Experience Still Matters With Robotic

A surgeon may have to perform well over a thousand robotic-assisted prostate cancer surgeries before becoming fully proficient at the procedure. Previous studies of robotic-assisted prostate surgery have suggested that it doesnt take long for surgeons to learn how to perform the procedure safely.

To assess the number of robotic-assisted prostate surgeries that a surgeon must perform before becoming fully proficient, researchers assessed 3,794 patients who were treated by one of three surgeons.

A key outcome that was considered in the study was the rate of positive surgical margins. A positive surgical margin means that cancer is found at the edge of the tissue that was removed during surgery it may be a sign that not all of the cancer was removed.

  • The rate of positive surgical margins declined as surgeons gained experience. The lowest rates of positive surgical margins came after a surgeon had performed more than 1,600 robotic-assisted prostate surgeries.6

    Radical Prostatectomy Survival Rates

Men who undergo radical prostatectomy have a high survival rate and low rates of cancer recurrence, cancer spread and death, according to a study of 10,332 men who had surgery between 1987 and 2004. The research showed that between 5 and 20 years after having the surgery, only 3% of the patients died of prostate cancer, 5% saw their cancer spread to other organs, and 6% had a localised recurrence.

Another large Scandinavian study compared men who chose active surveillance with those treated by radical prostatectomy. The results suggest that in the long term, younger men with higher-risk tumours who have a radical prostatectomy, have a definite survival advantage.

Results from another study at Johns Hopkins Hospital in Baltimore confirmed that 82% of men undergoing radical prostatectomy were free of recurrence at 15 years. The data from the research also indicated that in those men whose PSA level starts to rise again after surgery, the recurrent prostate cancer spreads in only around one-third of the men. In addition, unless a man had an aggressive grade of prostate cancer, the spreading of the disease would not become life-threatening for several years and would be amenable to treatment.

Some Side Effects Of Radical Prostatectomy Reduced If Surgeon Performs Many Procedures

New Prostate Cancer Scan âIs Going To Be Life-Saving,â Doctor Says

The risk of some side effects caused by a radical prostatectomy appear to be reduced if a patient is treated at a high-volume hospital and by a surgeon who performs a high volume of procedures annually

Recently, investigators from the Health Outcomes Research Group, Memorial Sloan-Kettering Cancer Center and the Applied Research Branch of the National Cancer Institute evaluated data from the Surveillance, Epidemiology, and End Results -Medicare database to evaluate the incidence of postoperative complications and death in relation to the volume of patients treated at a hospital or the volume of procedures performed by a surgeon. Four separate categories were evaluated: postoperative complications, including life-threatening events during the 30 days after surgery involving cardiac, respiratory, vascular, renal or bleeding events, shock or the need for reoperation late urinary complications including bladder obstructions, fistulas, strictures, pelvic abscess or early incontinence that occurred 31 to 365 days after surgery postoperative death and long-term incontinence lasting over one year following surgery.

This data appears to indicate that patients undergoing a radical prostatectomy may have a reduced risk for developing some side effects if treated in a high-volume hospital and by a surgeon who performs a large number of prostatectomies annually.

Alan Doherty Is Chosen As Uk Top Ten Prostate Cancer Surgeon In Daily Mail Vote By Fellow Urologists

    Jonathan Haas, MD: Mineola, NY

BPC urologist Alan Doherty has been named as one of the countrys top ten prostate cancer surgeons by a Daily Mail poll of urologists.

It is the second time that Mr Doherty has appeared in the Daily Mails top ten prostate cancer surgeons, having also been chosen in the first survey in 2010.

Only four specialists have made the top ten twice and Mr Doherty is the only prostate cancer surgeon in the whole Midland region to make the list.

The Daily Mails Good Doctor Guide is based on the principle that it is doctors themselves who are best placed to know the best specialists within their field. Consultant urologists were asked: If your own nearest and dearest were to need treatment, to whom would you refer them and why?

Prostate cancer surgeons with the most nominations are placed in the top ten in the Good Doctors Guide, while others with significant votes but outside the top ten are also noted.

Urologists described why they had chosen Mr Doherty. One described Mr Doherty as: A surgeon who does huge numbers of prostatectomies, including nerve-sparing, and is immensely skilled and kind. Frankly, the tool be it robot of scalpel doesnt matter, the outcomes do and his are very good.

Another urologist said: He gathers his data meticulously, the hallmark of the best surgeons.

The guide gives a comprehensive consideration of what makes a good doctor and the things a patient should ask when considering where to go for treatment.

Is Robotic Assisted Minimally Invasive Safer

Minimally-invasive radical prostatectomy proves safer than traditional open surgeryresulting in fewer post-surgical complications, fewer blood transfusions, and shorter hospital stays, according to the results of a study published in European Urology.

Prostatectomy may be performed using traditional open surgery, in which the surgeon makes a single, long incision, or through a laparoscopic procedure , in which several small incisions are made. During laparoscopy, the surgeon inserts a small video camera through one of the incisions in order to see inside the abdomen. In a variant of laparoscopic surgery known as robotic-assisted laparoscopic surgery, the surgeon sits at a console near the operating table and performs the surgery by controlling robotic arms that hold the surgical instruments.

This study evaluated data from 78,232 men over age 65 who underwent prostate removal between 2003 and 2007. What makes this study unique, however, is that it included the data from 100% of Medicare patients in the United Statesresulting in a comprehensive body of data that reflects results from both large and small communities from every region of the country. The large sample size enabled researchers to detect statistically significant differences in outcomes between the two methods that smaller studies might not have captured.

Men with prostate cancer are encouraged to speak with their physician to safely evaluate the risks and benefits of the different surgical procedures.
