The prostate gland produces a protein called PSA. Levels of this protein increase when the prostate is dealing with irritation, swelling, or the growth of cancerous cells.

High levels of PSA in the blood do not necessarily mean that an individual has prostate cancer, but they can suggest that further tests are necessary.

The Prostate Cancer Research Institute recommend that most men start having PSA tests at the age of 45.

Prostate cancer that returns after treatment is considered recurrent. When it returns to the area around the prostate, the disease is classified as a local recurrence. If the cancer is found in another part of the body, the recurrent cancer is considered metastatic. If the cancer metastasizes outside the prostate, it most likely develops in bones first. Metastatic prostate cancer most often spreads to the liver, bones and lungs.

After initial treatment for prostate cancer, PSA levels are expected to drop dramatically. The first sign of recurrent prostate cancer may be a rise in the PSA level. Other symptoms of recurrent cancer may depend on whether and where the cancer has spread. Symptoms include:

    • Blood in the urine
    • Difficulty breathing
    • Jaundice

Patients should discuss any symptoms with their doctor and ask about scheduling regular PSA tests after treatment.

Prostate cancer has been shown to metastasize by following the venous drainage system through the lower paravertebral plexus, or Batson’s plexus.4,9 Although hematogenous spread of other malignancies is most commonly to the lungs and liver, 90 percent of prostatic metastases involve the spine, with the lumbar spine affected three times more often than the cervical spine. Prostate cancer also spreads to the lungs in about 50 percent of patients with metastatic disease, and to the liver in about 25 percent of those with metastases.4

Epidural metastases are the result of contiguous spread from lesions of the calvaria to the meninges. Because of the protective layer of the dura mater, subdural and intra-parenchymal metastases from prostate cancer are rare .

Prostate pain is often described as discomfort or pain that lies deep to the penis and scrotum although most patients will report that it is not extending to the rectum. The character of the pain can vary significantly with some patients reporting a mild discomfort like the bladder is not completely empty pressure or fullness but not actual pain.

A doctors first step in finding out the cause of back pain typically is to take an image, usually an X-ray or CT scan.

However, if a patient has more advanced prostate cancer, we have to do additional tests to ensure theres no involvement of the cancer in the bone.

A doctor who suspects or already has diagnosed prostate cancer will look for characteristic changes in the bone. The X-ray or CT scan also can indicate how much of your spine is affected and where.

In addition, an MRI can detect problems that an X-ray or CT scan cant.

Chiropractors are often the ones who first spot or suggest the presence of prostate cancer. Joint pain, especially back pain, often sends people to chiropractic care when they dont have any other symptoms of prostate cancer.

Whether youre seeing a chiropractor or medical doctor, be sure to provide your complete medical history. This can help your doctor diagnose your back pain. Its especially important to mention any personal or family history of cancer.
