What Is a Normal PSA for a Man Without Prostate Cancer? | Ask a Prostate Expert, Mark Scholz, MD

The tissues lining the prostate and rectum are delicate and vulnerable to cuts, tears, and abrasions. Massaging the prostate too intensely can easily lead to soreness. In the study described above, for example, 8.3% of participants reported discomfort after prostate massage.

Overly aggressive prostate massage can also cause rectal bleeding, creating a risk of bacterial infection or aggravating hemorrhoids.

Furthermore, manual prostate massage is discouraged for men with acute bacterial prostatitis. The massage increases inflammation and may promote the spread of bacteria to the urethra, other parts of the urinary tract, and the bloodstream.

That said, a few small studies of the effects of prostate massage performed by a physician have shown it to be beneficial as a therapy for chronic prostatitis when paired with antibiotics.

What Is The Prostate

The prostate is a part of the male reproductive system, which includes the penis, prostate, seminal vesicles, and testicles. The prostate is located just below the bladder and in front of the rectum. It is about the size of a walnut and surrounds the urethra . It produces fluid that makes up a part of semen.

As a man ages, the prostate tends to increase in size. This can cause the urethra to narrow and decrease urine flow. This is called benign prostatic hyperplasia, and it is not the same as prostate cancer. Men may also have other prostate changes that are not cancer.external icon

Is Sex Good For Your Prostate

Whether or not sex is good for prostate problems is a question many men wonder, and its an important one that needs answering.

The prostate plays an active role during sexual activity.

Along with other structures, the prostate is responsible for making fluid in semen, which helps transport your sperm when you ejaculate.

The effect regular ejaculations have on the prostate should become a significant talking point, especially amongst men who are experiencing prostate problems.

This article will determine whether a healthy sex life can reduce the risk of developing prostate cancer.

Symptoms Of Prostate Disease

In its earliest stages, prostate disease may or may not be associated with symptoms. The symptoms of prostate disease depend on the condition, but may include:

  • difficulties urinating, such as trouble starting the flow of urine
  • the urge to urinate often, particularly at night
  • feeling as though the bladder can’t be fully emptied
  • painful urination
  • blood in the urine or blood coming from the urethra independent of urination.

Blood in the urine is often due to causes not related to the prostate. Always see your doctor if you find blood in your urine.

Conditions Prostate Massage Might Help

Although studies are limited, prostate massage may be helpful in some medical situations and with some health conditions.

Prostatitis

Prostatitis is swelling and inflammation of the prostate gland. It can cause symptoms like pain when you pee and pain around your groin and pelvis.

A doctor might do a prostate massage to diagnose the condition. They can test the fluid that comes from your prostate ducts to see whatâs causing the problem.

Although the most common treatments for prostatitis are antibiotics and non-steroidal anti-inflammatory drugs , prostate massage is another option. It can help ease pressure and swelling by releasing fluids that build up in the prostate. Small studies have found that massaging the area several times a week — along with taking antibiotics — can give relief from pain and pressure.

Diagnosing a Medical Condition

Sometimes a doctor might do a prostate massage during a prostate exam. The purpose is to get some fluid to examine under a microscope to look for signs of an infection or other problem.

Erection Problems

The main treatments for erectile dysfunction include medications, pumps, shots, and implants. Lifestyle changes that can help treat the condition include getting more exercise, eating a healthy diet, and doing pelvic floor exercises. Some men say they’ve had increased sexual pleasure and are able to ejaculate more easily with prostate massage, though there isnât scientific proof to back up those claims.

Treatment For Prostate Disease

Treatment for prostatitis may include antibacterial drugs and supportive treatments, depending on the type of prostatitis.Treatment for BPH may include medications to relax the smooth muscle of the gland or to shrink the size of the prostate, and surgery to produce a permanently widened channel in the part of the urethra that passes through the prostate.Treatment for prostate cancer is tailored to suit individual circumstances. The nature of the cancer, other health problems the person may have, and their wishes will all be taken into account.Management approaches for prostate cancer include:

  • active surveillance
  • surgery for example, prostatectomy
  • radiotherapy
  • ablative treatments such as high-intensity focused ultrasound and NanoKnife®
  • hormone treatment
  • chemotherapy

Is A Prostate Massage The Same Thing As Trying For A Prostate Orgasm

What is the prostate? | Ask a Nurse

Yes. If you massage the prostate in a way that feels good, youre probably going to have an orgasm if you keep at it.

Some doctors recommend prostate massage therapy to help relieve the symptoms of certain conditions, such as painful ejaculation prostatitis.

Yep! Though evidence on the efficacy of prostate massage for certain conditions is somewhat limited, it does appear to have some benefits.

Medication For Urinary Problems

Your doctor may suggest various medications to help ease your urinary problems, including:

  • medications to reduce the tone of the muscles of the urethra and prostate to minimise any constriction to urine flow caused when these muscles contract
  • medication to reduce the size of the prostate gland. These medications work by blocking the action of male hormones produced by the prostate gland
  • medications to relax the bladder, making unwanted contractions less likely and reducing the symptoms of urgency and frequency of urination
  • the over-the-counter preparation ‘saw palmetto’ is sometimes used. This may help some men, especially if frequent urination at night is a problem.

However, recent reviews of the evidence for using saw palmetto as a treatment for mild or moderate urinary symptoms did not show any improvement, compared to no treatment, in men with BPH.

How Does The Prostate Work

The prostate is a gland about the size of a chestnut and weighs about 30 grams . It is part of the male reproductive system and is located inside the body. The prostates most important function is the production of a fluid that, together with sperm cells from the testicles and fluids from other glands, makes up semen. The muscles of the prostate also ensure that the semen is forcefully pressed into the urethra and then expelled outwards during ejaculation.

The prostate is located directly below the bladder and above the muscles of the pelvic floor. The rectum is behind the prostate, making it possible to feel the gland from the rectum using the finger. The ducts in the prostate gland flow into the urethra, which passes through the prostate. The word prostate is taken from the Greek expression meaning one who stands before, which describes the position of the prostate gland. Viewed from below, where the urethra leaves the gland, the prostate stands before the bladder.

The tissue of the prostate gland can be divided into three different zones, listed here from innermost to outermost, which encircle the urethra like layers of an onion:

The prostate has various functions:

Production of fluid for semen:

Hormone metabolism: In the prostate the male sex hormone testosterone is transformed to a biologically active form, DHT .

What Does The Prostate Gland Actually Do

The prostate gland isn’t essential for life, but it is vital for reproduction and is part of the male reproductive system.

The function of the prostate is to produce a slightly alkaline fluid called the prostatic fluid that makes up part of the seminal fluid, or semen.

The rest of semen is composed of sperm cells from the testicles, fluid from the seminal vesicles, and secretions from the pea-sized bulbourethral gland.

The prostatic fluid contains substances that are important to the functioning and survival of sperm cells, such as the enzyme prostate-specific antigen , which thins or loosens up semen, helping the tadpole-like sperm cells swim freely to reach the egg.

Can Prostate Cancer Be Prevented

There are no clear prevention strategies for prostate cancer. There is some conflicting evidence that a healthy diet composed of low fat, high vegetables and fruits may help reduce your risk of prostate cancer. Routine screening, with PSA blood test and physical exam, is important to detect prostate cancer at an early stage. A healthy diet and regular exercise are also critical in maintaining good health and preventing disease in general.

When To Contact A Medical Professional

  • Less urine than usual
  • Back, side, or abdominal pain
  • Blood or pus in your urine

Also call if:

  • Your bladder does not feel completely empty after you urinate.
  • You take medicines that may cause urinary problems, such as diuretics, antihistamines, antidepressants, or sedatives. DO NOT stop or change your medicines without talking to your provider.
  • You have tried self-care steps for 2 months and symptoms have not improved.

What Does The Prostate Gland Do And Why Is It Important

What is prostate cancer? | Cancer Research UK

The prostate is responsible for producing prostatic fluid, which is full of enzymes, proteins, and minerals that are vital for the proper functioning of sperm.

During climax, smooth muscle fibers of the stroma contract, forcing the prostatic fluid through the ejaculatory ducts and into the urethra.

In the urethra, the prostatic fluid mixes with fluid from the seminal vesicle, sperm from the testes, and secretions from the bulbourethral gland, a pea-sized gland below the prostate.

In all, prostatic fluid makes up 20 to 30 percent of semen .

Aside from producing prostatic fluid, the prostate is responsible for closing up the urethra during ejaculation so that semen doesn’t enter the bladder. During urination, the muscles of the central zone close the prostate’s seminal ducts so that urine cannot enter the prostate.

Arazo Nutrition Prostate Support

Arazo Nutrition goes heavy on antioxidant ingredients to cut down on systemic inflammation in this prostate supplement.

It uses a blend vaguely reminiscent of a superfood supplement, with tomato extract, broccoli, green tea, and red raspberry among its ingredients.

It does include saw palmetto and beta sitosterol, two well-studied prostate health supplements, but the dosage isnt great. Its a decent choice if you think inflammation is largely to blame for your prostate issues, however.

Clinical Relevance Prostatic Carcinoma

Prostatic carcinoma represents the most commonly diagnosed cancer in men, especially in countries with high sociodemographic index. The malignant cells commonly originate from the peripheral zone, although carcinomas may arise from the central and transition zones too. It is still debatable that the latter tumors may present with lower malignant potential.

However the proximity of the peripheral zone to the neurovascular bundle that surrounds the prostate may facilitate spread along perineural and lymphatic pathways, thus increasing the metastatic potential of these tumors. Malignant cells may invade adjacent structures and/ or lymphatic and blood vessel routes to give distant metastases. Prostate carcinoma also commonly spreads via the Batson venous plexus to the vertebral bodies and cause skeletal metastases.

A DRE may reveal a hard, irregular prostate gland. In most cases the serum PSA values will be increased. However, due to the peripherally-advancing tumor, symptoms may be minimal, as obstruction occurs usually at late stages. One should also keep in mind that the high incidence of prostate carcinoma is found in elderly men, who may already have symptoms due to BPH.

How Radical Prostatectomy Affects A Relationship

A 2011 study of 63 men that had undergone a radical prostatectomy found that about 75 percent of them sought treatment for erectile dysfunction. Additionally, more than 50 percent reported having less sexual desire, and roughly an additional 40 percent were unable to have a satisfying orgasm.

The mental health effects of these symptoms were worse in highly sexually motivated participants. 52 percent reported that this had affected their self-esteem, and 36 percent said having performance anxiety.

Additionally, the last three items on the list above can be show-stoppers for a relationship. Few women can tolerate a high degree of urine leakage during sexual activity, and few men can handle the pain during intercourse.

Prostate Cancer And Sexual Health

Men and Prostate Cancer: Fact Vs. Fiction

A prostate cancer diagnosis can leave you feeling anxious, frustrated, and depressed.

It may also leave you wondering about your ability to have sex. You may also be questioning how prostate cancer treatment can affect your sex life.

Prostate cancer rarely causes problems with sex, but the treatments for prostate cancer often affect penile function and reproduction.

Prostate surgery and radiation are the two most common treatments for prostate cancer. Both of these can affect the nerves in your penis and could cause sexual problems like erectile dysfunction .

Cryotherapy and ultrasound therapy can also cause ED. Your doctor may recommend hormone therapy or radiation therapy, as well. This therapy can lower your sex drive.

Radical prostatectomy is the most effective nerve-sparing surgery for prostate cancer. It can substantially limit the duration of post-surgical ED but requires a surgeon to be highly skilled and trained in the process.

In this type of procedure, the surgeon takes care not to damage the erectile nerves that surround the prostate as it is removed.

However, if cancer has infiltrated these nerves, it may not be possible to save them.

Preventing injury to these nerves dramatically improves the chances of a man recovering erectile function within two years of the surgery.

In addition to ED, you may also find that your orgasms are different after undergoing treatment for prostate cancer.

They may not feel as strong, and you will not see as much ejaculate.

What Does A Prostate Orgasm Feel Like

P-spot orgasms are said to feel similar to penile orgasms, only way more intense and felt through the entire body.

There are reports of people having super orgasms, which are a stream of fast, continuous orgasms that cause the body to shudder.

Not everyone ejaculates during a prostate orgasm, but some release a dribble of milky fluid from the urethra.

A little bit of prep can help make the experience a good one whether youre flying solo, or on the giving or receiving end of things.

Other Possible Side Effects

Anorgasmia is the inability to achieve orgasm regardless of the level of stimulation.

2) Penile shrinkage

Removal of the prostate seems to initiate a phase of penile shrinkage that varies between 2 to 3 cm. For men with large organs, this may have little effect. But a man that starts on the small side might be very dismayed about it.

Early penile rehabilitation may avoid structural endothelial and smooth muscle damage by improving penile oxygenation.

3) Urine leakage or pain during intercourse

4) Urine leakage at orgasm

5) Pain or discomfort at orgasm

Helping To Produce Semen

2030% of fluid to the total semen volume. The remainder comes from the seminal vesicles and the testicles .

Prostatic fluid contains components that make semen an ideal substance for sperm cells to live in, including enzymes, zinc, and citric acid. One important enzyme is prostate-specific antigen , which helps make the semen thinner and more fluid.

The fluid in semen helps the sperm travel down the urethra and survive the journey towards an egg, which is essential for reproduction.

Prostatic fluid is slightly acidic, but other components of semen make it alkaline overall. This is to counteract the acidity of the vagina and protect the sperm from damage.
