Some patients have full urinary control as soon as the catheter is removed. Most patients experience some urinary leakage, which clears up in an average of three weeks but can take as long as 18 months. Small incontinence pads may be worn during this time to absorb any leaking urine and protect your clothing. For one out of 20 men, leakage may persist longer or even be permanent.
We define incontinence as needing more than two pads a day, says Dr. Mohler. Four out of five men wont need any pads at all. One in five will need one to two mini pads per day. One in 100 will end up needing more than two pads per day. This has a devastating effect on quality of life for these men. We dont fully understand why this happens, and we cannot predict who will have this experience. How Does The Robotic
It is similar to the conventional, minimally invasive laparoscopic prostatectomy. However, working from a special console in the operating room, the surgeon operates four precision-guided robotic arms to cut and remove the prostate. The procedure uses a small video camera, inserted through a keyhole-sized incision to provide surgeons with magnified, 3-D images of the prostate site. This expansive view allows doctors to see the nerve bundles and muscles surrounding the prostate. The robotic arms, with full 360-degree rotation capabilities, are placed through several other keyhole incisions allowing surgical instruments to move with greater precision, flexibility and range of motion than in a standard laparoscopy.
The procedure usually takes 2 to 3 hours under general anesthesia. Most patients experience only a small blood loss and blood transfusions are needed in less than one percent of patients. Prostatectomy patients typically spend one night in the hospital and are usually discharged as soon as their laboratory tests are acceptable, pain is controlled and they are able to retain liquids. Patients are discharged with special catheter, which is removed during an outpatient visit 5-7 days after the operation.

Types Of Prostate Surgery
There are several ways of removing the prostate keyhole surgery either by hand or robot-assisted, and open surgery.
Although robot-assisted keyhole surgery is the newest technique, the most recent research suggests all three techniques are as good as each other for treating prostate cancer, as long as the surgeon is experienced. They also have similar rates of side effects.

Your surgeon controls the tools from a console in the operating room via four or five robotic arms. Although its called robot-assisted, its still a surgeon who does the operation. You may hear the equipment called the da Vinci® Robot.
- Keyhole surgery by hand As with robot-assisted keyhole surgery, the surgeon will make four or five small cuts in your abdomen. But they will hold the surgical tools in their hands, rather than using robotic arms.

- Most men feel better within the first week following surgery however, do not lift, push or pull any heavy objects until after your doctor says it is OK.
- Full recovery takes at least one month. Remember that it may take up to a year to gain full control of your bladder and to achieve sexual function.

The introduction of prostate-specific antigen testing into clinical practice has greatly increased the detection of localized prostate cancer and, by doing so, has decreased the diagnosis of regional and metastatic disease. PSA testing has had such a profound clinical effect that questions have arisen regarding the significance of the cancers that are being detected.
Stage, grade, tumor volume, and PSA testing are used to determine whether a prostate cancer is clinically significant or insignificant. However, there is no generally accepted precise definition for this distinction.
The goal of early detection of prostate cancer is to identify clinically significant cancers at a time when treatment is most likely to be effective. The risk of death from prostate cancer is significant in those with moderate- to high-grade tumors. This is especially true in younger men. Long-term survival is compromised when the cancer has spread beyond the confines of the prostate, into the regional lymph nodes, and to distant sites.
Several studies have shown that with a PSA cutoff of 4.0 ng/mL, clinically insignificant cancers are detected in fewer than 20% of men, but nearly 50% of all the cancers detected because of an elevated PSA level are localized, and these patients are candidates for potentially curative therapy. Only a small proportion of prostate cancers detected by PSA testing and treated with radical prostatectomy are low-volume and low-grade tumors.

The prostate is a gland located underneath the bladder, in front of the rectum. It plays an important role in the part of the male reproductive system that produces fluids that carry sperm.
Surgery for partial or complete removal of the prostate is called a prostatectomy. The most common causes for prostate surgery are prostate cancer and an enlarged prostate, or benign prostatic hyperplasia .

What Can I Expect Immediately After Surgery
You will come round in an intensive care or high dependency unit. A prostatectomy is a major operation and although laparoscopic techniques significantly reduce the impact of surgery, it is still necessary to monitor you closely after your operation. You will have a drip fitted and receive fluids intravenously until you are able to start drinking adequately, which is usually about 24 hours after surgery. You normally spend one night in intensive or high dependency care before being moved to a general surgical ward.
Good pain management medication means patients often experience little pain or discomfort. You will be encouraged to get up and become mobile as soon as you are able to do so. The nursing staff will keep a record of how much fluid you are drinking because it is important to stay hydrated and pass urine regularly after a prostatectomy. In the first few days after surgery, you are likely to have a little blood in your urine.
Traditionally, patients are fitted with a urethral catheter which is worn for seven to 14 days, which means going home with the catheter in place. It is fitted to enable patients to pass urine while the join between the bladder and the urethra heals. - Urinary incontinence: Some people experience urinary incontinence, although most people recover continence. Your provider can help you manage loss of bladder control and urine leakage.

Adverse Reactions To The Anesthesia
If a person has a history of swelling of the face or generalized itching, it is important to inform the anesthetist before the procedure because, although allergic reactions to anesthesia can be mild, involving only wheezing or some skin irritation, it is also possible to experience an anaphylactic response which can be life-threatening.
Somebody suffering from an allergic reaction to an anesthetic may display signs like breathing difficulties, low blood pressure, rashes, hives and swollen skin around the eyes or mouth and throat.
