The most common prostate problem in men younger than age 50 is inflammation, called prostatitis. Prostate enlargement, or benign prostatic hyperplasia , is another common problem. Because the prostate continues to grow as a man ages, BPH is the most common prostate problem for men older than age 50. Older men are at risk for prostate cancer as well, but it is much less common than BPH.

Lymph Node Biopsy As A Separate Procedure

A lymph node biopsy is rarely done as a separate procedure. Its sometimes used when a radical prostatectomy isnt planned , but when its still important to know if the lymph nodes contain cancer.

Most often, this is done as a needle biopsy. To do this, the doctor uses an image to guide a long, hollow needle through the skin in the lower abdomen and into an enlarged node. The skin is numbed with local anesthesia before the needle is inserted to take a small tissue sample. The sample is then sent to the lab and looked at for cancer cells.

A prostate infection diagnosis is based on your medical history, a physical exam, and medical tests. Your doctor can also rule out other serious conditions such as prostate cancer during the exam. During a physical exam, your doctor will conduct a digital rectal exam to test your prostate and will look for:

  • enlarged or tender lymph nodes in the groin
  • swollen or tender scrotum

Your doctor may also ask about your symptoms, recent UTIs, and medications or supplements youre taking. Other medical tests that can help your diagnosis and treatment plan include:

  • urinalysis or , to look for infections
  • a prostate biopsy or a blood test for prostate-specific antigen
  • urodynamic tests, to see how your bladder and urethra store urine
  • cystoscopy, to look inside the urethra and bladder for blockage

Your doctor may also order an ultrasound to get a closer look. The cause will help determine the correct course of treatment.

How Is Acute Prostatitis Diagnosed

Your doctor will likely start by asking questions about your medical history. Theyll also conduct a physical examination.

Theyll probably conduct a digital rectal examination . During this procedure, theyll gently insert a gloved and lubricated finger into your rectum. Your prostate is located in front of your rectum, where your doctor can easily feel it. If you have acute bacterial prostatitis, it will likely be swollen and tender.

How Are Bacterial Forms Of Prostatitis Managed Or Treated

Antibiotics can kill bacteria that cause bacterial types of prostatitis. Men with acute bacterial prostatitis may need 14 to 30 days of antibiotics, starting with IV antibiotics in the hospital. Rarely, men need surgery to drain an abscess on the prostate.

Treating chronic bacterial prostatitis is challenging. You may need up to three months of antibiotics to sterilize the prostate. If the prostate cant be sterilized, low-dose antibiotics can be used long term to prevent recurrences. Some men need surgery to remove prostate stones or scar tissue in the urethra. Rarely, surgeons remove part or all of the prostate gland .

How Can You Get Tested For Prostate Cancer And Prostatitis

Digital rectal exam . For both conditions, your doctor may perform this exam. Theyâll use a glove and lubricant to insert their finger into your rectum. Your doctor can examine your prostate this way. If they notice any abnormal features in the texture, shape, or size of your prostate, they might suggest more tests.

Blood tests. They also may do blood tests for either prostate cancer or prostatitis. These measure prostate specific antigen , a natural substance that your prostate makes. High levels might mean that you have prostatitis, prostate cancer, or benign prostatic hyperplasia .

Urinalysis. This test checks your pee for bacteria and UTIs.

Cystoscopy. A cystoscopy looks for other urinary tract issues. It canât diagnose prostatitis, but it can help find what other things may cause your symptoms. Your doctor will use a cystoscope to look inside your urethra and bladder.

Ultrasonography And Computed Tomography

Transabdominal ultrasonography or a bladder scan is used to assess for volume of retained urine in cases of prostatitis associated with significant voiding dysfunction.

On transrectal ultrasonography, characteristic features of prostatitis are capsular thickening and prostatic calculi. A hypoechoic halo in the periurethral region, a heterogeneous echo pattern, and enlargement and thickening of the septa of the seminal vesicles may be seen.

Interpretation of transrectal ultrasound is highly subjective and therefore not very reliable for the diagnosis of prostatitis. This study is not routinely indicated in patients with prostatitis, except when prostatic abscess is suspected.

Computed tomography studies of the pelvis also may be useful in the evaluation of prostatic abscess or suspected neoplasm. Cystoscopy is useful in refractory cases with significant voiding dysfunction symptoms to rule out neoplasm of the bladder or interstitial cystitis. Voiding cystourethrography or retrograde urethrography may be appropriate for evaluation of the bladder neck anatomy and penile and anterior urethra in cases of suspected bladder neck dyssynergia or urethral stricture.

    What Are Additional Tests For Detecting Prostate Problems

    If the DRE or the PSA blood test indicates a problem may exist, the health care provider may order additional tests, including urinalysis, urodynamic tests, cystoscopy, abdominal ultrasound, transrectal ultrasound with prostate biopsy, and imaging studies such as magnetic resonance imaging or computerized tomography scan.

    The symptoms of both the bacterial form of chronic prostatitis and CP/CPPS are very similar. They usually start out mild and build in intensity over time.

    The cause of prostatitis is not always known. You can raise your risk of getting it with a bacterial infection or bladder infection, a catheter, an infection from sex, or a problem in the urinary tract. Your doctor may have to do several tests to find out exactly why you feel pain. But it is important to find out what may have caused it.

    Diagnostic tests for prostatitis are to get at minimum a dipstick of urine, but usually we would actually get a midstream urine. And we would assess the urine under the microscope.

    What we are looking for are for prostatitis are white blood cells, which are a market of inflammation. We look at red blood cells also. This is a separate condition altogether called hematuria. But of course, were looking for the growth of bacteria as well when we culture the urine. And if we can find that, then well that confirms that its an infective agent which then needs to be treated with antibiotics.
