    Androgen deprivation therapy is a powerful tool against prostate cancer, and more and more men are opting for the treatment as a growing array of hormone-based therapies become available.

    But it comes with a cost. Suppressing male hormones, including testosterone, that fuel cancer growth also means that patients lose strength and muscle mass and gain fat. And that puts the men at risk for other health problems, including heart disease and diabetes.

    But diet and moderate exercise proved to be valuable tools in fending off those side effects in the new research from The Ohio State University. The study appears in the journal Annals of Behavioral Medicine.

    âWe found that a comprehensive exercise and diet program in a group setting can make a difference for prostate cancer patients, and the difference was greater than I expected in a short period of time,â said lead author Brian Focht, a professor of human sciences at Ohio State.

    The National Cancer Institute supported the study.

    Will I Have Other Treatments In Addition To Hormone Therapy

    Your RMCC oncologist may recommend using hormone therapy in combination with other cancer treatments, such as chemotherapy and radiation therapy. When used with other treatments, hormone therapy can:

    Six Months Hormone Therapy Plus Radiation Therapy Improves Prostate Cancer Survival

    Men with prostate cancer that is small and confined to the prostate gland but at risk of growing and spreading, do better if they are treated with radiotherapy combined with androgen deprivation therapy , which lowers their testosterone levels. These research findings were presented at the 33rd conference of the European Society for Radiotherapy and Oncology in Vienna, Austria.

    Although we need longer follow-up to assess the impact on these mens overall survival, these findings need to be taken into account in daily clinical practice, said Michel Bolla, MD, a professor of radiation oncology at Grenoble University Hospital in France. They show that three-dimensional conformal radiotherapy, whether intensity-modulated or not, and regardless of the dose level, has to be combined with short-term androgen deprivation therapy in order to obtain a significant decrease in the risk of relapse.

    Bolla explained that these trial results indicate that this combined treatment approach should be an option proposed for men with localized prostate cancer that has an intermediate or high risk of growing and spreading.

    Bolla and colleagues from 37 centers in 14 countries recruited 819 men to a clinical trial. The patients had early stage prostate tumors, as confirmed by analyses of biopsy samples and levels of prostate specific antigen , that were at intermediate or high risk of growing and spreading to other parts of the body.

    Early Versus Delayed Treatment

    For men who need hormone therapy, such as men whose PSA levels are rising after surgery or radiation or men with advanced prostate cancer who dont yet have symptoms, its not always clear when it is best to start hormone treatment. Some doctors think that hormone therapy works better if its started as soon as possible, even if a man feels well and is not having any symptoms. Some studies have shown that hormone treatment may slow the disease down and perhaps even help men live longer.

    But not all doctors agree with this approach. Some are waiting for more evidence of benefit. They feel that because of the side effects of hormone therapy and the chance that the cancer could become resistant to therapy sooner, treatment shouldnt be started until a man has symptoms from the cancer. This issue is being studied.

    What Should Patients Know About Msks Approach To Treating Prostate Cancer

It requires a great deal of collaboration with our medical physics team to try to get the most accurate positioning of the prostate during the actual three or four minutes of the treatment.

    We make adjustments while the patient is still under anesthesia, so that when the procedure is completed, we have been able to achieve ideal placement of the radiation seeds. This translates into improved outcomes.

    Treating Advanced Prostate Cancer

    If the cancer has reached an advanced stage, it’s no longer possible to cure it. But it may be possible to slow its progression, prolong your life and relieve symptoms.

    Good Prostate Cancer Care

    Your MDT will be able to recommend what they feel are the best treatment options, but ultimately the decision is yours.

    You should be able to talk with a named specialist nurse about treatment options and possible side effects to help you make a decision.

    You should also be told about any clinical trials you may be eligible for.

    If you have side effects from treatment, you should be referred to specialist services to help stop or ease these side effects.

    Prostate cancer is the leading cause of cancer in men worldwide, and radiotherapy is one of the common forms of treatment.     Hormonal Therapy With Radiotherapy

    Hormonal therapy is often used to treat early and locally advanced prostate cancer. If you have low-risk early prostate cancer you do not usually need hormonal therapy.

    When Hormone Therapy Is Indicated

    HT can be administered before, during or after a localized treatment, such as radical prostatectomy, radiation, high-intensity focused ultrasound or cryotherapy. When given before a localized treatment, it is called neoadjuvant therapy. When given after localized treatment without evidence of prostate cancer recurrence, it is called adjuvant therapy. When HT is prescribed after localized treatment for a prostate cancer recurrence, it is called salvage therapy. If a patient’s PSA starts rising after a radical prostatectomy, HT is typically given in combination with radiation therapy. Treatment recommendations are based on each patient’s specific circumstances.

    Sometimes we give neoadjuvant HT while the patient is deciding on his primary treatment or to reduce the tumor’s size before starting primary treatment. Neoadjuvant HT will usually slow or stop cancer growth for a period of time.

    Many radiation oncologists use HT along with radiation treatment in the belief that HT weakens cancer cells so that they’re more susceptible to destruction by the radiation. Clinical studies have suggested a synergy between radiation therapy and hormone therapy meaning they work better together. Clinical trials have shown improved outcomes for patients who receive combined therapy.
