of HIFU is that 8% may have complications. These include obstruction of the biliary tract, pleural effusion, fistula formation, as well as burns, pain and fever.
Treatment with HIFU-therapy for liver cancer is possible in European clinics.
Predictions Of Pancreatic Cancer Treatment With Hifu
More than 80% of patients with pancreatic cancer have advanced disease and are not suitable for surgery. Standard treatment options for non-surgical pancreatic cancer include chemotherapy, radiation therapy, or a combination thereof.
The inclusion of HIFU therapy in this list can extend the life of the patient. At the same time, 81% of cancer patients have severe abdominal pain. The researchers called HIFU a safe and effective treatment for pancreatic cancer.

New Study Suggests That High
For some prostate cancer patients, radical treatment are treatment standards. However, these procedures may cause side effects including urinary incontinence or impotency.
A new study from USC Urology with Keck Medicine of USC demonstrates that focal high-intensity focused ultrasound ablation of the prostate is an effective alternative to surgery or radiation, with encouraging outcomes and shortened recovery time.
Focal HIFU ablation is an outpatient procedure that uses a focused ultrasound beam to raise the temperature inside the prostate to approximately 90 degrees Celsius to destroy targeted areas of prostate tissue. The procedure takes around two hours and the patients are often discharged home the same day.
The study, published in The Journal of Urology, followed 100 men in the United States who underwent a HIFU procedure for prostate cancer between 2015-2019. This is the first and largest study examining the outcomes of focal HIFU ablation as a primary treatment for prostate cancer in the United States.
During follow-up, 91% of HIFU patients successfully avoided radical treatment. Also, 73% of patients did not experience treatment failure, which the researchers defined as clinically significant cancer recurrence, metastases or mortality, or the need for additional hormone therapy, chemotherapy, surgery or radiation.

First Us Study Shows Promising Outcomes For High
High-intensity focused ultrasound , a technology used to treat localized prostate cancer, has shown adequate control of prostate cancer while avoiding major side effects of surgery or radiation therapy, according to a new study in The Journal of Urology, Official Journal of the American Urological Association

With this non-invasive high-intensity focal ultrasound strategy, nearly 90 percent of men with localized prostate cancer were able to avoid or delay radical treatment , suggests the study by Andre Luis Abreu, M.D., of University of Southern California, Los Angeles, and colleagues. They write, Focal HIFU ablation is safe and provides excellent potency and continence preservation with adequate short-term cancer control.

Who is Eligible for HIFU Treatment for prostate cancer?Eligible patients have low-to-intermediate risk prostate cancer, with prostates that are not too enlarged.
UCLA Expertise in HIFU TreatmentUCLA Urologists, Drs. Leonard Marks and Allan Pantuck, were the study investigators for the first U.S.-based HIFU clinical trails that began in March 2009. UCLA became the lead enroller nationwide for that study. What Is The Success Rate Of Hifu For Treatment Of Prostate Cancer
Over 90% of patients undergoing HIFU therapy will not require further treatment for their prostate cancer. In those developing a recurrence, they remain candidates for surgery, radiation or hormone therapy. HIFU treatment has a similar success rate to radical prostatectomy but has the major advantage of using non-invasive technology.
How Does Hifu Treat Prostate Cancer
AHIFU heats and destroys cancer cells in the prostate. Pulses of ultrasound waves travel through the wall of the back passage into the prostate gland and are focused onto a small area. This ultrasound energy causes the prostate cells to heat up. This heat destroys normal prostate cells and cancer cells. The surgeon is able to focus the treatment on the prostate gland and avoid damaging other nearby organs.

Less Tissue Destruction With Hifu
At the point where HIFU is focused, intense absorption of ultrasound waves creates a rapid temperature increase in the tissue, which destroys cells in the targeted zone. By precisely moving the focal point of each wave, it is possible to destroy a volume of tissue without damaging surrounding tissue.
As with any therapy, there are some side effects with HIFU. Urinary tract infection, scar tissue formation and erectile dysfunction can occur. The rate of cancer recurrence has been shown in some studies to be higher with HIFU and some patients go on to receive a second treatment, including repeat HIFU, radical prostatectomy or radiation.
Because the ultrasound is delivered to the prostate transrectally, the most feared complication is rectal injury due to the high heat moving through the rectum. While there are safety mechanisms in place to monitor and control for rectal temperature, the impact of HIFU on the posterior plane between the rectum and prostate may make second-line therapy after HIFU more challenging.
Several centers are using HIFU in the salvage setting, Scarpato said. I believe its a reasonable approach following radiation. The reason I hesitate to use HIFU for salvage therapy, at least initially, is that the tissue has already been subjected to some therapy and the tissue quality is likely compromised this may be associated with a higher risk of a rectal injury.

Salvage External Beam Radiotherapy After Hifu Failure In Localized Prostate Cancer: A Single Institution Experience

Purpose/objectives: High-intensity focused ultrasound remains investigational as primary treatment for localized prostate cancer but is sometimes offered to select patients. At HIFU failure, data guiding salvage treatment is limited to small retrospective series with short follow-up. We evaluated our institutional experience using salvage radiation therapy after HIFU failure.
Materials/methods: We conducted a retrospective analysis of patients with local failure post-HIFU who received salvage image-guided external beam radiation therapy delivered via intensity-modulated radiotherapy . Our primary endpoint was biochemical failure-free survival defined as prostate-specific antigen nadir + 2 ng/mL. Secondary endpoints included metastasis-free survival and overall survival . Endpoints were evaluated using Kaplan-Meier analysis.
To our knowledge, we report one of the largest series on contemporary SRT post HIFU failure. We show that SRT is feasible, effective and carries no additional acute or delayed toxicity.

The Sonablate Hifu Procedure
The HIFU procedure typically lasts between 1-4 hours, depending on the size of the prostate, and is either done under general or epidural anesthesia. .Dr Gange was trained by the two leading HIFU surgeons in the world and began performing HIFU in 2007 taking his patients outside of the US . Finally, FDA clearance in late 2015 paved the path for treatment in the US after some logistical hurdles, Dr. Gange now offers HIFU treatment for prostate cancer patients in Salt Lake City. Meanwhile, Dr. Gange has performed more HIFU procedures than any urologist in the Intermountain West. During the procedure, a suprapubic or foley catheter will be put into place to help with urination, and will typically stay in place for 1-3 weeks.
After the HIFU treatment, patients will be in recovery for about 2-4 hours. Once released from the treatment center, patients return home or to a hotel to recover more comfortably.
Though it is recommended that patients take things easy, most patients feel well enough to go out for dinner and maybe even a walk, the day of the procedure.
For early to intermediate risk cancer, HIFU therapy is equal to radical surgery in terms of cancer control. And it offers a much more favorable side effect profile. Primarily, minimal to no incontinence risks, and a minimal impact on erectile function. Steven Gange, MD
