The Dosimetric Study: Patient Selection Contouring And Planning

Since the two platforms involved in the study have an inherently different way of delivering the dose, and a different IGRT system which can affect the anatomical configuration of the organs, to have a truly representative dosimetric study, we compared patients who were imaged and have received CK treatment with patients who were imaged and received treatment with the VMAT by using the Clarity system. Of course, for the VMAT patients, the therapeutic plans were re-computed in accordance with the ultra hypofractionated prescription: 36.25 Gy in five fractions. At this point, it could be argued that the best or worst results achieved with CK or with SBRT-VMAT would not be directly comparable since they could depend on specific patient anatomical characteristics. Therefore, in order to reduce and smooth such patients differences, groups of patients were selected, and dosimetric results are expressed as average values.

For sake of completeness, in our institute as actual clinical practice, the patients selected for SBRT with CK satisfy the following settings: age > 18 years PCa diagnosed with transperineal core biopsy low risk class 10 ng/mL and Gleason Grade = 1) intermediate risk class prostate volume < 80 cc uninvolved prostate capsule ) performance status: 01 TC total body with contrast enhancement bone scintigraphy negative for metastasis .

Sbrt Limits The Field Of Radiation

CyberKnife is a form of stereotactic body radiation therapy , which is a coordinated system to exactly locate the tumor and limit the field of radiation treatment. SBRT delivers a single high dose of radiation or a few fractionized radiation treatments.

There are various types of SBRT, and I believe CyberKnife is the best. CyberKnife utilizes a robotic arm to deliver radiation in different ways and at different angles.

Ever try to slice a grape? It almost seems to squiggle away from the blade, and can result in a poor cut or even a smashed grape. But if you hold that grape against the cutting board between two fingers so it doesnt move, you can slice it precisely the way you want.

Comparing Side Effects Of The Various Treatments

Multiple studies have been published that compare the side effects of the 3 standard treatment options. The best of these studies are Health Related Quality Of Life studies , in which patients answer questionnaires that are validated by expert panels. Multiple HRQOL studies have been performed with the 3 standard treatment options. Unfortunately, none of these studies are randomized, thus patients in the surgical arms are typically younger and healthier before treatment than those in the EBRT or brachytherapy arms. This is because many patients that are not candidates for surgery are still good candidates for EBRT or brachytherapy. So, the average age and health of the surgically treated patient is more favorable than the average age and health EBRT or brachytherapy patient. Multiple studies prove that younger, healthier patients recover from virtually any type of treatment better than older, sicker patients. So, all these studies have a built-in bias favoring the surgically treated patients.

Having performed more than 1,500 CyberKnife prostate cancer treatments, if I didnt know better I would think that this technology was created specifically to treat this disease. It is completely suited to the specific challenges of prostate cancer treatment. Yet CyberKnife was first used mostly as a treatment for head, neck and spine tumors that were very difficult to reach.

The Food and Drug Administration opened the door in 2001 to the use of CyberKnife to treat other cancers in the body. Since then urologists and oncologists are turning more and more to CyberKnife, as are patients.

CyberKnifes primary advantages relate to reduced damage to healthy tissue surrounding the cancerous prostate tissues. This is largely due to CyberKnifes ability to precisely target a tumor in the prostate without radiating healthy tissue. Background: CyberKnife SBRT is capable of producing dosimetry comparable to that created by HDR brachytherapy. Our original CyberKnife prostate SBRT schedule of 3,800 cGy/4 fractions was based upon favorable published prostate HDR brachytherapy experience. Subsequently, our trial was modified to allow a lower dose of 3,400 cGy/5 fractions in selected cases.

Methods: Two hundred eighty-nine low and intermediate-risk patients were treated to either high dose or moderate dose . The dose selection was individualized high dose more commonly used in younger, intermediate-risk patients, and moderate dose more commonly used in older, low-risk patients.

Results: Median PSA reached 5-year nadir levels of 0.034 ng/mL in the high dose, vs. 0.1 ng/mL in the moderate dose groups, respectively , with 62 vs. 44% reaching an ablation PSA nadir by year 5, respectively. Five year biochemical relapse free survival rates measured 98.3% for moderate dose and 94.3% for high dose groups, respectively . Five-year actuarial grade 2 genitourinary toxicity rates measured 11.6 vs. 8.7% for high dose vs. moderate dose groups, respectively, with a far lower incidence of grade 3 GU and grade 2 GI toxicity rates in both groups.
