An enlarged prostate can also be the cause of other problems. If the enlarged prostate is causing symptoms, the best treatment would be a natural remedy. In the meantime, there are treatments for a wide range of conditions that cause a man to experience pain. A common surgical procedure involves an electric loop, laser, or electro-stimulation. The procedure is a safe and effective option for treating enlarged or symptomatic BPH.

What Treatments Are Used In Focal Therapy For Localized Prostate Cancer

Focal therapy uses ablation, which is the use of extreme temperatures to destroy tissue. In focal ablation, the area of the prostate that contains the index lesion is targeted, rather than treating the entire prostate gland. Focal ablation techniques include:

  • Cryotherapy: The use of very cold gases passed through needles to freeze and destroy cancer tissue.
  • HIFU : The use of high-frequency sound waves directed at the tumor through an ultrasound probe inserted into the rectum. The high intensity waves cause the diseased tissue to heat up and die.
  • : A drug called a photosensitizer is injected into the bloodstream. This drug then absorbs light rays directed at the tumor, and produces an active form of oxygen that destroys cancer cells.
  • Laser ablation: The use of laser radiation energy pinpointed to a very small area to burn away cancerous tissue. Some laser ablation has the advantage of being able to be performed at the same time as magnetic resonance imaging , allowing very specific targeting and also real-time views of results.

What Is Focal Therapy For Localized Prostate Cancer

Sperling Prostate Center, 4205 W Atlantic Ave D, Delray Beach, FL 33445 ...

Focal therapy is a treatment for prostate cancer of low to intermediate aggressiveness that is completely contained within the prostate.

For many years there have been two main approaches to managing localized prostate cancer. The first is active surveillance or watchful waiting. In cases of older men or low-risk patients, regular medical tests are conducted to make sure the cancer isnt spreading. No other action is taken because the cancer is stable or growing so slowly it wont cause problems right away. If tests show the cancer is spreading, treatments can begin. Of those men assigned to active surveillance, about one-third will eventually need to have surgery or radiation.

The second approach to treating localized prostate cancer has been the use of radiation or surgery. This is effective for removing any tumors present but is typically associated with side effects in the urinary, sexual, and bowel function. Two common side effects include loss of control over urinary functions and erectile dysfunction

Clinical Trial In Progress For Mri

The Sperling Medical Group is pleased to be participating in a Phase 2 & 3 clinical trial titled Focal ExAblate MR-Guided Focused Ultrasound Treatment for Management of Organ-Confined Intermediate Risk Prostate Cancer.

MRgFUS is the integration of two groundbreaking technologies into a precise, noninvasive treatment that destroys the tumor while sparing healthy structures. Real time MRI provides clear visualization of the anatomy and tumor with thermometry to monitor precision and effectiveness while focused ultrasound is used to ablate the tumorno surgery, no radiation.

The anticipated benefits of this treatment are:

  • Low complication rate
  • Non-invasive treatment, patients back to their life within a day or two
  • Non-ionized radiation

Sperling Prostate Center Joins Mr

DELRAY BEACH, Fla., May 2, 2017 /PRNewswire-USNewswire/ — The Sperling Prostate Center is pleased to announce its participation as a new study site for a clinical study of MR-guided focused ultrasound for the focal treatment of localized low and intermediate risk prostate lesions. They are now recruiting patients.

The Sperling Prostate Center is a diagnosis and treatment center with locations in Florida and New York City. The Center is headed by Dan Sperling, M.D., D.A.B.R., a renowned expert in the use of Magnetic Resonance Imaging technology for the diagnosis and treatment of prostate lesions.

“We are excited to join this study to evaluate MR-guided focused ultrasound as a non-invasive and non-radiation treatment option for prostate cancer patients who may currently be on watchful waiting or active surveillance,” states Dr. Sperling.

The multi-center FDA-approved clinical study is sponsored by INSIGHTEC, the developer and manufacturer of the Exablate Prostate system.

The Exablate Prostate system is based on INSIGHTEC’s proven MRI-guided focused ultrasound technology which received its CE mark in December 2016. It uses focused ultrasound waves to precisely ablate the identified prostate lesion. The treatment is done under MRI for visualization of the patient’s anatomy as well as real-time temperature monitoring. The treatment does not require incisions and is performed in a single session, allowing patients to quickly return to normal activity.

The Sperling Medical Group – Putting Patient Care First

Symptomatic treatment of an enlarged prostate usually involves a combination of medication and lifestyle changes. A diet rich in fruits and vegetables may be the best option if you suffer from chronic urination. It will help the body adjust to the increased size of the prostate. Also, taking regular urination intervals will help retrain the bladder to function properly. Inactivity also contributes to urine retention, and cold temperatures can increase the urge to urinate.

Invasive treatment of enlarged prostate includes medication that relieves the pressure on the urethra and bladder. However, if the condition is severe, it may require surgical intervention. If treatment is not successful, the enlarged prostate can become a potentially life-threatening disease. As the hormone levels in the body change, the enlarged prostate can lead to various complications, including urinary retention and even cancer. This is why it is critical to see a doctor for further evaluation.

Welcome To Sperling Prostate Center

Our advanced diagnosis and treatment systems in Florida offer effectiveness and quality of life unattainable with conventional methods. Our patients experience minimal side effects, low recurrence rates, and virtually no risk of incontinence or impotence.

Our MRI-guided detection methods utilize the worlds most advanced 3T MRI system along with multiparametric software to create a vivid, 3D prostate map revealing a 360° view of the gland and any irregularities. This technology allows us to perform more accurate biopsies with up to 80% fewer needle samples than conventional biopsies.

Patients also have access to epigenetic screening that detects DNA changes associated with prostate cancer, making the biopsy more effective and conclusive.

If a tumor is detected, our prostate laser center offers a revolutionary focal laser therapy for prostate cancer that dramatically reduces the risk of impotence and incontinence associated with conventional treatments. This minimally-invasive outpatient procedure eradicates tumors in a single one hour treatment, leaving patients ready for everyday life by the next day. Focal laser therapy also offers a superior therapy for benign prostatic hyperplasia or BPH.

Who Are Good Candidates For Focal Therapy

When focal therapy for prostate cancer first began to be used in about 2007, it was only as an alternative to active surveillance in very low-risk patients. Since then, the use of focal therapy has expanded to include those with favorable intermediate risk disease . The best candidates have a prostate gland of about 40cc in size or a tumor in the lower half of a larger gland, a single visible tumor on MRI, and the absence of cancer elsewhere in the gland .

In each case, the doctor will consider the patients general health and mental outlook, the size and location of the tumors present, and their chances of spreading more. Use of tests such as MRI, ultrasound, and biopsy can help decide if a patient will benefit more from focal therapy or from traditional treatments.

A Healthcare Actuarys Survey Of 100 Focal Laser Ablation Patients

Background: Until recently, newly diagnosed PCa patients often had an all or nothing choice. Assuming there was no evidence of spread outside the capsule, the patient was told that he could have either: definitive treatment, which includes prostatectomy or some type of radiation or Active Surveillance, which entails monitoring the situation indefinitely. For men with more aggressive PCa, the Option 1 would be suggested, perhaps in some combination with Androgen Deprivation Therapy. For patients who appear to have low risk factors including Gleason Score 6, Option 2 Active Surveillance might be considered.

What about men with low-to-intermediate risk? The Prostate Cancer Intervention Versus Observation Trial has showed no statistically significant improvement in longevity, based on 12 years follow-up. “The chance of recurrence is significant according to the meta-analysis of 31,000+ prostate studies performed by the Prostate Cancer Results Study Group, chaired by Peter Grimm, DO, Seattle, WA.And there is almost certain reduction in Quality of Life due to risks of impotence, wearing pads and other side effects and complications. Clearly, Option1 is over-treatment for these men.

But what about Option 2? The Urologist is often not supportive, and there is fear that AS could lead to progression and metastasis. So, Option 2 is viewed by many as Under-treatment.

2) Patients are well educated, higher socio-economic group, and have low-to-intermediate risk

Patients treated with focal therapy still need to be actively monitored afterwards. This includes watching for disease that may have gone undetected, has returned, is spreading, or is appearing for the first time. If follow-up tests show that some of the treated cancer still remains, additional ablation, surgery, or radiation may be needed.

What Is Focal Laser Ablation

Patient Dr. Michael Bedecs tells his story

Laser ablation generates intense heat that completely encompasses the targeted area. Under real-time MRI guidance, a special optical fiber is guided precisely into place at the core of the tumor. When activated, the laser emitted at the tip of the fiber destroys the tumor within minutes while special tracking called thermometry confirms the proper temperature. Afterward, multi-parametric MRI scans reveal that the destruction is complete, and the laser fiber is removed.

Surgical procedures to remove the diseased prostate are usually necessary. Surgical procedures are not always necessary. If the disease is caused by bacterial infections, a doctor can treat the symptoms using alpha-blockers or surgery. Physical therapy, relaxation exercises, and warm baths are all recommended. A physician may also prescribe antibiotics to cure the infection. A bacterial infection can also cause a recurrence of the condition.

An enlarged prostate can be uncomfortable for both men and women. Some of the symptoms of an enlarged male reproductive organ include a weakened urine stream, urgent need to urinate, and urinary tract infections. BPH can also cause damage to the kidneys. A sudden inability to urinate can be life-threatening, as it can lead to bladder and kidney damage. Unfortunately, most men with enlarged prostrates put up with the symptoms for years before they seek treatment. However, many of the men with symptoms finally decide to go to a doctor for proper gynecological evaluation and to begin enlarged prostatic therapy.
