Prostate hyperplasia, also known as benign prostate hyperplasia , is a fairly common problem for men. It can lead to urinary obstruction, often considered a medical emergency, if severe enough. Biopsy proven BPH increases from 8 percent in men aged 31-40, to 40-50 percent in men aged 51 to 60, to over 80 percent in men older than age 80. Benign prostatic hyperplasia is a result of urogenital aging. Recent studies suggest that an age-related impairment of the blood supply to the lower urinary tract plays a role in the development of BPH and thus may be a contributing factor in the development of BPH.

Medications may slightly reduce prostate size and improve symptoms but have less benefit as the prostate gets larger. A definitive solution for BPH is surgical, or other invasive interventions, to reduce the size of the prostate and improve the flow of urine. BPH has been treated with various types of destructive high energy electromagnetic radiation methods such as transurethral needle ablation , interstitial laser therapy , and holmium laser resection . Unfortunately, these therapies often lead to major complications, including urinary incontinence and impotence. Your MRI exam may use anendorectal coil, a thin wire covered with a latex balloon. The doctor places the coil a short distance into the rectum. The rectum is located immediately behind and up against the prostate. Placing the coil into the rectum so close to the prostate helps generate more detailed images. It also enables yourradiologist to perform magnetic resonance spectroscopy. MR spectroscopy can provide additional information on the chemical makeup of cells in the prostate. Additionally, prostate MRI may examine water molecule motion and blood flow within the prostate to help differentiate between diseased and normal prostate tissue.

Your doctor will typically use an endorectal coil with low-field MRI magnets if you have a metal orthopedic implant. Metal implants may interfere with imaging when using a high-field magnet MRI. The doctor will typically not use an endorectal coil with high-field MRI units.

You will need to change into a hospital gown. This is to prevent artifacts appearing on the final images and to comply with safety regulations related to the strong magnetic field.

Guidelines about eating and drinking before an MRI vary between specific exams and facilities. Take food and medications as usual unless your doctor tells you otherwise.

If you have claustrophobia or anxiety, ask your doctor to prescribe a mild sedative prior to the date of your exam.

Are There Side Effects Of The Combination Approach

There is a slightly higher chance that patients who receive the combined therapy will have rectal irritation or urinary side effects. This is common with prostate cancer radiation therapy because the radiation can damage cells in the tissues surrounding the prostate. But at MSK, we routinely use sophisticated computer-based planning techniques that help us reduce the dose given to normal tissues such as the rectum, bladder, and urethra, lessening the chances of side effects and complications. We have also found that, when treating with the combined approach, using the high-dose-rate brachytherapy compared to low-dose-rate brachytherapy may have less in the way of side effects.

In addition, at MSK, we routinely use a rectal spacer gel, which we inject between the prostate and the rectum while the patient is under mild anesthesia, to create a buffer between these two tissues. By creating this space, we can further reduce the dose of radiation the rectum is exposed to. This leads to fewer side effects for the patient. The rectal spacer gel is biodegradable and dissolves on its own within the body after a few months.

Proton Beam Radiation Therapy

Proton beam therapy focuses beams of protons instead of x-rays on the cancer. Unlike x-rays, which release energy both before and after they hit their target, protons cause little damage to tissues they pass through and release their energy only after traveling a certain distance. This means that proton beam radiation can, in theory, deliver more radiation to the prostate while doing less damage to nearby normal tissues. Proton beam radiation can be aimed with techniques similar to 3D-CRT and IMRT.

Although in theory proton beam therapy might be more effective than using x-rays, so far studies have not shown if this is true. Right now, proton beam therapy is not widely available. The machines needed to make protons are very expensive, and they arent available in many centers in the United States. Proton beam radiation might not be covered by all insurance companies at this time.

What Do I Need To Do To Get Ready For My Imrt Treatment

Getting ready for IMRT takes a great deal of planning. It can take two to three days for your radiation therapy team to plan your care. After giving you a physical exam and reviewing your medical history, your radiation oncologist will use different tools to plan your treatment. These tools will help your radiation oncologist decide the dose of radiation you need for your prostate cancer and how long your treatment will last.

  • PET Scan is a test where you are given contrast before you have x-rays taken. The contrast will travel to the parts of your body where your prostate cancer is active. The x-ray will give your doctor a better picture of what your prostate cancer looks like.

The stage of the cancer is based on the results of the staging and diagnostic tests, including the prostate-specific antigen test and the Grade Group. The tissue samples removed during the biopsy are used to find out the Gleason score. The Gleason score ranges from 2 to 10 and describes how different the cancer cells look from normal cells under a microscope and how likely it is that the tumor will spread. The lower the number, the more cancer cells look like normal cells and are likely to grow and spread slowly.

UI researchers investigate the use of magnetic rods to treat prostate cancer

IOWA CITY, Iowa — University of Iowa researchers are developing a new approach to treat prostate cancer. The treatment uses heat generated by implanted magnetic rods to destroy the cancer. The UI scientists hope the new technique will be as successful as surgery and radiation therapy in treating the disease, but will avoid the difficult and unpleasant side effects often associated with those standard treatments.

“Our results, and those of our international collaborators, suggest that these rods could be extremely effective in treating the cancer with potentially fewer side effects,” said Robert D. Tucker, M.D., Ph.D., UI associate professor of pathology and adjunct associate professor of biomedical engineering. “And we think that this approach could also prove useful against other localized tumors.”

Last year more than 180,000 men in the United States were diagnosed with prostate cancer. When the cancer is confined to the prostate gland, surgical removal of the prostate or radiation therapy are the two most commonly recommended treatment options. Although both these methods offer good odds for success, they each entail risks of damage to the tissue around the prostate, which in turn can cause incontinence and impotence.

What Will I Feel During This Procedure

Before treatment begins, the doctor may place an intravenous line into your arm or hand to deliver medication. Depending on the site of the tumor and your doctors recommendations, you may receive general anesthesia and/or a sedative to make you feel sleepy. If anesthesia or heavy sedation is used, you will be moved to a recovery room afterwards. Depending on the type of treatment, you may return home the same day or be moved to a hospital room.

Patients who have an afterloaded implant for temporary brachytherapy may hear a clicking or humming noise from the treatment machine. This occurs as the machine pushes the radioactive material into the treatment device previously placed at the tumor site. You will be alone during the procedure. However, you will be able to speak with your treatment team via a microphone. The team will be located nearby, where they can see and hear you. PDR treatment will allow you to have visitors between the treatment pulses. LDR treatment will require you to stay in the hospital. Visits are usually limited to about 30 minutes per visitor per day. Children and pregnant women may not visit. No radiation remains in your body after temporary brachytherapy, so there is no risk to others.

Some LDR, PDR and HDR treatments require one or more days in the hospital and may cause discomfort. This may be due to the catheters or applicators and having to stay relatively still for a prolonged period. Your doctor will prescribe medication to relieve your pain.
