Mayo Clinic Minute: Steam Treatment For Enlarged Prostate

Prostate gland enlargement can cause uncomfortable urinary symptoms for men. And, as they age, their risk increases. Approximately 30 percent of men experience symptoms by age 60 and 50 percent of men by age 80.

There are several effective treatments for an enlarged prostate, including a relatively new treatment that uses steam to reduce the size of the prostate and alleviate symptoms.

It’s a common problem for older men benign prostatic hyperplasia, or BPH.

“BPH is a fancy way of saying the prostate is getting larger, and we don’t want it to,” says Dr. Toby Kohler, a Mayo Clinic urologist.

Dr. Kohler says the enlarged prostate forces the urethra to narrow, causing a variety of urination problems. And, as men age, the symptoms occur more frequently. Treatment for BPH has long been medications and procedures, such as lasers or an electric loop, which burn the prostate from the inside out. But now a relatively new convective water therapy treatment uses steam to make the prostate smaller.

“For nine seconds, a steam ball is produced, and that kills all that prostate tissue that we don’t want or that has grown out of control,” says Dr. Kohler.

He says the procedure, performed right in the doctor’s office, has a low risk for complications or sexual side effects.

Why Should Steam Be The Preferred Treatment

In the Rezm Therapy procedure, steam delivers convective energy. RF energy is applied to water, and the water carries energy until it phase-shifts into steam. When the steam is delivered to the tissue, it condenses back into water delivering the stored energy convectively. This is an efficient and uniform way to transfer therapeutic energy inside the body because the vapor is convectively dispersed through the interstitial space in the prostate tissue, and the energy is delivered in a matter of seconds rather than minutes.2

Steam, which is delivered to the prostate at a temperature of about 103°C, does not penetrate tissue. Instead, steam travels through cell interstices until it meets a boundary, such as a cell membrane . On reaching the cell membrane, steam condenses to water releasing the stored energy, bringing the tissue temperature to 70°C-80°C, which leads to immediate cell death. Due to the convective nature of steam, the transfer of energy is uniform, which enhances the predictability of the procedure.8,9 Upon cell death, the bodys natural processes come into play. Inflammation occurs and the bodys inflammatory system sends out scavengers to the area of edema in the prostate.11 The body resorbs the necrotic tissue over time and the prostate volume is reduced.12 Rezm ablates tissue, like common surgical procedures, leading to a reduction in overall prostate size.

Literature Search Methodology For Data Sources

To identify relevant citations, the PubMed database was searched for references in the English language from 2001 to June 2017. The search strategy included the Medical Subject Headings for the following: Prostatic Hyperplasia AND Benign or BPH, and each of the selected therapies . The search included primary research studies or case series published in peer-reviewed journals. Reviews and reports of single cases were excluded. Of the potentially relevant citations, 40 articles were selected and used to source data for this assessment.

The effectiveness of treatment was evaluated with the International Prostate Symptom Score , a validated questionnaire for evaluating LUTS associated with BPH. The IPSS questionnaire is self-administered by the patient, comprising seven symptom questions and a quality-of-life item. Scores range from 0 to 35 a score of 819 indicates moderate LUTS, and 2035 severe LUTS. The instrument is used to screen for, rapidly diagnose, and track the symptoms of, and suggest management of the LUTS due to BPH. The IPSS questionnaire is a standard instrument to assess treatment outcomes relative to baseline in most clinical trials.

Up to 10,000 men with an enlarged prostate could receive a new treatment which improves their quality of life and preserves their sexual function, under new guidance from NICE.

NICE has recommended the use of Rezum, a minimally invasive procedure that involves injecting steam to destroy excess prostate tissue.

Some current treatments use heat to destroy excess prostate tissue and this can have considerable side effects.

Benign prostatic hyperplasia is the medical term used to describe a noncancerous enlargement of the prostate. Symptoms like needing to pass urine more frequently, trouble starting to urinate and loss of bladder control affect more than a third of men over the age of 50.

Clinical evidence analysed by NICEs medical technology advisory committee shows that using the Rezum procedure relieves lower urinary tract symptoms caused by BPH in men with moderate to severe symptoms who have a moderately enlarged prostate.

Evidence also shows that using Rezum is associated with improved quality of life and a low risk of sexual dysfunction.

Rezum is cost saving compared with standard treatments such as transurethral resection of the prostate and holmium laser enucleation of the prostate by more than £550 per person over 4 years.

Steam Therapy For Bph

AquaBeam a newer therapy using steam could be used to treat swollen prostates.

The therapy uses a tube to apply steam to the prostate glands. The steam can shrink up the glands by two-thirds. In addition, this approach has fewer side effects and takes less than 15 minutes.

This procedure can allow patients to go home the same day with very little downtime in contrast to the cauterization technique, which requires hospitalization for at least 3 days.

Prof Hashim Ahmed who is behind the ingenious technique said: I wanted to remove some harm we cause with treatment. Its less invasive and has fewer side effects. It could improve thousands of lives. The surgeon, of Londons Imperial College NHS Trust, hopes it will be widely used.

Can Rezm Therapy Cause Erectile Dysfunction

There are no reports of new erectile dysfunction as a result of Rezm treatment. Based on data from the Rezm pivotal study, US FDA approved clinical study, ejaculatory function was preserved however, men with BPH are at higher risk for other potential urinary/ejaculatory challenges and there is a possibility that an underlying condition may surface following treatment with the Rezm System.3 Be sure to talk with your urologist about your individual health situation.
