What Is The Success Rate Of Hifu For Treatment Of Prostate Cancer

Prostate Cancer – HIFU Treatment

Over 90% of patients undergoing HIFU therapy will not require further treatment for their prostate cancer. In those developing a recurrence, they remain candidates for surgery, radiation or hormone therapy. HIFU treatment has a similar success rate to radical prostatectomy but has the major advantage of using non-invasive technology.

Access To Clinical Trials

Our prostate cancer specialists conduct clinical trials at the IU Health Academic Health Center. One trial involves using PSMA PET imaging and MRI scans to plan HIFU treatments. PSMA PET imaging helps doctors more accurately target and destroy cancerous tissue while saving surrounding healthy tissue and nerves.

Who Can Have Hifu

HIFU will provide literally thousands of men with the opportunity to receive less invasive treatment and retain maximum functionality ongoing.

The advantages of HIFU treatment for prostate cancer are superior to most current alternatives as it is usually a day stay procedure with a short recovery time of one to two weeks.

HIFU is minimally invasive with no surgical requirement . It can also be repeated should your cancer recur, and it does not exclude other treatments if required such as radiotherapy or surgery.

Unfortunately, HIFU is available in very few NHS hospitals right now but you can talk to your GP, consultant or nurse to see if it is available in your area.

Making HIFU available to men across the UK is the main driving force of Prost8 as a charity and our aim is to raise funds and campaign to deploy at least one unit in each of the 14 UK NHS regions.

Our ambition is high as each unit costs in the region of £500,000 but, if successful, our plan will achieve this within the next five years.

However, it is unlikely that HIFU will be offered as an alternative if the cancer has spread outside of the prostate to other parts of the body although HIFU can be used in cases where the cancer has reoccurred after other treatments such as radiotherapy and is offered in these cases .

When Do You Need Hifu Therapy

It is important to know that targeted focal therapy for prostate cancer is not suitable for all patients, the overall state of health and other certain criteria should be judiciously examined.

The doctor can recommend HIFU if:

    • The patient is diagnosed with an initial stage of prostate cancer
    • The cancer is localized i.e. confined to the prostate gland only
    • There is recurrent localized prostate cancer
    • Prostate cancer is limited to one or very few foci in the prostate
    • The Gleason score which is an estimator of the level and grade of tissue change should not be greater than 3+3 or 3+4
    • The PSA value which is an indicator of the existence of prostate cancer is under 15 ng/ml
    • The operation is not an option due to health problems or a personal choice

More than 80% of patients have a multifocal tumor necessitating treatment of the whole prostate therefore HIFU is not possible as well as if prostate cancer has expanded to other parts of the body the treatment is not performed.

How Does Hifu Treat Prostate Cancer

AHIFU heats and destroys cancer cells in the prostate. Pulses of ultrasound waves travel through the wall of the back passage into the prostate gland and are focused onto a small area. This ultrasound energy causes the prostate cells to heat up. This heat destroys normal prostate cells and cancer cells. The surgeon is able to focus the treatment on the prostate gland and avoid damaging other nearby organs.

Faqs About Prostate Cancer Treatment Costs

    • How much does open and robotic prostate surgery cost differ?
    • There are 2 main types of prostatectomy : open and robotic one. The second one is less traumatic, the recovery period is faster, the urinating and erectile functions are preserved in most of the cases. But the cost of robotic prostate removal is about 25-30% higher than the open one. The treatment effectiveness is about the same level.
    • Can a low cost of prostate oncology treatment be a sign of a poor quality or unprofessionalism?
    • Yes, it can, but not in all cases. To be sure in quality of treatment, learn the info about a hospital and a doctor who will manage the therapy. If the medical center has international certificates and positive reviews from patients, you do not need to worry. The lower cost of private treatment for prostate cancer is usually explained by the general price policy in a particular country.
    • What additional charges can occur during prostate cancer therapy?
    • Some hospitals already include such services as a transfer, admission, language assistance in the cost of prostate cancer treating. But some provide a quote without these charges. So you have to specify this before going to treatment. Also, a doctor can prescribe some additional treatment options to increase the effectiveness after the examination in the hospital.

    Less Tissue Destruction With Hifu

At the point where HIFU is focused, intense absorption of ultrasound waves creates a rapid temperature increase in the tissue, which destroys cells in the targeted zone. By precisely moving the focal point of each wave, it is possible to destroy a volume of tissue without damaging surrounding tissue.

As with any therapy, there are some side effects with HIFU. Urinary tract infection, scar tissue formation and erectile dysfunction can occur. The rate of cancer recurrence has been shown in some studies to be higher with HIFU and some patients go on to receive a second treatment, including repeat HIFU, radical prostatectomy or radiation.

Because the ultrasound is delivered to the prostate transrectally, the most feared complication is rectal injury due to the high heat moving through the rectum. While there are safety mechanisms in place to monitor and control for rectal temperature, the impact of HIFU on the posterior plane between the rectum and prostate may make second-line therapy after HIFU more challenging.

Several centers are using HIFU in the salvage setting, Scarpato said. I believe its a reasonable approach following radiation. The reason I hesitate to use HIFU for salvage therapy, at least initially, is that the tissue has already been subjected to some therapy and the tissue quality is likely compromised this may be associated with a higher risk of a rectal injury.

How Does High Intensity Focused Ultrasound Work

HIFU destroys prostate cancer through the delivery of precise and focused sound waves to a targeted spot of diseased prostate tissue. This technology uses ultrasound energy, not radiation, to destroy the targeted tissue. The Ultrasound energy, or sound waves, is transmitted through the rectal wall and focused at desired locations within the prostate identified by MRI and confirmed by ultrasound. This targeted and minimally invasive approach to treating prostate cancer leaves the healthy tissue untouched and unharmed and minimizes the chance of side effects.

The basic concept of how HIFU targets, burns and destroys specific prostate tissue is similar to how a magnifying glass can use the suns rays of light to burn a precise hole in a leaf at the point of intersection. Similar principles apply to HIFU. Instead of using light as the source of energy, HIFU uses sound, and instead of a magnifying glass, HIFU uses a transducer. The sound waves that are aimed at the prostate tissue, rapidly increase tissue temperature, destroying only the cancerous lesions and protecting the healthy surrounding tissue.

Prostate Cancer Patients Are Paying Hefty Price For Controversial New Treatment

Men hoping to avoid some side effects of prostate cancer treatment are shelling out tens of thousands of dollars for a procedure whose long-term effects are unknown and insurers, including Medicare, wont pay for.

Proponents say high-intensity focused ultrasound can have fewer negative side effects than surgery or radiation, while giving some patients another option between actively watching their cancer and those more aggressive steps. Critics, however, say the procedure is being oversold, leading some patients to get a treatment they dont need.

Device makers are busy selling the $500,000-and-up machines to doctors around the country and offering training courses. Billboards advertising this new non-invasive treatment for prostate cancer are springing up, while treatment center websites promise a safer method with benefits such as no erectile dysfunction and no incontinence, although studies show those side effects can occur, but less often than with other types of more aggressive treatments. The treatment can range in cost from $15,000 to $25,000.

HIFU is the latest treatment to prompt concerns over whether there should be limits such as requiring tracking of results placed on expensive new technology while additional data is gathered.

In the U.S., advisory committees to the Food and Drug Administration twice turned down applications from manufacturers to market HIFU devices as a treatment for prostate cancer, citing not enough long-term evidence.

Another type of prostate issue is chronic prostatitis, or chronic pelvic pain syndrome. This condition causes pain in the lower back and groin area, and may cause urinary retention. Symptoms include leaking and discomfort. In severe cases, a catheter may be required to relieve the symptoms. If the problem is unresponsive to other treatments, your doctor may suggest a surgical procedure. If these do not work, your symptoms could progress and become chronic.

An acute bacterial infection can cause a burning sensation. Inflammation of the prostate can affect the bladder and result in discomfort and other symptoms. This is the most common urinary tract problem in men under 50, and the third most common in men over 65. The symptoms of acute bacterial prostatitis are similar to those of CPPS. Patients may experience a fever or chills as a result of the infection.

Indications For Hifu Therapy Of Pc

The most widespread use of HIFU, and initially the only indication for its use, has been in patients with localized PC who are not candidates for surgery because of age, general health status, a prohibiting comorbidity or a preference not to undergo radical prostatectomy . However, with the accumulation of clinical experience and expansion of research protocols, these indications have broadened .

However, the indications have been expanded based on clinical experience to include partial and focal therapy in unilateral low volume, low-Gleason tumors, incidental prostate cancer after trans-urethral resection , salvage therapy in recurrent prostate cancer after radical prostatectomy, radiotherapy, or hormone ablation, for locally advanced prostate cancer as adjuvant local tumor-debulking therapy, non-metastatic as well as metastatic stages, and hormone-resistant prostate cancer . It is well accepted that the gland can also be downsized by 30% within 3 months of androgen deprivation therapy , besides TURP. Still remaining contraindications for the HIFU device are a missing or a small rectum and a damaged rectal wall, caused by previous prostatic/rectal therapies .

    psa reduction treatment by HIFU

    1. HIFU is approved by the FDA for treatment of prostate cancer.Although the high-intensity focused ultrasound procedure was hanging in limbo for several years waiting for regulatory approval, it is now available to men in the United States when they receive a diagnosis of prostate cancer. There are two systems currently available that can treat tumors or destroy diseased tissue, making it a useful option for many men. Medicare has already agreed to cover the treatment with their coverage protocol as well, which should impact its level of availability in the near future.

    2. It doesnât take long to produce the results that you want. The HIFU treatment option takes four hours or less to successfully deal with the prostate cancer. Before you can have it done, your doctor will want you to have an enema to ensure that your bowels are empty. There is also a restriction in place that you cannot eat or drink anything for at least six hours before having the operation.

Some men report that they have trouble urinating after having the HIFU procedure. They might also leak urine between trips to the toilet. There can also be pain between your rectum and testicles that medication can treat. Rare side effects include a UTI, blood in the urine, and a testicular infection.
